Indication and value of bladder neck incision.

200 bladder neck incisions in 172 patients with mechanical or functional outlet obstruction of neurogenic and nonneurogenic etiology are reported. The incision was performed with an electric cautery needle at 5--7--12 o'clock. Parameters of success were, beside the subjective reports, residual urine, urinary flow and a sophisticated urodynamic study. 71% of the women and 76% of the men showed improvement or relief of symptoms. Retrograde ejaculation was observed in only 7% of the men; the rate of complications was 2.5%. Despite limited experience and the lack of long-term follow-up, this technique is recommended for routine use.

Effect of flavoxate on hyperactive detrusor muscle.

The effect of Flavoxate on hyperactive detrusor contraction was studied in 37 patients, including 11 patients in whom the drug was administered intravenously during the urodynamic study. Beside the evaluation of frequency, nocturia and enuresis, the amplitude and the onset of uninhibited detrusor contraction (in correlation to bladder volume) was registered. The results showed subjective improvement in 61.3%. The complaints mentioned above improved in approximately 50%. The urodynamic data showed diminishing of the mean pressure during uninhibited detrusor contraction by almost 50% and the delay of the onset by 80% of bladder capacity. However, average bladder capacity increase was only 19%. It finally became obvious that the effect of Flavoxate was markedly worse in the neurogenic bladder group in comparison to the motoric urge patients.

Evaluation of urodynamic studies by computer.

In an attempt to simplify urodynamic evaluation, the data obtained from cystometry, urethral profile and flowmetry were analysed using the ICS recommendations for standardization and terminology. All studies were compiled from check lists feasible for computation. The aim was to establish standards and "typical" changes for the normal bladder and pathological states to enable automatic readout of computed data. The results did, in fact, show "typical" changes but failed to establish standard measurements suitable for completely computed diagnoses.

Transurethral bladder neck operation in neurogenic bladder.

86 adults with bladder outlet obstruction and neurogenic bladder underwent bladder neck resection or incision to establish effective vesical emptying or damping of detrusor reflex activity. Follow-up examinations revealed a good effect on flow, reflux and hydronephrosis, infection rate and symptoms such as frequency, dysuria, nocturia and urgency. Since 1974, bladder neck incision is preferred; resection remains for selected cases only.

[Conservative-surgical therapy of the neurogenic bladder].

If physical therapy and drugs remain unsuccessful in the treatment of the neurogenic bladder, conservative-operative interventions should be performed prior to urinary diversion. Bladder neck incision, supraselective neurectomy and implantation of a bladder pacemaker in highly selected patients offer a good chance of avoiding diversion. In 64 cases of bladder neck incision, 80% improved or were cured. Experience with the neurectomy in patients with hyperactive detrusor is still limited. The preliminary results, however, proved that this technique should be included in the treatment of neurogenic bladder. 8 of 11 patients in whom a bladder pacemaker was implanted showed voiding following implantation. 7 patients displayed reflex evacuation.

[Inguinovaginal sling operation in the treatment of female urinary incontinence (author's transl)].

Sling operation have long occupied an established position in the treatment of severe female urinary incontinence and recidivous incontinence. The inguino vaginal sling operation as modified by Narik and Palmrich is described and the operation results for 73 female patients are differentially evaluated. A precise preoperative urodynamic clarification and a previous or simultaneous hysterectomy are prerequisites for satisfactory operation results. In certain instances, this surgical procedure represents a possible method for the restoration of continence even in the case of neurogenic bladder dysfunction.

Urodynamic equipment for screening and office use: CO2 cystometry.

A newly available compact urodynamic set-up is presented herein, consisting of a 2-channel CO2 pressure recording unit with flow registration. Flow, bladder and rectal pressure recording are considered the minimal requirements for obtaining sufficient information on functional changes of the lower urinary tract.

'Diagnostic overkill?' An analysis of 88 negative renal angiograms.

11.3% of 776 renal angiograms evaluated retrospectively displayed normal findings. The main reasons which had led to the misuse of angiography in our cases were: technically poor urograms, incorrect interpretation of anatomical variations and the lack of other examinations associated with the urogram. A diagnostic plan including all less invasive and dangerous procedures is proposed placing sonography in central position.

Renal cell carcinoma: immunohistological investigation of expression of the integrin alpha v beta 3.

alpha v beta 3 Integrin has been shown in various tumor entities to promote binding to superficial structures of the basement membrane during metastasis. The goal of the present paper was a structural demonstration of this Integrin in renal cell carcinoma (RCC). After removal of paraffin from formalin-fixed tissue, the cells were labelled with the antibody (VNR 147, H. Biermann, Bad Nauheim) using the peroxidase-antiperoxidase method (DAKO Diagnostical GmbH, Hamburg). Evaluation was carried out microscopically and semiquantitatively from missing (-) through moderate (+) to strong (+2) staining. A total of n = 79 RCCs and n = 53 healthy areas were examined. Semiquantitative staining results: there was a grading-dependent increase (+2) of stainability and thus alpha v beta 3 expression. Two of 53 benign specimens showed strong staining, 11 of 53, only weak staining. Four of 18 G1 RCCs showed strong staining, 11 of 18 only weak staining. Results for G2-RCCs: 11 with strong staining, 21 of 40 with weak staining. G3-RCCs: 4 with strong staining, 2 of 7 with weak staining. Of the metastases, on the other hand, 2 of 14 showed strong staining, another 8 of 14 only weak staining. There were no deviations within the histologic (clear-cell, chromophil, or chromophobe) subpopulations. This grading-dependent expression permits the conclusion that the probability of binding to the human basement membrane mediated by alpha v beta 3 Integrin rises with increasing grading, but the already metastatic cell exhibited this Integrin less strongly, since a basement membrane adhesion is no longer necessary for this cell group.
