[Influence of pirenzepin on healing of gastric ulcer. A double blind study in in-patients (author's transl)].

In a double-blind study the influence of Pirenzepin on ulcer healing was studied in 28 in-patients with gastric ulcer under endoscopic control. The ulcer size was reduced by Pirenzepin significantly quicker, and the number of ulcers healed after three weeks treatment with Pirenzepin was significantly higher than under placebo. As to accompanying symptoms appetite stimulation was significantly more frequent with Pirenzepin compared to placebo. No patient complained of dryness of the mouth.

The intermediate-sized filaments in rat kangaroo PtK2 cells. II. Structure and composition of isolated filaments.

When cultured cells of the rat kangaroo cell line PtK2 grown on plastic or glass surfaces are lysed and extracted with combinations of low and high salt buffers and the non-ionic detergent Triton X-100 cytoskeletal preparations are obtained that show an enrichment of 6 to 11 nm thick filaments. The arrays of these filaments have been examined by various light and electron microscopic techniques, including ultrathin sectioning, whole mount transmission electron microscopy, negative staining, and indirect immunofluorescence microscopy. In addition, 6 to 11 nm filaments isolated from these cells with similar extraction procedures and with centrifugation techniques have been examined by electron microscopy. The arrays of these isolated intermediate-sized filaments, their ultrastructure and their specific decoration by certain antibodies present in normal rabbit sera as well as by guinea pig antibodies against purified bovine prekeratin is demonstrated. When preparations enriched in these intermediate-sized filaments are examined by SDS-polyacrylamide gel electrophoresis a corresponding enrichment of three polypeptide bands with apparent molecular weights of about 45 000, 52 000 and 58 000 (the latter component sometimes appears split into two bands) is observed, besides some residual actin and a few high molecular weight bands. The morphology of the isolated filaments, their immunological reaction with antibodies decorating prekeratin-containing structures, and the sizes of their constitutive polypeptides suggest that these filaments are closely related to prekeratin-containing filaments observed in a variety of epithelial cells.

[Diagnostic accuracy of fiber endoscopy in esophageal and gastric carcinoma. Attempt at error analysis].

A retrospective analysis was performed in 332 cases with cancer of the upper gastrointestinal tract. A limit of one month was set for delayed and missed bioptically verified endoscopic diagnosis. Malignant cycle in ulcerated lesions and the size of elevated processes were the probable causes of delayed diagnosis in 7 out of 37 cases with early cancer. In advanced gastric cancer missed diagnosis was probably caused by the indication for endoscopy made too late in an unknown number of cases or patient's refusal to undergo controls. However, failure of the endoscopist to recognize a malignant process must be considered too. The percentage of endoscopist's mistakes was below 2% in our series.

[Liver circulation during dopamine therapy].

Intravenous dopamine (4 and 8 microgram/kg/min) causes an increase of hepatic flow and cardiac index, while the ratio hepatic flow:cardiac index remains unchanged. The increase of renal flow after dopamine therefore does not occur at the expense of hepatic flow.

[Diphtheria epidemic in eastern Switzerland in 1974].

A diphtheria epidemic in the eastern part of Switzerland in 1974 and the measures which were taken for its management and eradication are described. In particular it is pointed out that at present diphtheria displays atypical clinical symptoms in Europe, a fact that renders the diagnosis very difficult in many cases. Therefore, it is important to perform a throat swab in every patient with suspected diphtheria; it is also necessary to explicitly require a search for C. diphtheriae. The early recognition and early treatment of diphtheria with penicillin or erythromycin is of the utmost importance in order to avoid neurological and cardiac complications. With regard to the management and eradication of a diphtheria epidemic, the most important measures are rapid antibiotic protection of all contacts and a widespread immunization of the population. Vaccination of adults with a DiTe vaccine containing a reduced amount of Di-toxoid is recommanded.

Chromosome analyses of children after ecological lead exposure.

In the present work chromosome analysis was performed in a group of 30 children living in a town with a lead plant. Due to the emission of the smelter the individual lead uptake through food, drinking water and inhalation was increased. They were selected out of 1600 children whose blood lead level, delta-aminolevulinic acid dehydratase activity in the erythrocytes and erythrocyte porphyrine level was measured. In the investigated group of children the values of these parameters showed to be indicative for a significant lead exposure. A total of 10,000 cells was scored after 48 h culture time. Despite a significantly increased lead load as compared with two groups of 10 children from a suburb and the isle of Helgoland there was neither evidence for a higher number of cells with structural chromosome aberrations, nor for an increased aberration yield.

[General anaesthesia with ketamine for electro-cochleography in children (author's transl)].

To enable recordings to be made of compound action potentials of the acoustic nerve using transtympanic electro-cochleography, a method of modified general anaesthesia with ketamine has been tested on forty children. Sufficient sedation and analgesia causing the least mental disturbance and possibly minimal stress to the cardio-respiratory system was of special interest. We used a combination of droperidol and fentanyl as premedication in low dosage to obtain a maximal efficiency of the possibly minimal medication of intravenously injected ketamine. A recovery phase without troubles was ensured in older children by additional i.v. diazepam medication during the time of ketamine administration.

[Osseous metaplasia of renal medulla (author's transl)].

Bone formation in renal medulla arises from papillary necrosis and is observed almost exclusively following chronic pyelonephritis and chronic interstitial nephritis. Since papillary necrosis occurs in the early stage of chronic interstitial nephritis, this kind of osseous metaplasia has specific diagnostic significance. By means of a case report formal pathogenesis, experimental basis, clinical diagnostic considerations and prognosis of the bone formation in renal medullary parenchyma are discussed.

The cytogenetic effect of an X-ray contrast medium in Chinese hamster cell cultures.

Chinese hamster cell cultures were treated with different concentrations of Joduron, a water-soluble iodized X-ray contrast medium. The cytogenetic effect was analysed for a treatment period of 2 or 16 h. Jorudron primarily acts on the mitotic apparatus of the cell. The type of damage ranges from different forms of initial "C-mitosis" to unpolarized stathmokinesis. Additionally, structural chromosome damage, mainly of the chromatid type, was observed, especially after a Joduron treatment for 2 h.

Analysis of the time relationship for the interaction of X-ray induced primary breaks in the formation of dicentric chromosomes.

In split-dose experiments, the time relationship for the interaction of primary breaks in the formation of dicentric chromosomes was analysed. Human peripheral lymphocytes were irradiated with a dose of 340 R of 220 kV X-rays split into two equal fractions separated by intervals up to 360 min. Assuming an exponential decline of the time-dependent quadratic component of the dose realtion of dicentrics a theoretical formula was deduced. Fitting this formula to the empirical data a mean time of 110 min could be calculated in which primary breaks induced by both dose-fractions can interact to form dicentric chromosomes.
