Cooperativity in O2 binding to iron porphyrins.

The solid-gas O2 binding equilibrium has been studied for for ferrous "picket fence" porphyrinates with sterically hindered axial imidazoles. Such systems show significant cooperativity in their binding of O2: at low O2 pressures a low O2 affinity form exists, and at high O2 pressures a higher O2 affinity form develops. Direct analogies are drawn to the cooperativity shown in O2 binding by hemoglobin. These model systems mimic hemoglobin quantitatively.

Sliding appendiceal inguinal hernia.

Rarely, the mesoappendix forms the sliding component of an inguinal hernia. Appendectomy, in such instances, permits effective repair of the hernia. This was accomplished in 16 patients without wound complication or recurrence of the hernia.

[An arterial catheter for multiple use in pediatrics (author's transl)].

The authors describe a pediatric catheter for use mainly in highly selective abdominal angiography (splenic, gastrica sinistra, and intestinal arteries). The catheter is made of polyurethane, is 65 cm long and is curved. It has a bore of 5 F and is introduced with a wo 32 guide, allowing a flow-rate of 10 ml/sec. The catheter point is polished by chemical means allowing penetration of the arterial wall without preliminary dilatation. This avoids vascular spasm and damage to the vascular wall at the site of puncture.

[A case of acute ischemia of the extremities in disseminated intravascular coagulation in an infant].

The authors report a case of diffuse intravascular coagulation occurring in a 15 day old infant suffering from several congenital malformations (meningocoele, megacolon and bilateral hydronephrosis). During reparative surgery, acute and symmetrical of the lower limbs developed in a context of shock and respiratory distress. Associated with a coagulopathy, it progressed towards the death of the child on the second postoperative day. The authors review the cases of gangrene of the extremities with diffuse intravascular coagulation occurring in the young child found in the literature, and discuss the factors which favourise its development.

Intestinal bypass arthritis: association with circulating immune complexes and HLA B27.

Two patients are described in whom arthritis following intestinal bypass surgery for obesity was associated with the presence of circulating immune complexes (CLC) and HLA B27. The arthritis was characteristically intermittent and controlled by low dose prednisone, indomethacin, and tetracycline therapy. The findings suggest that immune complexes play a role in the pathogenesis of arthritis associated with condition, and that perhaps the association with HLA B27 may predispose to the development of this complication.
