[A case of pulmonary alveolar proteinosis associated with fibrotic changes and alveolitis].

A 62-year-old male, who had worked for 20 years as a farmer and who had been exposed to respirable agricultural chemicals apart from silica, was admitted in April, 1987, because of progressed exertional dyspnea. The chest X-ray film on admission showed irregularly-distributed infiltrative shadows in both lung fields. The chest CT also revealed irregular increase of densities in lung fields. Though the TBLB specimen from right B8 indicated only fibrotic changes, the tissues obtained by open lung biopsy from right S3 and S8 revealed PAS-positive proteinaceous deposits in the alveoli as well as alveolitis or fibrotic change of alveolar septum. Bronchoalveolar lavage fluid (BALF) contained phospholipids composed mainly of lecithin. According to these findings, the case was diagnosed as pulmonary alveolar proteinosis associated with alveolitis and patchy fibrotic changes. It is not completely clear whether fibrotic changes are induced by pulmonary alveolar proteinosis itself or fibrosis and proteinosis result coincidentally from direct response to fibrogenic agents. Inhaled agricultural chemicals may play a role for the pathogenesis of this case, for which further studies are required.

Combined effects of L-ascorbic acid, citric acid or their sodium salts on tumor induction by N-butyl-N-(4-hydroxybutyl)nitrosamine or N-ethyl-N-(4-hydroxybutyl)nitrosamine in the rat urinary bladder.

L-Ascorbic acid, citric acid or their sodium salts (at levels equivalent to 5% sodium L-ascorbate) were fed in the diet simultaneously with N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN) or N-ethyl-N-(4-hydroxybutyl)nitrosamine (EHBN) (0.025% BBN or 0.021% EHBN) in the drinking water to male F344 rats for 20 weeks to determine whether urinary pH changes affect the carcinogenicity of BBN or EHBN. In the urine, pH was decreased in rats fed the acidic chemicals and increased in rats fed their corresponding sodium salts. Histopathologically, the incidences and numbers of preneoplastic and neoplastic lesions in groups treated with each test chemical were not different from those in control groups except for sodium citrate-treated groups in which induction of carcinomas was higher, resulting from increased intake of either carcinogen and also from increased urinary excretion of main carcinogenic metabolites. These results show that the test chemicals do not affect the carcinogenicity of BBN or EHBN on the rat urinary bladder when simultaneously administered despite significant differences in urinary pH.

An experimental approach to 31P spin-lattice relaxation time measurement in biological systems.

The accuracy of measurement of the spin-lattice relaxation time (T1) in biological systems using the variable nutation angle method is discussed using computer simulations. This method could reduce the time required for measurement to 10% of that using the inversion recovery method. In the perfused salivary gland of the rat (310 K), the T1 values of sugar phosphates, creatine phosphate, gamma-phosphate, and beta-phosphate of nucleotide triphosphates determined by this method were 5.2, 4.8, 0.9, and 0.8 s, respectively. During secretion of saliva, the T1 value of sugar phosphates decreased to 3.7 s, which could be due to an increased chemical exchange of phosphate in glycolysis.

Further studies on the resolution of human mercapt- and nonmercaptalbumin and on human serum albumin in the elderly by high-performance liquid chromatography.

High-performance liquid chromatographic (HPLC) analysis of human serum albumin (HSA) on Asahipak GS-520 showed at least two peaks, the principal component corresponding to human mercaptalbumin (HMA) and the secondary one to nonmercaptalbumin (HNA). HPLC analysis of HSA on Asahipak ES-520 N showed three peaks, the principal component corresponding to HMA, the secondary one to HNA having mixed disulfide with cysteine or glutathione and the tertiary one to HNA oxidized higher than mixed disulfide. Two kinds of rapid HPLC for the resolution of HSA into HMA and HNA were developed by the present authors. Using these HPLC, the present authors found a significant decrease in the fraction of HMA in the elderly.

Effect of graded doses of erythorbic acid on activities of drug metabolic enzyme and phosphatases in guinea pigs.

