[Early ophthalmological manifestation of subacute sclerosing panencephalitis. Diagnostic and possible therapeutic aspects (author's transl)].

A case of a 6-year-old boy is referred. Ophthalmological findings suggested subacute sclerosing panencephalitis. Only very distinct neurological alterations could be found. Positive measle titers in serum and cerebrospinal fluid and positive subacute sclerosing panencephalitis factors proved the diagnosis. While the first eye showed a macular scar and mild optic neuritis the second eye developed the acute stage of retinal lesion and immediate visual loss. Differential diagnosis and possible therapeutical aspects are discussed.

[Conclusions of the first-aid treatment of emergency patients from a prospective study of 106 lethal outcomes (author's transl)].

To obtain an objective judgement on the efficiency of emergency help, 106 emergency patients who died during a six month period were subjected to an examination in which all aspects of their treatment, beginning at the scene of the emergency and lasting until their death were documented. It could be shown that 8.5% of the examined patients would have had a chance for survival if proper and optimal treatment had been given, despite the effects of their injuries. Only by improving knowledge of first-aid procedures can an increase in the efficiency of emergency measures be attained as will be proven in this paper. Intensive schooling is necessary not only for the layman and ambulance orderlies but for doctors who are to be employed in rescue work in particular. The present criteria (one-year clinical activity and competency are by no means sufficient as a foundation.

[Electron microscopy of the neural and synaptic organization of the regio cingularis in the rat].

The neuronal structure and synaptology of the regio cingularis of the rat was examined with electron microscopical methods. On the basis of GOLGI-impregnation studies of the cingulate region ultrastructural characteristics of pyramidal and nonpyramidal neurons were investigated concerning the variations in size and content of intracytoplasmatic organelles, morphology of the nucleus and the number, site and kind of synaptic contacts. There are mainly small and medium sized pyramidal cells. The axo-somatic synapses appear as symmetrical contacts (type GRAY II). The number and distribution of small and medium sized nonpyramidal neurons (interneurons) are different in various cortical layers. Both types of synapses, asymmetrical as well as symmetrical contacts (type GRAY I and type GRAY II) were found on the pericarya of these nonpyramidal cells. As has been established for the neocortex, the dendrites of cingulate pyramidal neurons are bundled. The numerous spines along these dendrites are of different size and shape. The asymmetric axo-spinodendritic contacts (type GRAY I) are the most frequent type of synapses compared with axo-dendritic junctions. The dendrites of nonpyramidal neurons are of the spiny type or of the sparsely spined type with dilations of "beads" (varicosities) that are present along them. The ultrastructural investigations and the results following terminal degeneration (electrolytical stereotaxic lesions in the nucleus mediodorsalis and in the nucleus anteromedialis thalami) cause the suggestion that the specific thalamic afferents reach monosynaptically the layer III or V pyramids and specific interneurons. Recurrent axon collaterals of the pyramids and inhibitory interneurons represent the morphological correlates for the recurrent inhibition of the pyramids. These results demonstrate a similarity between the cingulate cortex and the neocortex concerning the synaptic organization made up by excitatory and inhibitory neurons.

[Quantitative studies of layer III pyramidal cells of the cingulate cortex of the rat].

The dendritic tree and the spine distribution at layer III pyramidal cells of the cingulate cortex in three months old male rats were evaluated quantitatively with Golgi-Kopsch impregnated brains. The analysed pyramidal cells were classified into three groups as to different lengths of their apical main dendrites. The spine values for a basal individual dendritic field show significant differences between these three groups. The layer III pyramidal neurons of the cingulate cortex exhibit on an average a total length of the dendrites of 2253 micron with a total 1507 visible spines. The total average of spines on the apical main dendrites amounts to 0,57 spines per micron, the average length of the apical main dendrites amounts to 217 micron. The apical values of spine densities are smaller than that of the basal values. The basal dendritic tree is more branched and exhibit a higher total length of the dendritic branches than that of the apical dendritic tree. The apical and basal differences as to spine densities and lengths of the dendritic branches results in an equality as to the total number of apical and basal spines per neuron. The values of the cingulate layer III pyramidal cells are smaller by comparison with those of the striate area. These results are closely related to the number of afferent terminations in both regions.

[Serum thyroglobulin estimation for follow-up of patients with thyroid carcinoma (author's transl)].

Thyroglobulin concentrations were determined radioimmunologically in the sera of 53 patients operated for thyroid carcinoma. 37 patients without metastases or recurrence had either very low (up to maximally 15 ng/ml) or non-demonstrable thyroglobulin concentrations, whereas 16 patients with metastases showed values of more than 40 ng/ml up to maximal values of more than 1000 ng/ml. In the majority of patients follow-up controls, in part up to 2 years, were performed. A far-reaching conformity of thyroglobulin concentrations with scintigraphic or clinical findings of metastases was shown. Thyroglobulin estimation in operated thyroid cancer patients is thus an important contribution to the aftercare.

