The basic health care system in Botswana: a study of the distribution and cost in the period 1973-1979.

Since 1973 Botswana has developed its basic health service with an extensive network of clinics and health posts staffed with nurses, health assistants and Family Welfare Educators (11 weeks training in preventive medicine). In 1977 it was estimated that 80% of the population lived within 15km of either a health post or a clinic. In the study period the annual number of new registered outpatient diagnosis per inhabitant increased from 0.65 to 1.50, but even after the heavy investment in the rural areas the annual number of visits per rural inhabitant, was in 1979, 0.8 compared to 2.8 in the urban areas. In 1979 the per capita health expenditure was US+18.7, of which the basic health service accounted for US+8.8. The relationship between Botswana's basic health service and a primary health care system as described in the Declaration of Alma Ata is discussed.

Steady-state visual evoked potentials during migraine prophylaxis by propranolol and femoxetine.

The amplitude of visual evoked potentials (VEPs) for flickering light has been reported to be increased in migraine. In the present study, we have examined whether the VEPs are attenuated when the clinical state of the patient improves during a double-blind experiment with propranolol and femoxetine. VEPs for sinusoidally-modulated light were measured by spectral analysis, and an index depicting the visual reaction type was calculated. The group mean VEP index closely followed the group mean attack frequency, but individual variance was considerable. The changes were most evident in VEPs elicited by stimuli of about 20 Hz. During the treatments, the VEP and headache were also significantly correlated among subjects. The results suggest a close relationship between the enlarged VEPs and the headache mechanisms.

Treatment of alcohol withdrawal symptoms in hospitalized patients. A randomized, double-blind comparison of carbamazepine (Tegretol) and barbital (Diemal).

Seventy-two hospitalized patients with alcohol withdrawal symptoms were treated with either carbamazepine (Tegretol) or barbital ( Diemal ) in a randomized, double-blind trial. The dose of trial medication as well as the duration of treatment was individual, corresponding to the conventional treatment schedule. During the trial period daily records were kept of target withdrawal symptoms, global evaluation, the patient's subjective feeling and unwanted effects. Sixty patients completed the treatment successfully. The two treatment groups were homogeneous as regards patient characteristics, pre-treatment disease severity and drop-out rate. No statistically significant differences were found in efficacy between the two treatments, and both drugs were well tolerated. It is concluded that carbamazepine is a valuable alternative drug in the treatment of mild and moderate alcohol withdrawal symptoms.

Serum concentrations of the isomers of clopenthixol and a metabolite in patients given cis(Z)-clopenthixol decanoate in viscoleo.

Cis(Z)-clopenthixol decanoate in Viscoleo (Sordinol Depot, Cisordinol Depot, Clopixol Inj.) was given intramuscularly to nine schizophrenic patients with dosage intervals of 1 or 2 weeks. Serum concentrations of the two geometric isomers of clopenthixol and its N-dealkyl metabolite were recorded in two successive dosage intervals. Significant correlations were found for dose vs area under the serum concentration curve and vs serum concentrations measured on individual days. The last mentioned concentrations are good measures of the area under the serum concentration curve, which expresses the drug load of the patient. The serum concentration curves in two successive dosage intervals were very similar. Maximum serum concentration was seen 5-7 days after injection and the mean maximum/minimum fluctuation was 1.6 with the 2-week dosage interval. The finding of very low amounts of the trans(E)-isomers of clopenthixol and the N-dealkyl-metabolite shows that isomerization of the cis(Z)-compounds into the corresponding trans(E)-isomers does not take place within the organism.

Femoxetine - a new 5-HT uptake inhibitor - and propranolol in the prophylactic treatment of migraine.

The prophylactic effect of a 5-HT uptake inhibitor, femoxetine, was compared with that of propranolol in a double-blind crossover study of 6-months duration. 29 patients commenced the experiment. 3 subjects withdrew because of side effects and 2 because the program was inconvenient for them. In the 24 patients who continued the study to the end, the periods of propranolol (administered 160 mg daily) and of femoxetine (given 400 mg daily) differed significantly from each other with respect to the attack frequency and headache index. A significant reduction in the use of medication relieving attacks was observed during the propranolol treatment as compared with the pre-treatment period. The study showed that partial depletion of thrombocyte 5-HT uptake inhibitor did not lead to a marked improvement in headache, contrary to what might be expected on the grounds of the 5-HT hypothesis of migraine.

Mebendazole treatment of Echinococcus granulosus infection. Report of a case.

A patient with an Echinococcus granulosus cyst of the liver was treated with mebendazole for 94 days before operation. The serum levels of mebendazole varied from 39.4-274 ng/ml. After operation, cyst materials were inoculated into mice which developed hydatid cysts 10 months later. Intestinal absorption of mebendazole is poor and variable, and determination of serum concentrations is necessary during treatment. No apparently successful cases of drug treatment of E. granulosus infection have been verified by animal inoculation of cyst material; therefore, surgery must still be considered the treatment of choice. It is recommended that mebendazole be given prophylactically to prevent spread of the disease at operation.

Relief from pain localized to the lower back in early labour. A double blind trial of ketocaine, a new, highly effective, local, cutaneous analgesic (A 2358, Ane-Pad), and placebo.

Pain localized to the lower back in the first stage of labour in 47 primiparous women was treated by epicutaneous application of ketocaine or placebo compresses. Twenty-three patients were treated with ketocaine compresses (Ane-Ped), and 24 patients with compresses soaked with isotonic saline. The pain relieving effect was almost the same in both treatment groups. Pain localized to the lower back was reduced in about 50% of the patients after one hour's application of ketocaine or placebo compresses. Supplementary analgesics had to be administered in about 60% of the patients in each group. No maternal or foetal systemic side effects related to the treatment were registered. Local erythema on the application area was only reported in the ketocaine group. The frequency of feeling of warmth during the application of compresses of ketocaine were high compared to the placebo group, 78% and 29% respectively.

Visual and somatosensory evoked cortical potentials in multiple sclerosis.

The diagnostic value of the pattern reversal evoked cortical potential (VEP) and the somatosensory evoked cortical potential (SEP) has been compared in 50 patients with established or suspected multiple sclerosis. A prolonged latency of VEP was found in 96% of definite cases of multiple sclerosis, 58% of probable cases, and 20% of possible cases. A prolonged latency of SEP by stimulation of median or peroneal nerves or both was found in 86% of definite cases of multiple sclerosis, 83% of probable cases, and 50% of possibe cases. When combining the results of all three tests the diagnostic yield increased to 100%, 92%, and 50%, respectively.
