[Esophageal and gastric involvement in diabetes mellitus].

The most recent literature concerning the involvement of the esophagus and stomach in diabetes mellitus is reviewed. This pathology has rarely been studied as a whole, may be due to an underestimation of its importance in medical treatment. The following conclusions are drawn: the extent of autonomic damage varies greatly in individual patients; there is a high frequency of neuropathy with simultaneous development at various levels, thus causing the nervous lesion to manifest itself in different ways.

[Clinical and statistical study of 843 hospitalized diabetics].

A retrospective cross study of 843 hospitalized diabetics is reported. The heterogeneity of the sample was obviated to some extent by subdividing it into age, duration and diabetes mellitus treatment groups. The major part of the sample consisted of elderly patients presenting the characteristic correlations between hyperglycaemia, overweight and triglyceridaemia common to insulin-resistant diabetes. Impairment of the cardiovascular system and particularly the coronary circulation, was particularly serious. Stress is laid on the need to standardize study of the diabetic in the interests of a correct, epidemiological and clinical approach to the disease.

Myocardial ischemia in Vaquez's disease.

The authors report a study performed on the ECG findings in patients affected by polycythemia rubra vera. Clinicopathological correlations are made on the basis of autopsic examinations of 11 of these patients (out of the 21 subjects studied). The clinical investigation was performed on the ECG findings according to the Minnesota Code; pathologic investigation pointed out some particular lesions of myocardial district, as myocardium sclerosis, acute infarction, post-infarction scars, vascular lesions of the macro- and microcirculation. The results showed that ECG and the pathological findings in polycythemia vera appear to have a physiopathologic background although the latter is not linked to a well-defined anatomopathologic picture.

[Carcinoid cardiopathy. A study of 40 cases].

Carcinoid is a slowly growing type of tumor; its pathological effects are primarily due to its endocrine symptomatology. Among the main causes of death are heart failure, liver disease and complications due to the tumor size. The purpose of this study was to investigate carcinoid heart disease in a wide necropsy sample. We analyzed 26,921 necropsies performed at the Institute of Pathology of the University of Trieste from January 1, 1976 to December 31, 1985. Out of the 26,921 necropsies we found 59 cases with carcinoid tumor. It is interesting to underline the presence of a second primary tumor in 28.8% of cases and of multiple tumors in 2 cases. In the heart we observed valvular abnormalities with no peculiar features in 16.1% of cases, ischemic heart disease in 52.2% and endocardial thickening in 15.9%. Our series, although rather large, did not show the high frequency of carcinoid heart disease that has been reported by other authors; instead, we observed a rather high prevalence of lesions typical of ischemic heart disease. The endocardial thickening seems to be the most interesting and specific finding.

[The treadmill test in the clinical evaluation of the lower limbs. With special reference to an elderly population group].

The authors show that the treadmill is a good method for studying the peripheral circulation of the inferior limbs, without altering the coronary district capacity. This technique is fitter than the test of Master and/or cycloergometry because it swits the characteristics of elderly patients. The estimate of the vascular function has been carried out by the means of the Winsor Index (WI), which is based on the proportion between the values of the occluding arterial pressure of the posterior tibial in comparison with the humeral arterial pressure. This quotient is able to discriminate the real peripheral vascular insufficiency from the borderline cases as well as from the normal ones, even if not in the presence of painful pathology. We have examined 50 patients; in this group 17 subjects (34%) resulted affected by peripheral arteriopathy of the inferior limbs; 8 of them presented a bilateral form with a pathologic WI; 2 patients of the same above mentioned group resulted symptomatic for pain. The remaining 9 presented a unilateral arteriopathy; of these patients, 6 resulted symptomatic for pains and 3 of them had an already pathologic WI before the stress test. On the contrary, no patients visualized signs of coronary disease; the superficial ECG suffered no alteration; the cardiac frequency has in total increased by 5.8 heart rate.

[Hyperhomocysteinemia: a new independent vascular risk factor?].

The positive correlation existing between hyperhomocysteinemia and atherosclerosis has firmly been established through data derived from numerous epidemiologic and experimental observations as well as from intervention trials. Although most of the clinical data have been obtained in relation to coronary heart disease, hyperhomocysteinemia is also observed in patients with cerebral and peripheral arterial occlusive disease or peripheral venous thrombosis. The prevalence of the heterozygous state is today estimated about 1 to 2 percent of the population. Mild and moderate hyperhomocysteinemia have recently been proposed as an additional and independent risk factor for vascular disease. In this review we therefore describe recent findings about the pathogenesis of hyperhomocysteinemia and their implications for optimal drug therapy.

[Computerized digital thermometry in Raynaud's disease. Personal experience].

BACKGROUND: Computerized digital thermometry has been used for instrumental diagnosis of Raynaud's disease, that is characterized by diminution of the cutaneous temperature of the fingers and the late delayed recovery. Thermometry permits to measure basal temperature of the ten fingers, during cooling to 10 degrees C ("cold test") and the response, measuring temperature minute by minute up to 25 degrees. METHODS: In order to assess vasospastic ischemic disease a total of 66 subjects have been examined: 19 were asymptomatic for acrolocalised pathologies (control subjects) and 47 were symptomatic. RESULTS: No close correlation was observed between clinic and instrumental data. In fact 31.5% of the asymptomatic subjects had a "non-normal" reaction to the test; on the other hand, in the group of female over-50-years-old with symptoms suggesting Raynaud's disease, 38.5% of cases revealed "normal" instrumental patterns. Therefore no discriminating parameters were identified which might have allowed the instrumental identification of subjects suffering from Raynaud's disease compared to healthy individuals. CONCLUSIONS: In conclusion, computerised digital thermometry is a technique with a good level of sensitivity, while the specificity is scarce.
