Lymphocyte transformation in patients with cobalt dermatitis.

A lymhocyte transformation test with cobalt chloride was performed in 14 patients who had positive cobalt patch tests and eczema compatible with a cobalt induced contact dermatitis and in 9 controls. Eight of the patients had strongly positive patch tests, while 6 had weakly positive patch tests. Patients and controls showed equivalent responses to the mitogens, phytohaemagglutinin, pokeweed mitogen, and concanavalin A, as well as to the recall antigens, purified protein derivative and Candida albicans. A 1:5000 dilution in RPMI-1640 medium of a 1% aqueous solution of cobalt chloride proved optimal in distinguishing patients from controls. With this dilution, a significantly higher lymphocyte transformation was found in patients with strongly positive cobalt patch tests than in the controls. No significant difference was detected when the results for patients with weakly positive patch tests were compared with those of the controls.

Lymphocyte transformation induced by nickel sulphate: an in vitro study of subjects with and without a positive nickel patch test.

Lymphocytes from 8 patients with contact dermatitis and a positive nickel patch test, 7 patients with contact dermatitis due to other factors and with a negative nickel patch test, and 9 other subjects, 7 of whom suffered from other dermatological disorders, were tested with the lymphocyte transformation test (LTT), using nickel sulphate in various concentrations. All execpt one of the nickel allergics showed a significant response to nickel, whereas the controls showed borderline (3 patients) or no response (12 patients). These observations confirm previous findings indicating that nickel hypersensitivity can be diagnosed in vitro. This may be of importance in cases of acute contact dermatitis where patch testing is undesirable. Some individuals with a negative nickel patch test responded significantly though weakly to nickel in the LTT, indicating that apart from acting as a specific antigen (hapten?) nickel sulphate may also have weak non-specific mitogenic properties.

T and B lymphocytes in contact and atopic dermatitis.

Twenty patients with contact dermatitis, eighteen with atopic dermatitis, two with mixed dermatitis, and 20 controls were examined for the number of T and B lymphocytes, and serum concentrations of IgD and IgE. Significantly higher counts of B lymphocytes bearing IgE and high serum IgE values were found in the atopic group. No other significant differences were found. In particular, we found a normal frequency of IgD bearing lymphocytes in contact dermatitis, and normal T lymphocyte counts in both groups of patients.

Studies of the antigenic structure of Trichophyton rubrum, Trichophyton mentagrophytes, Microsporon canis, and Epidermophyton floccosum by crossed immuno-electrophoresis.

By means of crossed immuno-electrophoresis, 35 antigens of T. rubrum, 26 of T. mentagrophytes, 35 of M. canis, and 25 of E. floccosum have been demonstrated. Tandem crossed immuno-electrophoresis has revealed two antigens common to T. rubrum and the other species, which, however, differ within the species. No common antigens have been demonstrated between T. mentagrophytes, M. canis, and E. floccosum. The technique for preparation of extracts and production of immune serum is described, and the procedure for the electrophoresis is outlined.

Lymphocyte transformation in vitro in dermatophytosis.

Peripheral blood lymphocytes from 59 patients with dermatophytosis and from nine young healthy women were studied by the lymphocyte transformation test (LT) using mitogens and bacterial as well as fungal antigens. The latter included Candida albicans (CA) and four dermatophyte species, viz. Trichophyton rubrum (TR), Trichophyton mentagrophytes (TM), Epidermophyton floccosum (EF) and Microsporum canis (MF). Most of the patients showed normal transformation in response to mitogens and non-dermatophyte antigens, indicating that they have no functional T-cell deficiency. Dermatophyte antigens act as stimulators in LT. In general, patient lymphocytes responded more strongly to these antigens than lymphocytes from controls. In most patients suffering from TM infections, response to the TM antigen was significantly stronger (p less than 0.05) than that in the other patients, indicating that this antigen preparation shows species specificity. In patients with Trichophyton (TR + TM) infections, response to the corresponding antigens was significantly stronger than that in the other patients, which suggests the existence of genus specificity. Any differences between patients suffering from chronic TR infections and those with acute TR infections were not observed, a finding which is in contrast to those obtained in other studies. However, a few patients with chronic TM infections responded weakly to mitogens and non-dermatophyte antigens. LT in four patients with id-reaction to TM infection was not found to differ from that in the remaining TM patients.

Miconazole in the treatment of candidiasis of the digestive tract.

Miconazole nitrate tablets were used in the treatment of candidiasis of the oral cavity and the digestive tract. In 5 out of 6 patients with severe, predisposing factors and who had received previous unsuccessful antimycotic therapy, clinical improvement occurred after a few days of miconazole treatment. Mycological cure of these 5 patients was achieved after 1-11 weeks of treatment. Three of the 5 cured patients relapsed after 3 months, due to basic failure. One patient complained of the drug's unpleasant taste. Otherwise, no side effects were observed. There was no apparent development of any resistance to the drug.

Reiter's disease and the histocompatibility antigen, HL-A 27.

HL-A27 were found in 31 of 48 patients with Reiter's disease (65%) as compared with 8% of 2 103 health controls. It is suggested that HL-A27 is closely related to the inheritance of a special immune response leading to an increased susceptibility to a special type of reactive arthritis which Reiter's disease has in common with ankylosing spondylitis and a certain type of psoriatic arthritis.