The enzyme activities which depended on the ascorbic acid (AsA) tissue levels were assayed to investigate the effect of erythorbic acid (ErA) administration on the AsA availability in the guinea pigs administered 5 mg of AsA/day or 1 mg of AsA/day. The guinea pigs were given 5 mg of AsA and 1, 5, 20, 100 mg of ErA/day, or 1 mg of AsA and 1 or 20 mg of ErA/day for 16 days. The animals were sacrificed, blood was collected, and their livers were removed. The activities of liver aniline hydroxylase, liver acid phosphatase, and serum alkaline phosphatase, as well as the liver cytochrome P-450 content were measured. These enzyme activities and the liver cytochrome P-450 content of animals administered 5 mg of AsA seemed to show no change regardless of ErA supplement. Animals administered 1 mg of AsA showed different activities of liver aniline hydroxylase and liver acid phosphatase compared with those of animals administered 5 mg of AsA; however, the enzyme activities in animals administered 20 mg of ErA together with 1 mg of AsA were similar to those of the animals administered only 5 mg of AsA. These results indicated that ErA administration had no effect on the enzyme activities and the liver cytochrome P-450 content in the 5 mg AsA-supplemented animals, but administration of 20 mg of ErA was effective to maintain at normal levels the activities of liver aniline hydroxylase and liver acid phosphatase in the 1 mg AsA-supplemented animals.

Naturally occurring subclinical Corynebacterium kutscheri infection in laboratory rats: strain and age related antibody response.

Naturally occurring subclinical Corynebacterium kutscheri infection was analyzed by antibody response related to the strain of rats. Wistar-Lewis, Wistar and Spraque-Dawley rats were high responders in seroconversion rates and antibody titers, while Brown Norway and Fischer rats were low responders. The antibody response was related to age also. Some young rats had maternal antibody to C. kutscheri, but antibody disappeared before 8 weeks of age. Rats were antibody-negative for several months thereafter and became antibody-positive after 6 months of age. The antibody response was highest at 8 to 9 months of age in subclinical C. kutscheri infection. This antibody response was very late, compared to the antibody response to Sendai virus and Mycoplasma infections.

Cefpodoxime proxetil, its in vitro antibacterial activity, affinity to bacterial penicillin-binding proteins, and synergy of bactericidal activity with serum complement and mouse-cultured macrophages.

Cefpodoxime proxetil (CS-807) is an orally active prodrug of an oxime-type cephem antibiotic. The MIC60 values of cefpodoxime (R-3746) the active form of CS-807, were 3.13, 6.25, 0.05, 0.38, 0.2, 0.1, 3.13, 3.13, 6.25, 6.25, 0.1 and 12.5 micrograms/ml against S. aureus, coagulase-negative staphylococci, S. pneumoniae, E. coli carrying R plasmids, P. vulgaris, P. rettgeri, C. freundii, S. marcescens, A. calcoaceticus, P. cepacia, ampicillin-resistant H. influenzae and B. fragilis, respectively. Its activity was stronger than that of cefaclor and ampicillin. R-3746 manifested little activity against P. aeruginosa, methicillin-resistant S. aureus, and Enterococcus spp. R-3746 showed stronger binding affinity than cefaclor with the PBP2 of S. aureus, PBPs 1a, 1bs, 2 and 3 of E. coli, PBPs 1b, 1c and 3 of P. rettgeri, and the PBP3 of P. aeruginosa than cefaclor. Synergy of the bactericidal effect between R-3746 and serum complement was moderate, although the cells of E. coli NIHJ-JC2 and S. aureus 209P were well engulfed and rapidly digested by mouse-cultured macrophages in the presence of greater than 1/8 MIC of R-3746. Good clinical efficacy can be expected of CS-807 provided its pharmacokinetics prove to be good.

The hyperosmolarity-induced response of the ocular standing potential in mature rabbits.

The hyperosmolarity-induced response of the ocular standing potential (SP) provides a method of testing the function of the retinal pigment epithelium (RPE) without using light stimulation. In this study, the changes in potential level occurring after a short-term intravenous injection of 10 ml of 20% mannitol were determined by means of a direct current amplifier for the following groups: Group 1, normal rabbit eyes; Group 2, rabbit eyes in which the RPE was damaged by sodium iodate; Group 3, rabbit eyes in which the photoreceptors were damaged by monoiodoacetic acid; Group 4, rabbit eyes with uveoretinitis experimentally induced by Arthus-type inflammation. The following results were obtained: 1. The hyperosmolarity-induced SP response consisted of a transient increase in potential level (positive wave) in Group 1. 2. For Group 2 a transient decrease in potential level (negative wave) was obtained, i.e., a reversal of the normal positive response to negative wave. 3. A positive wave and no reversal was found for Group 3. 4. For Group 4 a negative wave and a reversal of the normal response was obtained. These hyperosmolarity-induced SP responses provide additional information concerning the possibilities of the method for studying the function of the RPE.
