The coenzyme nicotinamide adenine dinucleotide (NADH) improves the disability of parkinsonian patients.

The coenzyme nicotinamide adenine dinucleotide (NADH) has been used in an open label trial as novel medication in 34 patients with Parkinson's disease, using an intravenous administration technique. In all patients a beneficial clinical effect was observed. 21 patients (61.7%) showed a very good (better than 30%) improvement of disability, 13 patients (38.3%) a moderate (up to 30%) improvement. Concomitant with the improvement of the disability the urine level of homovanillic acid (HVA) increased significantly in all patients (in some patients by more than a 100%). The daily "on phases" of the patients could be increased from 2 up to 9 hours in the individual patients by NADH administration.

Penumbra around chronic cerebral infarction?

In a series of 13 patients with cerebrovascular occlusive disease regional cerebral blood flow (rCBF) measurements (two-dimensional intravenous 133Xe clearance method) and quantitative EEG analysis (sensorimotor rhythms) as well as electronic measurement of handforce were performed before and during intravenous infusion of 1 microgram kg/min of one of the lipophilic dihydropyridine calcium channel blocker nimodipine (Nimotop). The aim of the study was to test the hypothesis of the existence of hypoperfusion (ischaemic penumbra) in the surroundings of chronic cerebral infarcts. All 3 parameters improved in one patient. Sensorimotor rhythms increased in 5 patients, rCBF in 3. EEG and rCBF improved in 2 patients. In 3 instances, a redistribution of rCBF in favour of the peri-infarct zone was noted (significant increase of rCBF from 35 +/- 2 SEM to 53 +/- 4 ml/100 g/min (p less than 0.01), whereas rCBF fell from 61 +/- 5 to 46 +/- 2 ml/100 g/min on a collimator remote from the infarct but in the infarcted hemisphere. The parallel improvement of rCBF and EEG in brain regions surrounding chronic infarcts in 3 patients was interpreted as functional improvement as a consequence of nimodipine-induced normalization of peri-infarct hypoperfusion, i.e. reversal of flow-dependent neuronal silence and/or dysfunction.

Hemorheologic treatment of cerebral reversible ischemic episodes with pentoxifylline--a prospective study.

One hundred patients (49 males, 51 females) with reversible neurologic deficit due to cerebral ischemia of vascular origin in the carotid artery territory were recruited for a long-term observation to follow up recurrence of ischemic events. Fifty-eight patients (mean age sixty-four years) were treated daily with 1200 mg oral pentoxifylline in addition to basic therapy (antihypertensives, antidiabetic drugs, etc), and 42 matching patients (mean age sixty-two years) of a control group had no pentoxifylline or other hemorheologic medication. The mean observation period was fifty-six months (range thirty-six to sixty). On admission patients presented with increased platelet aggregation and/or impaired red cell filterability and with enhanced red cell aggregation. Five patients in the pentoxifylline group (8.6%) and 16 control patients (38%) suffered a relapse of an ischemic episode. These data support previous reports of a beneficial effect of pentoxifylline in the prevention of cerebral ischemic events.

Present state of hemorheology.

Hemorheological disturbances are present in more than 50% of cerebrovascular disease (CVD). An inverse relationship between cerebral blood flow (CBF) and viscosity has been established. There also exists an interaction of vessel wall changes, whole blood viscosity, and plasma fibrinogen. The acute ischemic episode is associated with hyperaggregability of the blood platelets, increased blood viscosity, hyperaggregability of the red cells and an increase in plasma fibrinogen. Hemorheological changes to a certain extend may occur not only in CVD but also in senile dementia of Alzheimer type (SDAT) and certain cases of Parkinson's disease. This fact allows to differentiate between idiopathic parkinsonism and Parkinson's disease with high hemodynamic risk.
