[Continuous, low dosage estradiol administration with a vaginal ring: a placebo-controlled study].

UNLABELLED: Estrogen replacement therapy is used in postmenopausal women with symptoms of urogenital atrophy. The aim of this study was to detect differences in efficacy of a low-dose estradiol-releasing vaginal ring and a placebo ring in the relief of symptoms of estrogen deficiency and reduction of urogenital atrophy (pH and vaginal mucosal maturation) in postmenopausal women. DESIGN: This was a multicentre double-blind, placebo-controlled study. PATIENT SAMPLE: 84 postmenopausal women, of whom 67 completed the full 24 weeks of treatment. RESULTS: Vaginal pH decreased significantly (p = 0.0006) in the estradiol group compared with the placebo group. Similarly, in the estradiol group the mean maturation value increased significantly (p = 0.0004), while incidence of pallor and friability were significantly reduced. Patients in the estradiol group reported significantly greater relief of dyspareunia. CONCLUSION: The results indicate that the estradiol-releasing vaginal ring is effective and well tolerated in the treatment of local estrogen deficiency.

Skeletal alkaline phosphatase: a marker for individual follow-up in patients with advanced prostatic cancer.

Skeletal alkaline phosphatase (sAP) is a tumor marker indicating osseous metastases, e.g. of prostate cancer. Sera of healthy men and patients with benign prostatic hyperplasia (BPH), localized and advanced prostatic cancer (PCa) were analyzed with Tandem-R-Ostase and prostate-specific antigen (PSA). No significant differences were found in sAP levels between healthy men and patients with BPH and localized PCa, but there was a significant difference with the group of patients with advanced PCa. In some cases, the individual follow-up was better and earlier with sAP compared to PSA. It is possible to discriminate between localized and advanced PCa with sAP. The individual follow-up shows in 30% of patients with advanced PCa an earlier increase in sAP, compared to PSA, during progression of disease.

Investigations on bone metabolism of urological tumors forming metastases.

Bone metastases of urological tumors occur in nearly 40% of all primary tumors of the prostate, the kidney and the bladder. The quality of metastases may be described as osteolytic, osteoplastic or mixed lesion. Whereas prostate cancers produce mainly osteoplastic lesions, renal cell carcinomas predominantly generate osteolytic lesions. In bladder cancer both forms of metastases occur in tantamount numbers. However, diagnostics still presents many difficulties, since it is not feasible to identify very small metastases until symptoms have manifested themselves. The purpose of our study was to evaluate measurement technique and classification of significant serum markers for monitoring the course of disease. Patients with primary urological tumors and metastases in the skeleton were investigated and compared with healthy volunteers. Osteodensitometry was used to confirm and to replace radiological diagnosis of bone metastases. Thus it was possible to locate the extent and obtain information on the maximum charge and the stability of metastases. Our examinations revealed that distinct serum markers describe the changes in bone evoked by metastases. In comparison with healthy volunteers, patients with osteoplastic lesions and osteolytic lesions showed increases in hydroxproline and pyridinium crosslinks (significance at least p < 0.005). Osteocalcin was elevated only in osteoplastic lesions versus healthy volunteers (p < 0.01). For diagnostics of osteoplastic and osteolytic metastases, either alkaline phosphatase or the skeleton-specific phosphatase (ostase) can be measured serologically. Both parameters showed significant elevation in the patient groups when set against the healthy controls (both p < 0.0001). Compared with lytic lesions osteoplastic carcinomas revealed significant increase of alkaline phosphatase (p < 0.0001) and osteocalcin (p < 0.005). In examination of bone metabolism in patients with skeletal metastases the following parameters are of eminent interest: osteocalcin, hydroxyproline or pyridinium crosslinks, alkaline phosphatase or ostase. These serological parameters could be helpful even with regard to early diagnosis of bone metastases. Evaluation of measuring techniques suggests quantifying pyridinium crosslinks instead of hydroxyproline, because they may be assessed without taking the patient's diet into account. Determination of bone density may be helpful in diagnostics or control of therapy modalities.

Assessment of cardiac output from noninvasive determination of arterial pressure profile in subjects at rest.

