Influence of different factors on the formation of calcium oxalate stones. I. Discriminant analytical computations of morphological parameters of the pelvic-calyceal systems of calcium oxalate stone formers and controls.

Until now almost exclusively clinicochemical urine parameters have been used to determine the risk of urinary stone formation. In part I of this study, pelvic-calyceal systems of healthy persons and calcium oxalate stone formers are compared with regard to their morphology, whereas urine parameters will be briefly included in part II. Discriminant functions are given by means of which it is possible to calculate the risk and, e.g. to quantify a follow-up.

[Direct superior oblique muscle trauma and dysfunction--surgical possibilities and results].

Direct trauma to the superior-oblique muscle of the eyeball may result in active and passive ocular motility disturbance, e.g. paresis, Brown phenomenon, cyclorotation disorder, head-tilt and faulty head posture. In individual cases combinations of these and rather complex forms may occur. Surgery should aim at sufficient centralization of the binocular field of vision. The author usually recommends indirect procedures, at the ipsilateral inferior oblique muscle, to correct excyclorotation, or at the contralateral inferior rectus muscle to correct paresis. A pronounced Brown phenomenon should be treated by recession of the superior oblique muscle, or rather its transposition to the nasal side of the superior rectus. In addition, recession of the contralateral superior-rectus muscle or resection of the ipsilateral inferior-rectus muscle may be advisable. Out of eight patients, one refused surgery, five were sufficiently cured in one session, and another patient in three surgical sessions. One patient could not be satisfactority treated by surgery; in addition to a Brown phenomenon she had excyclorotation, especially in down-gaze, and paresis of the superior oblique muscle plus severe neuralgic pain in up-gaze. In this patient the trochlea had been accidentally lost during a foreign-body excision in the upper medial orbit. A secondary reconstruction of the trochlea was found not to be advisable in this patient, because the neuralgia was thought to be due to traction within the superior oblique tendon or the periost.

[The synthesis of P-substituted DL-2-phenylglycine octylesters].

9 Titel compounds (p-RC6H4)CH(NH2.HCl)COO (CH2)7CH3 [R = F, Cl, HO, CH3O, CH3(CH2)3O, C6H5CH2O, CH3, (CH3)2CH and (CH3)2N.HCl] were obtained by esterification of p-substituted DL-2-phenylglycines with octanol either in the presence of dry hydrogen chloride (procedure A) or with thionyl chloride (procedure B). The advantages of the procedure B are the mild conditions, the more pure products, the possibility of esterification of diamino acids, and the stability of alkoxy groups as substituents. The synthesized octylesters are potential antiinflammatory and analgesic substances with spasmolytic activity.

[Non-medicamentous prevention of calcium oxalate urinary calculi].

In a prospective study the efficacy and actual outcome of non-medicamentous preventive measures with metabolically unremarkable calcium-oxalate urolithiasis were investigated in 49 idiopathic calcium-oxalate stone formers. Overnight urine excretion was not increased. The mean concentration and excretion of essential parameters in terms of lithogenesis were hardly influenced. The obvious reduction of the pelvi-calyceal area in both lithogenous and non-lithogenous kidneys after one year of calculus prophylaxis was striking. By means of a discriminant function including morphological parameters and high-molecular urine components some developments may be verified in the discriminant space suggesting a good prophylactic result in many patients.

[Accidental fall-induced craniocerebral injury].

If the effects of force on the head due to falls are subdivided into three subgroups (forehead, lateral parts of the head and occiput), it is to be observed that occipital traumas take first place in falling on or from stairs (51%) and in falling on the same level (57%). Persons under the influence of alcohol fall on the back of the head to a greater extent than persons not under the influence of alcohol. It is noteworthy that the base of the skull is frequently involved in cranial fractures (91%). Subdural hematoma was associated with a cranial fracture in a high percentage of cases (fall on or from stairs 91%, fall on the same level 75%). Epidural hematomas were both found in isolation and in combination with subdural hematomas (fall on or from stairs 12%, fall on the same level 17%). Falling as a fatal cause of death takes first place today before traffic accidents.

[Diagnostic value of lymphocyte differentiation in the early phase following kidney transplantation].

In 50 patients who had a renal transplantation, treated with ciclosporin, regular monitoring of lymphocyte subpopulations was undertaken prospectively to assess its value with respect to cellular rejection, herpes virus infection, and ciclosporin overdosage. Herpes virus infection was characterized by inversion of the T4/T8 ratio below 1.0 (sensitivity 90%, specificity 88%), caused by proliferation of the T8 subpopulation, which--compared with the findings in patients with rejection crises--was significantly raised (P less than 0.001). But such rejection crises could not be predicted from the T4/T8 ratio. Ciclosporin had no effect on the ratio, total lymphocyte count in this group being higher (P less than 0.002) than in patients with rejection.

