Under controlled clinical observation we administered Cimetidine (600 mg/m2/day) in 10 children with cystic fibrosis. All patients received identical diets (40% fat, 45% carbohydrate, 15% protein) as well as same standardized enzyme preparation (lyophilised whole pancreas). 3-day fecal fat and nitrogen excretion analyses were performed. After 6 days of Cimetidine therapy, an improvement of fat and nitrogen absorption could be detected in 5 patients. Statistical analysis of the group as a whole showed no improvement in fat absorption coefficients. We conclude that this drug therapy should only be recommended for routine clinical use after long-term trials have shown a real improvement of absorption.

Authors review the epidemiology of deep venous thrombosis and the prothrombotic state in high risk patients; particularly antithrombin III role in blood clotting is considered. A fall in antithrombin III concentration and/or activity can induce hypercoagulability and thrombosis. Moreover authors stress the need to evaluate antithrombin III as anti-Xa activity, to detect high risk patients for deep venous thrombosis.
Prostacyclin production was significantly depressed in foetal and placental vascular tissues from five patients with severe pre-eclampsia in comparison to vascular tissues from women with uncomplicated pregnancy. Such an abnormality may be responsible for a reduced blood flow and defective fetal nutrition thus playing a major role in the pathogenesis of this syndrome.

Bilirubin displays a detectable fluorescence emission only when it is complexed with serum albumin, whereas free bilirubin has a very low fluorescence yield. Actually, nearly complete disappearance of bilirubin emission was obtained when the unirradiated human serum albumin-bilirubin complex was precipitated with acetone to extract the pigment; complete removal of protein-bound bilirubin (as monitored by fluorescence spectroscopy) was achieved by repeating the acetone extraction after incubation of the complex in the phosphate buffer, pH 7.4, containing 7 M guanidinium chloride; the latter compound causes on extensive unfolding of protein molecules. On the other hand, in the case of irradiated solutions, even after denaturation of the protein with 7 M guanidinium chloride, a detectable amount of bilirubin-type fluorescent material was found to be associated with albumin. This finding clearly shows that bilirubin and/or some photoproduct underwent in part a photoinduced covalent binding with human serum albumin. Fragmentation of the bovine albumin polypeptide chain according to the procedure detailed in the experimental section yielded only one peptide-containing material fluorescent in the 530 nm region. This fact underlines the selective nature of the photobinding reaction. The amino acid composition of the isolated peptide is shown in Table 2; the composition is closely similar with that found for peptide 187-397 of native bovine serum albumin. In the case of the jaundiced babies who were subjected to phototherapy, we were able to demonstrate that only after 7 to 9 hr of exposure to light a detectable amount of bilirubin-type fluorescent material was present even at the end of the serum treatment with acetone and guanidinium chloride (see Fig. 1; Table 1). Fractional precipitation of the serum proteins by addition of controlled amounts of ammonium sulphate showed that the fluorescent material was present only in the albumin fraction. The photoadduct disappeared about 15 to 20 days after the phototherapy had been discontinued. This period of time represents the natural turnover period of human serum albumin.

A comparative review of the various properties of human breast milk and cows' milk is presented. The advantages of breast feeding are emphasized. The recommendations of the ESPGAN and other international organizations for the formulation of artificial feeds are discussed.
A double-blind evaluation of the clinical effectiveness of a single dose of naproxen, ibuprofen, and acetaminophen was performed on 98 patients who had undergone periodontal surgery. A statistically significant difference was found when the effect of these medications was compared with that of a placebo, but no statistically significant difference was found among these three drugs. No side effects resulted from the use of these medications.
The long-term clinical course of 11 adults with hemolytic-uremic syndrome (HUS) is reported. All patients were treated with heparin and antiplatelet drugs, and ten required dialysis. One patient died after 38 days; the others recovered from anuria after seven to 400 days. One patient was resubmitted to regular dialysis five years later, and another died because of cerebral hemorrhage. Among the remaining eight patients, four show renal failure and four have normal renal function after one to ten years of observation. All but three require vigorous antihypertensive therapy. It is concluded that in adults with HUS (1) recovery may occur even after a prolonged anuria; (2) severe hypertension and progressive renal failure may appear later in apparently recovered patients; and (3) heparin and antiplatelet drugs seem to be beneficial in reversing acute renal failure.
A case of severe hypertension and acute renal failure developing in a 56 year old woman affected by diffuse scleroderma is reported. The patient was treated by peritoneal dialysis, antihypertensive agents and plasma infusions, which led to the correction of arterial hypertension and partial recovery of renal function. This report emphasizes the possibility that the correction of malignant hypertension may prevent progressive renal falure in some patients affected by diffuse scleroderma.