The stroke volume of the left ventricle (SV) was calculated from noninvasive recordings of the arterial pressure using a finger photoplethysmograph and compared to the values obtained by pulsed Doppler echocardiography (PDE). A group of 19 healthy men and 12 women [mean ages: 20.8 (SD 1.6) and 22.2 (SD 1.6) years respectively] were studied at rest in the supine position. The ratio of the area below the ejection phase of the arterial pressure wave (A(s)) to SV, as obtained by PDE, yielded a "calibration factor" dimensionally equal to the hydraulic impedance of the system (Zao = A(s).SV-1). The Zao amounted on average to 0.062 (SD 0.018) mmHg.s.cm-3 for the men and to 0.104 (SD 0.024) mmHg.s.cm-3 for the women. The Zao was also estimated from the equation: Zao = a.(d + b.HR + c.PP + e.MAP)-1, where HR was the heart rate, PP the pulse pressure, MAP the mean arterial pressure and the coefficients of the equation were obtained by an iterating statistical package. The value of Zao thus obtained allowed the calculation of SV from measurements derived from the photoplethysmograph only. The mean percentage error between the SV thus obtained and those experimentally determined by PDE amounted to 14.8 and 15.6 for the men and the women, respectively. The error of the estimate was reduced to 12.3 and to 11.1, respectively, if the factor Zao, experimentally obtained from a given heart beat, was subsequently applied to other beats to obtain SV from the A(s) measurement in the same subject.

[Are small urodynamic evaluation units competitive?].

Urodynamic investigation allows a classification and differential diagnosis of disorders of the lower urinary tract. The number of parameters registered is fixed by patient selection and then economical feasibility of the urodynamic unit. Differentiating stress urinary incontinence and urge incontinence or neurogenic disorders, which is of clinical importance, small two-channel recorders showed to be a competitive screening-method.

[Results of treatment with budesonide (Pulmicort) in corticosteroid-dependent intrinsic asthma].

Budesonid is a topical corticosteroid for application by inhalation with a high anti-inflammatory effect. 25 patients (15 women, 10 men, average age 45.3 years) with intrinsic asthma requiring corticosteroids were treated for an average of 36 days with budesonid (Pulmicort dosage aerosol, Astra Sweden) in a dose of 3 X 400 micrograms per day. The basic therapy with beta sympathicomimetics, theophylline preparations and systemic corticosteroids was continued. Changes in the plasmacortisol level (PCS) and the course of the asthma were studied under reduction of the systemic corticosteroid dosage. The mean reduction in dosage of the systemic corticosteroid achieved was almost 2/3 of the initial dosage (p 0.001). The pathologically reduced PCS at the start of the study normalized in 13 patients (52%) to values within the standard range. A significant (p 0.001) mean increase in PCS of 43% was achieved. The eosinophils in the blood underwent no significant change. The peak flow improved by a mean 26% (p 0.001) with respect to the initial values. Despite a reduction in systemic corticosteroids, the symptoms of breathlessness, coughing and expectoration improved or declined practically without exception. Apart from some temporary hoarseness and throat irritation in 5 patients, there were no undesired side effects or incompatibility.

[Ureteral endometriosis].

In the literature, figures quoted for the involvement of the urinary tract range between 1% and 11%, adenomyosis being the exception rather than the rule. The problems of preoperative diagnosis are illustrated with reference to five of the author's cases. In four-fifths of the cases definitive confirmation of the findings was only obtained by intraoperative examination with a rapid incision. In cases with preserved renal function, ureteroneocystostomy employing the psoas hitch technique with subsequent hormone treatment was the treatment of choice. Summarizing, endometriosis should be considered in differential diagnosis in premenopausal women with unclear complaints and micturition disorders in the distal one-third of the ureter.

[Value of flowmetry in disorders of bladder emptying].

After definition of the 'rate of increase in urinary flow' uroflowmetry still does not seem to be of any value for the differentiation of abnormalities of micturition in women as a screening method. Correlating several parameters of uroflowmetry with different subjective and objective data and the final urodynamic diagnosis in 1113 sophisticated urodynamic studies we did not find a discriminatory value of any of the parameters described.
