Gigantic cystosarcoma phyllodes in a man with gynecomastia.

A 70-year-old man had a breast tumor for 50 years. The tumor, which weighed 8.6 kg, proved to be a malignant cystosarcoma phyllodes. It was successfully removed by a simple mastectomy with low axillary dissection, and a skin graft was performed. The lesion was associated with gynecomastia, and it is believed to have represented a cystosarcomatous transformation of a giant fibroadenoma.

Melanomas of the lower extremity among native Puerto Ricans.

The clinical features of 57 melanomas of the lower extremity occurring in native Puerto Ricans were studied. Whereas most melanomas of the lower extremity among Caucasians occur above the ankle, the group studied showed a predilection for the foot, particularly in the minimally pigmented zones (sole, heel, and nail bed), a distribution similar to that reported in black patients. The possibility that pigment-deficient areas may represent the target organ for sunligh-induced melanomas in Negroes and dark-skinned subjects is discussed.

Sacrococcygeal dermoids and teratomas: historical review.

Palpable rectally, when not externally visible, sacrococcygeal teratomas have been recognized since antiquity, but their histologic segregation from other sacrococcygeal masses lagged into the twentieth century. That these tumors are potentially malignant, and that the incidence of this complication increases with age during infancy, was realized early this century, but it is only in the last two decades that the importance of (1) early surgery, (2) excision of the coccyx, and (3) a combined abdominosacral approach for lesions with retroperitoneal extension has been realized.

Retroperitoneal teratoma: historical review.

A case of retroperitoneal teratoma in an unusual location is presented. The tumor occurred in the right iliac fossa of a 17-year-old girl and proved fatal within 18 months, despite a seemingly adequate initial operation. The literature on the subject has been reviewed from an historical perspective.

A molded acrylic applicator guiding the peroral X-ray beam.

An intraoral acrylic mold technique to guide and maintain beam direction during oral roentgen therapy is described. It is used in combination with the treatment cone and a lead tube and protective diaphragm for collimation of the beam.

Cystic hamartoma of the liver in a pediatric patient. Angiographic findings.

A case of cystic mesenchymal hamartoma in a 2-year-old boy is presented. The overall angiographic appearance closely resembled that of the more common renal angiomyolipoma, with the addition of multiple focal avascular areas representing the cystic component of the lesion. The coexistence of these angiographic findings in a young patient with an otherwise asymptomatic upper abdominal mass should suggest cystic mesenchymal hamartoma.

Neoplastic involvement of terminal digits masquerading clinically as benign disease.

Two patients with primary and four with metastatic distal phalangeal lesions in whom a wrong diagnosis of benign disease was initially made are presented with a brief review of the literature and a discussion of the clinico-radiographic features. Primary phalangeal lesions may be either epidermoid carcinoma or malignant melanoma are are often confused with paronychia, while most metastatic lesions are due to pulmonary or breast cancer and simulate a felon clinically. Primary nail bed lesions are characterized radiographically by external cortical erosions; metastatic lesions tend to show a diffuse demineralization and extensive destruction of the whole digit.

Cutaneous lymphangiosarcoma of Stewart-Treves.

Lymphangiosarcoma of an edematous extremity is a highly malignant cutaneous neoplasm seen most often ten years after a successful radical mastectomy. The sarcoma presents with an ecchymosis and/or purplish cutaneous nodules on the edematous arm; it spreads through the bloodstream and is fatal in most cases. A case is presented which shows some typical and some unusual features of this disease.

Radiotherapy in malignant teratomas.

Twenty-seven patients with malignant teratoma, 19 of which received radiation, are presented and the radiation response analyzed. Radiotherapy seems most effective in the palliation of undifferentiated (embryonal) teratoma of the gonads and sacrococcygeal region in early childhood. It is less effective, yet useful, in undifferentiated gonadal and sacrococcygeal teratomas of older children and adults, but seems of little value in carcinomatous ovarian dermoids and in malignant mediastinal teratomas.