["Possessedness" and exorcism in the year 1976 (author's transl)].

A 23 year-old woman died as a result of deficient food and liquid intake as well as of physical exhaustion brought about by an unchecked pathological displacement impulse. Finally pneumonia developed in a typical manner. During the previous 9 months exorcism had been carried out a total of 60 to 70 times, the last time being on the eve of her death, to "expel demons" and to heal the "possessed". Summarized the medical diagnosis of "possession" was: Parnoid-hallucinatory psychosis with epilepsy in the background of special psycho-social factors. During the time of influence by the priests who carried out the exorcism all medical treatment was denied. This, it was pointed out by the participants, occurred at the express wish of the "possessed" and also due to the conviction that medical aid would be ineffective. This fateful development took place in a milieu of belief in demons fostered by the priests and uncritical rejection of medico-scientific treatment methods. According to the results of the hearing of witnesses during the now legally valid proceedings ending with the exorcists and the deceased's parents being convicted for accidental homicide a doctor probably also participated in what happend in a reprehensible manner.

[The application of highly hydrophilic contact lenses in the Fuchs-Steven-Johnson syndrome (author's transl)].

In 3 patients with status after Fuchs-Steven-Johnson syndrome highly hydrophilic contact lenses (72% H2O-up take) were adapted and worn permanently. 2 patients had worn already for a long time usual HEMA-lenses. The lenses showed themselves to be favorable against mechanical irritation from cilia and accessory eyelashes and from scarring conjunctival conditions, and they affected favorably the signs of dessication of the cornea. Corneal defects healed, and the patients had subjectively no complaints. It is sometimes necessary to replace the tears. An increase in the corneal vascularisation already present could not be observed during the 18 months observation time.

[Report on corneo-scleral ulceration after cataract extraction (author's transl)].

A case of corneo-scleral ulceration beginning 4 months after uncomplicated cataract-extraction is reported on. Neither conservative nor operative treatment was able to stop the ulceration, so that enucleation was necessary. The clinical picture and serological and ophthalmopathological findings are shown. The probably immunological aetiology is discussed.

[Localization of thalamic afferents in the regio cingularis of the rat].

After lesioning the nucleus mediodorsalis and the nucleus anteromedialis thalami, where from specific afferents project to the cingulate cortex, we confirm signs of transneuronal degeneratiopn at lamina-V-pyramidal neurons of the cingulate cortex. By the GOLGI-KOPSCH-method operated animals show statistically significant decrease of dendritic spines at the apical main dendrites of lamina-V-pyramidal cells compared with the controls. The average spine-decrease along 300 micrometer length of the apical main dendrite amounts to 16%. Maximal spine-decrease is present at the central part of the apical main dendrite: 23% within a 50-micrometer-segment of this dendrite. This central dendritic part passes through the lamina III of the cingulate cortex. By the FINK-HEIMER-method degeneration particles are preferentially dispersed in laminae I, III and lower layers. The results give evidence for a more precise neuronal localization of the specific afferent terminations in this region.

[Erythropoietin in serum and urine in healthy persons and patients with chronic renal disease upon hypoxic stimulation and hypoxic stimulation after pretreatment with fluoxymesterone (author's transl)].

The question of a latent erythropoietin (ESF)-deficiency was studied in incipient renal anemia using a double stimulation technique for ESF. After a 4-week stimulation of ESF production with oral administration of fluoxymesterone (flu) intermittent hypoxic ESF stimulation was performed corresponding to a maximum altitude of 4000 m in 7 patients with chronic renal disease without or with incipient renal anemia (mean hematocrit 40%) and in 11 normal subjects (mean hematocrit 46%). Double stimulation in patients was compared with hypoxic stimulation alone and both were compared with controls. After flu alone only ESF excretion was increased in patients and in normal subjects. After flu plus 10 h of hypoxia serum ESF--titers were higher in healthy subjects than in the patients. The mean ESF titer after double stimulation was 81 mU/ml in patients and 115 mU/ml in normal persons. Forty-eight hour ESF excretion was 11 U and 43 U respectively. Compared to hypoxic stimulation alone double stimulation increased serum ESF titers in patients by 5% versus 80% in controls. Correspondingly, ESF excretion was enhanced by 19% and 49%, respectively. Finally, renal ESF clearance was increased by 42% versus 200%. After hypoxia alone non-anemic patients had higher serum ESF titers than healthy controls excluding a latent ESF deficiency in incipient renal anemia. It is concluded that decreased ESF production after double stimulation in patients was due to a nephrotoxic effect of flu followed by a decreased excretory and ESF-producing function of the damaged kidneys.

[Deadly scald or Lyell syndrome? (author's transl)].

The changes of skin of the Lyell Syndrome have an extreme likeness with those of a 2nd degree scald. As this illness is more frequently observed as of late, it may get forensic importance. Based on the case of a man, 48 years of age, who suddenly died without previous diagnosis and therapy the aetiologic and morphological problems are discussed.
