[Catamnestic findings in patients following gastric resection].

A radomized study was done in 2 groups of male patients operated upon 3-17 years ago either by partial gastrectomy (Billroth II type) or hermiotomy. The inquiries covered eating, drinking and smoking habits, occupation, body weight, subjective complaints, and use of drugs. Body weight and professional activity after operation were not different in both groups. Gastrectomized patients avoided milk or foods with milk, sweets and legumes more often. Feeling of fullness, vomiting and diarrhea occurred more frequently in gastrectomized subjects, as well as statements about good or even ravenous appetite and thirst. Gastrectomized men, besides, had a higher consumption of alcoholic drinks, preferably of beer.

[Endoscopic diagnosis of carcinoma in the operated stomach].

In 24 000 gastroscopies at 4 medical departments 705 subjects with gastric operations done more than 5 years before had been studied. 39 of 608 patients with Billroth II, 5 of 81 with Billroth I and 4 out of 15 with gastroenterostomy were found to be suffering from carcinoma of the gastric stump. The primary gastric operations were performed at a medium age of 40 years. The interval between surgery and development of carcinoma averaged 24 years and was shorter in patients operated at a higher age. Besides age and interval since operation risk factors were apparently former gastric ulcers or polyps. Indications for endoscopic follow-up studies of gastrectomized patients to improve surgical results with carcinoma of the gastric stump have been suggested.

Adenocarcinoma↗

[Tuberculosis of tongue (author's transl)].

Report on a nodular form of tuberculosis of the tongue in a 91-year-old patient, which was cured completely by exstirpation and tuberculostatic therapy. The histologic examination of this "tumor of the tongue" lead to the radiologic proof of a chronic-specific lung process, which is interpreted as the source of the tuberculosis of the tongue by blood-stream dissemination. Discussion of the clinical and morphological forms of the tuberculosis of the tongue, of the differential diagnosis, as well as references to the importance of histologic examinations of ulcers of the tongue resulting in the detection of a specific lung process in some cases.

[Acute pancreatitis in Mycoplasma pneumoniae infections].

Report on a man of 29 years with acute bronchopneumonia, antibodies against Mycoplasma pneumoniae and acute pancreatitis lasting three weeks. In some cases M. p. must be considered to be the cause of acute pancreatitis.

Acute Disease↗

[Gas gangrene panophthalmitis (author's transl)].

Gas gangrene panophthalmitis is a rare condition of penetrating injury to the globe. The infecting organism is usually Clostridium perfringens. Characteristic symptoms are a brawny swelling of the lids, marked chemosis, coffee-coloured discharge, hypopyon, ring abscess of the cornea, formation of gas bubbles in the anterior chamber, rise of intraocular tension and early amaurosis. Treatment consists in the evisceration or enucleation of the globe, rarely in the exenteration of the orbit. Antibiotics along (Penicillin, Tetracyclines) are insufficient. Administration of antiserum is almost completely abandoned, it is probably more dangerous than helpful. The use of hyperbaric oxygen is not indicated in cases of gas gangrene panophthalmitis. Extraocular extension of the infection and its danger for the individual is prevented by well-times surgical procedure.

Dynamics of canine pancreatic blood flow and of insulin secretion during an intravenous glucose load.

Blood flow through the superior pancreaticoduodenal artery of anaesthetized dogs increases immediately after an i.v. glucose load and parallels to glycemia, but not to insulin output. Flow of superior mesenteric, and femoral arteries as well as of portal vein are also enhanced after rapid i.v. injections of glucose or mannitol. Extent and duration of all flow increases are dose-dependent and strongly related to the alterations of arterial plasma osmolality (maximum increase 24 mosmoles/kg). Heart rate, mean arterial blood pressure, and hematocrit remain unchanged after the first test minute. There is a considerable decrease of peripheral vascular resistance in the circulatory regions investigated. Flow through aorta ascendens is augmented for a short time. Possible mechanisms of vasodilatation are reflexes produced by osmoreceptors and/or local effects of osmolality on the tonus of vascular muscle. Within the physiological range pancreatic perfusion does not determine the amount of insulin released.

Chromosome aberrations in human lymphocytes after irradiation with 15.0-MeV neutrons in vitro. I. Dose-response relation and RBE.

Human peripheral lymphocytes were irradiated with different doses of 15.0-MeV neutrons. The frequency of different aberration types was determined and the dose-response relation was calculated. The data were fitted by least-squares regression analysis to different models. The dicentric, dicentric plus centric ring, and different acentric data gave the best fit to the linear quadratic model. The RBE of 15.0-MeV neutrons versus 220 kV X-rays decreased significantly with increasing dose.

Chromosome aberrations in human lymphocytes after irradiation with 15.0-MeV neutrons in vitro. II. Analysis of the number of absorption events and the interaction distance in the formation of dicentric chromosomes.

The rejoining distance for the formation of dicentric chromosomes in human lymphocytes after irradiation with 15.0-MeV neutrons has been derived on the basis of microdosimetric concepts. For the formation of a dicentric chromosome, primary lesions produced by absorption events can interact within the nucleus over a distance of at least I.8 mum. The mean number of absorption events within this range is lower than for sparsely ionizing radiation. The intercellular distribution of dicentric chromosomes is consistent with a negative binomial distribution.

[Effect of ethrane on the circulatory parameters of geriatric patients (author's transl)].

The cardiovascular effects of enflurane in geriatric patients have been compared with the effects of this anaesthetic agent in a group of young patients. The degree of cardiovascular depression in geriatric patients was much greater than in the young patients despite the smaller inspired concentration of enflurane necessary to produce a similar depth of anaesthesia. In comparison with halothane and neuroleptanaesthesia, enflurane produced the greatest decrease in cardiac output. The mean arterial pressure, however, was quite well maintained due to a simultaneous increase in total peripheral resistance.

[On mucoepidermoid tumors with rare locations in laryngological areas (author's transl)].

Report of three cases of mucoepidermoid tumors with rare locations in laryngological areas (fundus of the tongue, retromolar trigonum, nasopharynx and sinuses). In two cases the tumors could be removed radically, the third case resulted in an extensive tumorous destruction of the right half of the face after a protracted course of the disease, as the consequence of which the patient died. With the aid of the literature on the subject, the location, clinical features, pathological anatomy and histology, origin, biological valence and the prognosis of mucoepidermoid tumors are discussed. It is generally agreed that they are optionally malignant tumors which can induce a locally infiltrating or destructive growth, show a tendency to recurrence and may lead to distant metastases. The recommended therapy is the radical exstirpation of the tumor.
