[A positive effect of combined therapy (plasmapheresis in combination with pulse therapy) on the course of cryoglobulinemic glomerulonephritis and ulcerative-necrotic vasculitis in patients with Sjögren's disease].

The authors provided evidence of a positive effect of combined therapy (plasmapheresis in combination with pulse-therapy) on a course of cryoglobulinemic glomerulonephritis and ulcerative-necrotic vasculitis in patients with Sjögren's disease (SD). It was shown that plasmapheresis with double plasma filtration considerably decreased the cryoglobulin level in the blood whereas the level of alpha 2-globulins, C-P, RF and CIC did not change 16 hrs after the sessions of plasmapheresis. These indices significantly decreased after pulse-therapy with high doses of 6-methylprednisolone and cyclophosphamide following plasmapheresis. Combined therapy resulted in a complete epithelization of leg ulcers, regression of the symptoms of renal failure. Clinical improvement correlated with a decrease in indices of the inflammatory and immunological activity and with an increase in the cutaneous blood flow. Combined therapy can be an effective therapeutic method of SD complicated by cryoglobulinemic glomerulonephritis and ulcerative-necrotic vasculitis.

[Microcirculatory disorders in the genesis of sclerodermic nephropathy].

Biomicroscopy of the vessels of the conjunctiva (BMV), the determination of erythrocyte and platelet aggregation capacity and investigations of the effective cutaneous and muscular blood flow and blood plasma renin activity were employed to study microcirculation in 110 patients with systemic sclerodermia (SSD) (74 with and 36 without renal lesion). The detected disturbances were correlated with the nature and gravity of sclerodermic nephropathy (SN). In BMV the frequency of intravascular platelet aggregation increased with a rise of the gravity of renal pathology. The quantitative determination of erythrocyte and platelet aggregation capacity revealed a significant acceleration of the aggregation of the blood cell elements in the SSD patients which correlated with SN presence and gravity as well as with renal function indices. Renin plasma activity was significantly raised in patients with severe renal pathology. There was correlation between the indices of function of the kidneys and ultrastructural signs of lesion of their microcirculatory bed. The results of the study served as a basis for the differential use of vascular drugs and the development of new pharmacotherapeutic methods for SSD patients with renal lesion.

[Place of hemocarboperfusion and hyperbaric oxygenation in the treatment of patients with rheumatoid arthritis with systemic symptoms].

Altogether 12 patients were treated by the hemocarboperfusion method and 20 patients with systemic symptoms by the hyperbaric oxygenation (HBO) method. Positive results were obtained and it made it possible to recommend the above methods for multimodality therapy of patients with rheumatoid arthritis with systemic symptoms. A severe, rapidly progressive course of rheumatoid arthritis with the development of various systemic symptoms, a high titer of the rheumatoid factor should serve as an indication for hemocarboperfusion. HBO is found appropriate in such systemic symptoms as ischemic polyneuropathy, digital arteritis, trophic ulcers and Raynaud's syndrome.

[Status of tissue microcirculation in psoriatic arthritis].

Microcirculation (the skin and skeletal muscle) was studied using a method of 133Xe clearance from the interstitial depot in 35 patients with psoriatic arthritis. It showed significant microcirculatory disorders expressed in a statistically significant decrease in the effective cutaneous blood flow in unaffected skin zones (mostly in women and in a high inflammatory activity of the process) and in the muscular microcirculation especially noted in the period of reactive hyperemia. The results obtained indicate the presence of generalized involvement of the microcirculatory bed vessels in psoriatic arthritis.

[Corinfar in the treatment of Raynaud's syndrome in systemic scleroderma (preliminary results)].

Twenty patients with systemic scleroderma (SSD) and Raynaud's syndrome (RS) received 30-80 mg of corinfar, a blocker of calcium channels, during 14 days. Seventeen patients responded to the treatment. The clinical effect was manifested by a decrease in the rate, duration and intensity of RS attacks. Side effects were recorded in 6 patients, but the drug was discontinued only in 2 patients because of the allergic reactions. During treatment, the patients showed an increase in tissue blood flow parameters. Corinfar may be regarded a drug of choice in the treatment of marked RS in SSD.
