[Evaluation of radon effects on lung cancer development].

In order to evaluate the effect of indoor exposure to radon and thoron on the development of lung cancer in the population of two towns of Sverdlovsk Region, epidemiologic studies were conducted using a multifactarial method of analysis. Both towns, Pervouralsk and Karpinsk, are located within the geological area with the gamma-radiation exposure dose ranging from 5 to 12 mu r/hr, and are characterized by an increased cancer incidence rate--323.1 and 364.6 cases per 100,000 of population, respectively. The mean values of the voluminous indoor activity (VA) of radon in Pervouralsk and Karpinsk were 23 and 75 Bq/m3 (with maximal indices of radon VA being 395 and 739 Bq/m3), equivalent equilibrium concentrations (EEC) of residential radon and thoron were 0.6 and 2.5 Bq/m3 (maximal indices of EEC of thoron being 5 and 13 Bq/m3), respectively. The results of multifactorial analysis of 22 different lung cancer risk factors carried out using the pattern recognition method proved that the contribution of thoron and radon in the development of lung cancer in the population of Pervouralsk and Karpinsk was not significant--0.5 and 0.6%, respectively. The calculations performed in a monofactorial model of risk evaluation BEIR VI gave different results--11-16% and 35-52% for the towns of Pervouralsk and Karpinsk, respectively. The discussion of the results provides arguments for the reliability and adequacy of the application of multifactorial method of radiation risk evaluation as compared to the traditionally applied monofactorial method.

[Genetic markers of individual predisposition to lung cancer].

The investigation concerning the dependence of cancer incidence on genetic HLA-markers and character of dermatoglyphs was carried out. The use of the pattern recognition method for multifactorial estimation of the above markers as well as the elimination of the main non-genetic oncogenic risk factors allowed obtaining the proof of the existence of the genetic predisposition to lung cancer. It was estimated that the character of dermatoglyphic prints is the most adequate marker for the investigated predisposition, and the reliable prediction of the genetic risk of the lung cancer development can be given as based on the joint evaluation of both markers.

[Causes of differences in emergency care utilization in municipalities of Sverdlovskaya Oblast].

Quantitative evaluation of impact of such groups of factors as health care system potential, demographic structure, degree of social economic development and emergency care services potential on the level of emergency care appealability in municipalities of subject of Russian Federation is discussed. Multivariate analysis was applied as research tool. Compound complex of causes impacts development of emergency care appealability. Degree of municipal health care system development is the most important of them. Mathematical forecast of effectiveness of certain managerial options targeting decrease of emergency care appealability is given.

[The dermatoglyphic characteristics of patients with ischemic heart disease].

The dermatoglyphic studies showing the existence of genetic predisposition to the development of coronary heart disease were conducted. Methods based on the mathematical theory of pattern recognition were used for multifactorial analysis of the material. It was established that complex evaluation of 20 dermatoglyphic parameters could be reliable in predicting genetic risk factor in the development of this cardiovascular pathology.

The principles of predicting the individual risk of silicosis and silicotuberculosis.

A series of investigations conducted in different "silicosis-risk" industries using a methodology based on the mathematical theory of pattern recognition has shown that in the given conditions of dust exposure, the probability of contracting pneumoconiosis depends for each individual on a complex influence of many factors, both environmental and intrinsic for the individual. Genetic predisposition was one of the most important factors and while the direction in which a factor influences predisposition was the same in every industry, its relative contribution to predisposition to simple silicosis was different in the studied working populations. So a reliable prediction of the high probability of this form of pneumoconiosis on the basis of such a multifactorial analysis is possible only with respect to specific conditions of a particular industry. The complex of factors determining predisposition to silicotuberculosis is more general: this complex comprises both factors influencing susceptibility to silica dust and specially those influencing susceptibility to tuberculosis. Despite the low prevalence of genetic resistance to pneumoconiosis in working populations, the risk of contracting the disease in modern industrial conditions of relatively low dust exposure is high only for a proportion of workers for whom the genotype of predisposition to silicosis or to silicotuberculosis coincides with a most unfavourable combination of non-genetic factors enhancing this predisposition. In the opinion of the authors, the task of screening off those applicants for a "silicosis-risk" employment for whom the risk may be estimated as high on the basis of the developed methodology, is quite feasible.

Risk factors for asbestosis in workers of asbestos mills.

The analysis was based on a set of fifteen factors, characterising: the value of exposure to dust hazard, social status of the subjects involved, presence of diseases in the anamnesis, habits, etc., with the aim of establishing their effect on the possibility of developing asbestosis among the female workers of asbestos mills. Mathematical methods of pattern recognition were used for this multifactorial analysis. The decisive rule, obtained on the "training" principle, helped to differentiate the observations made in a reference group with the following accuracy: "without asbestosis": 87.5% correct answers, and "with asbestosis": 88.9% correct answers, which confirms the possibility of predicting the individual risk of developing asbestosis by analysing the set of risk factors. The use of the improved dermatoglyphic technique contributed to demonstrating the significance of the genotype in producing a predisposition towards asbestosis. The experience acquired demonstrates the possibility of medico-occupational selection for work involving asbestos exposure with simultaneous improvement of dust abatement measures at the factory.

[Use of complex assessment of dermatoglyphic signs for prediction of the risk of development of occupational diseases].

Dermatoglyphic investigations conducted by the authors made it possible to demonstrate a genetic predisposition to occupational diseases, in particular, to silicosis and silicotuberculosis. Methods based on the mathematical theory of pattern recognition were used for multifactor analysis of the data. It was shown that complex evaluation of 10 dermatoglyphic indices gave a reliable prognosis of the risk of the above two kinds of disease due to silica dust. The dermatoglyphic patterns which predict predisposition to simple silicosis or to silicotuberculosis were found to differ significantly.

[Dermatoglyphics and cancerous diseases].

The dermatoglyphic analysis of finger and palm prints was conducted in oncologic patients living in the same town and in corresponding with respect to the number control groups of inhabitants of the same town that were matched to each patient by the most relevant risk factors. The study was based on 61 dermatoglyphic characters. Using computer pattern recognition programs sufficiently informative subcomplexes of these characters were formed. They permitted to discriminate between ill and control persons.

[Dermatoglyphic indices in assessing the predisposition to occupational fluorosis].

Dermatoglyphic investigations which prove the existence of genetic predisposition to occupational fluorosis in workers of aluminum and criolite plants were carried out. Mathematical methods of pattern recognition were used for the multifactorial analysis. It was found that the complex analysis of ten the most informative dermatoglyphic parameters permits to prognosticate a genetically determined risk of this occupational disease.

[Assessment of radioactive irradiation of Kamensk-Uralski inhabitants and its impact on their descendants' health].

The radioactive radiation of the inhabitants of the town of Kamensk-Uralsky, Sverdlovsk Region, was studied for its impact on the health of their descendants. The radiation of parents was found to be a risk factor for increasing the high incidence of frequent respiratory diseases of their children. The dietary deficiency of vitamins in children and other factors are also causes of frequent respiratory diseases.
