Chlamydial serum IgG antibodies in patients with acute salpingitis measured by an enzyme-linked immunosorbent assay.

An enzyme-linked immunosorbent assay (ELISA) for the detection of serum IgG antibodies to Chlamydia trachomatis has been developed. The C. trachomatis subtype LGV-2 was used as antigen. The ELISA was reproducible and its sensitivity and specificity compared well with that of the single-antigen immunofluorescence test (r = 0.83). 29 (85%) of the 34 patients with acute salpingitis had chlamydial serum IgG antibodies measured by the ELISA technique. The detection level in single blood donor specimens was 30%. Among the 34 patients with acute salpingitis, 16 paired serum specimens showed a fourfold or greater rise/fall in antibody titres, and 12 of these belonged to the 19 who harboured C. trachomatis in the lower genital tract.

Therapeutic abortion and Chlamydia trachomatis infection.

Chlamydia trachomatis was isolated from the cervix of 30 of 218 (13.8%) women admitted for legal termination of pregnancy. During the first two weeks after the abortion seven of the 30 (23.3%) patients developed pelvic inflammatory disease. Four of these had serological evidence of recent active chlamydial infection. Thus, routine examination of patients for genital chlamydial infection before termination of pregnancy is recommended.

Severe infection following pudendal anesthesia.

A case of severe infection following pudendal anesthesia is presented. The probable spread of the infection to the subgluteal space and the complications are discussed. Musculoskeletal symptoms arising in the hip area and the pelvis during the early postpartum period pose a challenging diagnostic problem. When these symptoms are accompanied by fever and pain, the possibility of a severe infection with possible abscess in the subgluteal or retropsoas space must be considered. The treatment consists of prompt surgical drainage and appropriate antibiotics.

Carcinoma of the colon in pregnancy.

Carcinoma of the large bowel in association with pregnancy is extremely rare. A case report is presented, where the patient had a malignant tumour in the sigmoid colon with metastases in the liver. The diagnosis was made at caesarean section after acute onset of symptoms 3 weeks prior to the expected date of delivery. The case is discussed in some detail.

Mechanical small bowel obstruction due to adhesions.

During the years 1971-1975, 103 patients were operated on for adhesive small bowel obstruction, giving an incidence of 80 per 1 million inhabitants per year. Previous laparotomy caused the obstruction-provoking adhesions in 90% of the cases. Appendicectomy, gynaecological operations, and operations on the intestine were the three most common surgical procedures performed prior to the obstruction. In 40 cases with clinically less severe symptoms and signs follow through radiological studies with contrast medium secured the diagnosis of obstruction. The mortality rate was 5.8% and increased with the duration of symptoms. Two patients died of complications caused by intestinal fistula following lysis of extensive, firm adhesions. Bypass or bowel resection might be safer procedures in those cases. Bowel resection did not affect the mortality rate. None of the 23 patients with strangulation died. This may be due to early operation since the mortality rate in the series as a whole doubled when symptoms had lasted for more than 24 hours. Recurrence rate after operation for adhesive small bowel obstruction was 27.8% and eight patients required relaparotomy.
