Hydroxylation of desmethylimipramine: dependence on the debrisoquin hydroxylation phenotype.

The 2-hydroxylation of desmethylimipramine (DMI) was studied in 14 healthy subjects previously phenotyped with respect to debrisoquin hydroxylation. After a single oral dose (25 mg), slow hydroxylators of debrisoquin had significantly lower total and metabolic clearances and longer plasma half-lives of DMI and excreted less 2-hydroxydesmethylimipramine than did rapid hydroxylators. These findings strengthen the hypothesis that the hydroxylations of debrisoquin and DMI may be under common enzymatic control.

Sigmoid diverticular abscesses: percutaneous drainage as an adjunct to surgical resection in 24 cases.

Twenty-four patients with acute sigmoid diverticulitis and associated pelvic fluid collections seen on computed tomographic scans underwent percutaneous catheter drainage as an adjunct to surgical therapy. Fourteen of the 24 underwent a single-stage surgical procedure within 10 days of drainage. Five patients required two-stage surgical procedures because localized inflammatory changes precluded a primary resection despite the absence of a residual abscess at surgery. Two of the three remaining patients initially had no surgery, but they had recrudescences of their symptoms that required surgical drainage within 8 months. One patient in whom surgical resection was deferred remained asymptomatic 10 months after percutaneous drainage. A retrospective review of 87 patients undergoing surgery for diverticulitis suggested that the percentage of two-stage surgical procedures has decreased in the last 5-10 years, but there remains a substantial number of patients who might benefit from percutaneous catheter drainage of diverticular abscess of the sigmoid colon.

Sonographic guidance in diagnostic and therapeutic interventions in the pleural space.

One hundred eighty-seven diagnostic and therapeutic interventional procedures in the pleural space were performed by using sonographic guidance. These consisted of diagnostic aspiration (118), drainage of malignant and nonmalignant effusions (41), empyema drainage (17), pleural sclerotherapy with tetracycline or bleomycin (7), and pleural biopsy (4). Diagnostic aspiration was performed with 20-gauge needles, and therapeutic and empyema drainages were performed by trocar technique with either a 7-French Sacks catheter or a specially designed empyema drainage catheter. Pneumothoraces were seen in 3% of the patients, and most of these were treated by the radiologist with placement of a Heimlich valve. We conclude that the use of sonography allows rapid localization of pleural fluid collections and instant monitoring of drainage of noninfected fluid collections and empyemas.

Transcystic biopsy: a new approach to posterior pelvic lesions.

Six patients with masses located posterior to the bladder were biopsied using an anterior transcystic approach. Conventional biopsy needles were introduced through the bladder with both sonographic (five) and CT guidance (one). Clinically important diagnoses of recurrent carcinoma (four) or cytology-negative fluid (two) were made in all patients. One patient had a transient episode of minimal hematuria immediately after the biopsy. Otherwise, there were no complications. Transcystic biopsy appears to be a safe and effective technique for biopsy or aspiration of posterior pelvic masses.

Three-dimensional computed tomography in maxillofacial trauma.

Roentgenographic evaluation of maxillofacial trauma is an integral component in the management of the acutely injured patient. Recent developments in computerized software permit three-dimensional computed tomographic (CT) reconstructions of complex anatomic parts. Five examples of complex maxillofacial trauma were demonstrated by conventional plain films, followed by two-dimensional and three-dimensional CT. Three-dimensional CT vividly demonstrates spatial relationships not easily conceptualized by present modalities.

Hepatocellular carcinoma presenting with intractable diarrhea. A radiologic-pathologic correlation.

A 44-year-old woman with hepatocellular carcinoma presented with intractable watery diarrhea and her condition was evaluated angiographically. Surgical ablation of the tumor resulted in complete resolution of the diarrhea. The tumor cells of the hepatocellular carcinoma were found to contain vasoactive intestinal polypeptide, gastrin, and prostaglandinlike immunoactivity. To our knowledge, this is the first report of such an association.

Neurologic complications in diabetics after metrizamide lumbar myelography.

Recognized risk factors for metrizamide myelography are seizure disorder, seizure-threshold-lowering drugs, dehydration, and possibly age. After observing serious neurologic complications in diabetic patients after routine metrizamide myelography, a retrospective study was conducted to determine if diabetes should be considered another independent and important risk factor. Forty-one diabetic patients who had lumbar metrizamide myelograms were compared with a control group of 110 nondiabetic patients. A significantly higher incidence was found of severe vomiting (15% vs. 3%, p less than 0.01) and neurologic complications (20% vs. 2%, p less than 0.001) in the diabetic population. Neurologic complications included one case each of seizure, severe encephalopathy, auditory and visual hallucinations, and prolonged somnolence and four cases of confusion-anxiety. Four of the diabetic patients had major transient elevations of blood pressure. These findings suggest that diabetics are a high-risk population for metrizamide myelography. The dose of metrizamide should be minimized, whenever possible. The new nonionic myelographic agents may prove to be safer in this population, but caution and careful follow-up should be exercised in the initial trials with these patients.

Sonographic examination of the abdominal aorta through the left flank: a prospective study.

Ultrasonographic evaluation of the abdominal aorta is most often done with the patient in the supine position. The right lateral decubitus position, which views the aorta through the left flank has, until now, been considered unsatisfactory for aortic evaluation. One hundred consecutive patients were prospectively examined for visualization of the aorta both through the left flank and the anterior abdomen. Twenty-one patients were then comparatively examined from the right coronal and left coronal approach. These studies showed that the aorta was clearly visualized using the left flank approach in the majority of patients (96 per cent). The combined approach yielded 99 per cent satisfactory visualization, and in a few select cases (13 per cent) the left flank was actually superior. The left flank approach was superior when directly compared with the right flank in 42 per cent of patients and comparable in 48 per cent. The right flank approach was superior in only 10 per cent.

[A method for puncturing the deep neck muscles in humans].

The authors developed a method to puncture neck muscles by means of computer tomographic control, enabling the introduction of an electrode into the obliquus capitis inferior muscle, to investigate the particular role of neck muscles in posture and head movement.