[Therapy of amyloid nephrosis in Crohn disease: plasmapheresis plus azathioprine?].

A 38-year old woman with serious nephrotic syndrome due to kidney amyloidosis in consequence of Crohn's disease was observed. The long-term prognosis of this illness is serious, but since in the literature several good results have been mentioned under treatment with immunosuppressives, in the present case an aggressive immunosuppression was carried out, which was started under the conception that, in consequence of elimination of preamyloid substances from the serum by plasmapheresis in combination with an additional inhibition of the reproduction of these substances by azathioprine, the clinical picture could be favourably influenced. During therapy with long-term intermittent plasmapheresis in combination with azathioprine a complete remission of the nephrotic syndrome was achieved.

The pitfalls of technetium Tc 99m/thallium 201 parathyroid scanning.

Technetium Tc 99m/thallium 201 parathyroid scanning has been proved to be effective by multiple researchers. A review of 54 patients who underwent scanning and operations at an institution showed the scans to be accurate in 83%. There were 15% false-negative and 2% false-positive scans. The pitfalls of parathyroid scanning can be related to false-positives and false-negatives.

Automated turbidimetry for rapid determination of the bacteriological quality of raw meat and processed meat products.

A fully automated centrifugal spectrophotometer for turbidimetric measurements linked to a computer for data processing was used to record and analyze growth of different types of bacteria associated with spoilage of raw meat and processed meat products (sliced meat for sandwiches) in order to predict bacterial counts. Media for this specific analysis were developed to enumerate total viable count, lactic acid bacteria. Brochothrix thermosphacta and Pseudomonas spp. Correlation coefficients of -0.91 to -0.97 were obtained between the conventional plate count technique and the turbidimetric method (linear regression). Lactic acid bacteria in the range 10(3) to 10(9) cfu/g could be detected within 48 h, whereas total viable count, Brochothrix thermosphacta and Pseudomonas spp. generally could be detected within 24 h. Counts exceeding 10(7) cfu/g could be detected within 6 h.

[Quantitative histological studies of lamina V and lamina III pyramidal neurons in the motor cortex of the rat. The question of laterality].

Laterality of several brain functions has been demonstrated in the human brain cortex as well as in several animal species indicating an evolutionary genesis of the brain's asymmetry. To get some morphological data, we investigated Golgi impregnated pyramidal neurons in lamina III of regio precentralis agranularis of the major cortex from adult rats. The spine number and density and the length of the dendrites forming a basal or apical single dendritic field (SDF = EDF) of these neurons were estimated. The statistical significance of the data was proved by comparing the neuronal parameters obtained from the right or left hemisphere of the same animal (individual laterality), as well as by testing the mean values from right or left cortex of several animals (species laterality). However, in both cases no statistical significance could be obtained. From these results two conclusions should be drawn: Firstly, the parameters in a single dendritic field of cortical pyramidal neurons of both brain sides are statistically equivalent and not asymmetrical developed. Secondly, the single dendritic field of cortical pyramidal neurons can not serve as a morphological counterpart for functional asymmetries in the rat brain. Here, further interdisciplinary research is necessary.

In vitro activity of mecillinam and amoxicillin/clavulanic acid against strains of Escherichia coli producing TEM-1, Oxa-1 and chromosomal beta-lactamases.

The in vitro activity of mecillinam and amoxicillin/clavulanic acid against Escherichia coli strains producing beta-lactamases of the TEM-1, Oxa-1 and chromosomal type were studied using the broth and agar dilution technique. In addition the beta-lactamase activity was determined. The experiments indicated that the beta-lactamase types differed in their influence on the minimum inhibitory concentrations. Both, mecillinam and amoxicillin/clavulanic acid were considerably more active against TEM-1 strains than ampicillin alone, but, with increasing specific beta-lactamase activity, a decrease of susceptibility was found. Strains producing Oxa-1 and chromosomal beta-lactamases usually where highly susceptible to mecillinam but only moderately sensitive or resistant to the combination of amoxicillin/clavulanic acid. In vitro activity of TEM-1 E. coli to mecillinam was strongly reduced when tested in a broth dilution technique. These test systems, however, did not affect the susceptibility of Oxa-1 and chromosomal beta-lactamase producing E. coli strains to either antibiotic.
