Deep neck infections.

From January 1967 to August 1978, 65 patients with cervical abscesses were referred to the ENT Clinic of Turku University Hospital. The origin of these deep neck infections was odontogenic in 19, tonsillitis or tonsillectomy in 14, trauma in seven, salivary glands in five and branchiogenic cysts in five and other known causes in three cases. In 12 cases the origin was unknown. The cervical abscesses of odontogenic origin were located mostly in the submandibular space (11/19). The rest of the deep cervical infections were mostly found in the parapharyngeal space (25/46). Etiological factors and treatment of these severe infections are discussed.

Tonsillectomy and immune responses.

Immune response were examined in 10-year-old boys just before and one month after tonsillectomy, and in young male adults tonsillectomized 10 years earlier. All the parameters studied were found to be quite normal.

Myringotomy in the treatment of acute otitis media in children.

The treatment of acute otitis media was studied in 158 children. All children (mean age 4 years) received penicillin orally 80 000--100 000 IU per day for 10 days. Myringotomy was performed on 68 children at the time of the diagnosis. The other 90 children were treated with penicillin and ear drops. The bacteriological findings from the nasopharynx at the time of diagnosis were equivalent in both groups. After 2 weeks, 42% of the children without myringotomy and 71% of the children with myringotomy were cured. The children who were not cured were treated with amoxicillin for 10 days. Four weeks after diagnosis 71% and 90% of the children respectively were cured. The differences between the two groups are significant. The observations indicate that myringotomy clearly accelerates the recovery from acute otitis media.

Vestibular neuronitis; a follow-up study.

Forty-three patients suffering from vestibular neuronitis were examinated during the acute attack and on an average 3.2 years afterwards. In the follow-up examination there was no caloric reactions in 7 patients, lowered in 15 patients and normal symmetric reactions in 21 persons. In the first examination all had normal symmetric hearing, in the follow-up examination there was 5 to 10 db hearing loss in 4.6 and 8 kHz on the diseased side in 24 patients.

[Alcohol and balance disorders (author's transl)].

The optokinetic, horizontal, per- and postrotatoric nystagmus was recorded before the consumption of alcoholic beverages by 20 young test persons as well as 2, 4, and 6 h later. The results showed that the most evident alterations cannot be recorded during the highest blood alcohol concentration but only 6 h after the consumption of alcohol. It was evaluated that the decomposition products of alcohol provoked the vestibular disorders.

Diphenylpyraline (Lergobine) in the treatment of patients suffering from allergic and vasomotor rhinitis.

In a double-blind study, diphenylpyraline (Lergobine) was given to 63 patients whose main symptoms were stuffiness of the nose, increased secretion of mucus, snuffling, sneezing and redness of the eyes. Fifty-seven patients were given placebo for identical symptoms. Diphenylpyraline was found to have a better effect on all the symptoms than placebo. The difference was statistically significant in respect of the discharge of mucus and redness of the eyes, and when the total symptoms were considered as a whole. In atopic patients the better effect of diphenylpyraline was highly significant.

The eustachian tube function in secretory otitis media in children: a follow-up study.

Twenty-three patients suffering from secretory otitis media were examined. The mean age of the group was 6 years. Adenoidectomy and insertion of polyethylene tympanostomy tubes was made. The tubal function was studied by measuring the middle ear pressure on the morning after operation and once a month during the following five months by using the air pressure equilibration technique. In 23 ears (79%) the pressure was considered to be normal and in 10 ears (30%) abnormal at the first measurement. After 5 months the corresponding groups were 26 (79%) and 7 (21%).

The effect of loud noise on the vestibular system.

The effect of a 2 sec 113--123 dB ISO pure tone stimulus on ENG was studied. The material consisted of patients with operated and unoperated otosclerosis, normal ears, patients with sensorineural defects, and cases of chronic otitis media. ENG changes were found in 45% of patients with sensorineural hearing loss, in 35% of operated otosclerotics, in 15% of unoperated otosclerotics, in 15% of patients with chronic media, but in none of the normal-hearing patients. Most of the patients showing ENG changes felt dizzy. These results seem to imply a possible risk to patients with sensorineural hearing loss working in high noise levels.

Surgical and conservative treatment of mandibular fractures. A follow-up study.

During 1970-1973 a total of 57 patients with mandibular fractures were treated at the Ear Department of the University of Turku Central Hospital in conjunction with the Department of Oral Surgery. The average age of patients was 31 years. Below-the-jaw strapping was used as the only means of immobilization in nine cases. Eight of these nine cases involved only a fracture of the condylar process and one was also a corpus mandibulae fracture. Metal wire osteosynthesis was used with 44 patients and osteosynthesis plates were used with two patients. A Guinning slint was prepared in 10 cases and immobilization was augmented with intermaxillary fixation in 40 cases. 36-50 months (average 40 months) after fracture the patients were requested to come to a follow-up examination; 40 patients (70%) came to the examination. In conjunction with the follow-up examination 32 patients (80%) stated that they were satisfied with the results of treatment. Sensation disturbances in the lower lip, clicking in the temporomandibular joint during movement, and occasional tenderness were reasons for dissatisfaction.

[Experiences with polyethylene ventilation tubes in children with recurrent middle ear inflammation (author's transl)].

Middle-ear ventilation tubes (of the Sheehy modification) were placed in 401 ears of 209 children under 16 years of age as treatment for recurrent middle-ear infection. The same procedure was later required in 26% of the cases studied because of further recurrence of middle-ear infection. In four per cent of all patients studied, the ventilating tube failed to cure middle ear disease in spite of controlled concomitant antibacterial therapy. In these cases, an early extrusion of the tube was commonly found. Among all patients, the tube was spontaneously extruded in 76 per cent of the ears, whereas the intubation time was longer than half a year in 53 per cent of the cases. No complications from insertion of the ventilation tubes was seen in the present series of patients. The tympanic membrane was found to be normal in 71 per cent of all cases when follow-up examination was obtained three months after completion in intubation. However, a permanent tympanic membrane perforation was found in five ears (1% of patients). The present study has confirmed the usefulness of middle-ear ventilation tubes, particularly in patients with recurrent middle-ear infections.
