The speckled immunofluorescence of epidermal nuclei as an in vivo phenomenon.

The deposition of speckled-patterned immunoglobulins in epidermal nuclei has been investigated in seventeen patients with various diseases other than mixed connective tissue disease. The phenomenon was seen not only with low titres of circulating antinuclear antibodies of IgG and IgM classes, but even in their absence. The clinical significance of this finding remains obscure, but the common explanation, that it is a simple artifact, seems quite untenable.

Urticaria-like follicular mucinosis.

Two middle-aged men with a cyclic eruption of transient urticarial papules of the face are described. Deposits of mucinous material were observed in cycstic spaces in their hair follicles. In direct immunofluorescence cells bearing IgE and C1q were found scattered in the dermis and were identified as mast-cells. Both patients experienced some relief of their disease in summer.

Dermatobia hominis infestation.

A patient is reported who, after leaving Venezuela, developed some boils on the left upper limb inhabited by Dermatobia hominis larvae. The curious life-cycle of this tropical fly is described with some considerations about the diagnostic problem. A simple unreported way of larvae extraction is suggested.

Immunofluorescence findings in haematoporphyrin-induced keloid.

A case of haematoporphyrin-induced keloid is described in which immunofluorescence (IF) revealed antibodies eluted from circulating lymphoid cells directed against fibroblasts. The antibodies eluted belonged to the main Ig classes IgD and IgM and they did not bind complement in vitro. Their possible activity in stimulating fibroblasts is discussed.

[Hypokalemic myopathy in arterial hypertension].

Three cases of arterial hypertension and severe hypokaliemia are reported; in two cases an acute rhabdomyolisis was also observed. The syndrome was iatrogenic in two cases depending on the licorice eating and the combined use of diuretics and a 9-alpha-fluoroprednisolone-containing nasal spray in one patient, and on a long-term use of the same nasal decongestant in the other. In the patient 2 the persistence of low PRA and aldosterone levels seems to indicate an excess of mineralcorticoid activity other than aldosterone. The syndrome of factitious mineralcorticoid excess should, therefore, be considered in the differential diagnosis of arterial hypertension with hypokaliemia; the risks of a long-term administration of steroid-containing nasal sprays, responsible of serious side effects, should also be stressed.

Immunopathological studies on a case of Sweet's syndrome.

Immunopathological studies in a case of Sweet's syndrome revealed IgM- and IgE-bearing polymorphonuclear leucocytes in involved skin when the direct immunofluorescence technique was used. Circulating polymorphonuclear neutrophils showed defective chemotaxis with normal random migration, phagocytosis, killing, and hexose monophosphate shunt activity.

Acute diffuse scleroderma.

A case of acute diffuse scleroderma without contemporaneous visceral involvement is described. Antibodies to endothelial cells have been found in the skin.

[The amenorrhea-galactorrhea syndrome. Clinical and therapeutic aspects of 5 treated cases].

The behaviour of plasma prolactin in response to various pharmacological tests has been examined together with that of other hypophyseal function parameters in five patients suffering from amenorrhoea-galactorrhoea syndrome. Evaluation of plasma prolactin and hypophyseal tropinic reserve levels did not prove sufficient to discriminate between functional and organic forms of hyperprolactinaemia. A woman patient was submitted to neurosurgical operation for removal of a hypophyseal microadenoma. The other patients were treated medically with 2-alpha-bromoergocryptin which produced quick normalization of prolactinaemia with restoration of normal memstrual cycles and cessation of galactorrhoea.

Immunopathological studies on alopecia areata.

Anti-endothelial cell antibodies could be removed from circulating lymphocytes by means of acid elution techniques in eight patients with different degrees of alopecia areata. These antibodies were specifically directed against the endothelial cells in the capillary network of the hair bulb, indicating the existence of an antigen, which is unique to these particular endothelial cells. These antibodies do not bind complement "in vitro" and are species-specific. Circulating ANA (speckled type) were only noticed in case with alopecia areata in spots. A significant decrease in circulating T cells was noticed in six of eight patients with a certain degree of alopecia.

Immunopathological studies on rosacea.

Seven patients with papulo-pustular rosacea were investigated with immunofluorescence (IF) techniques. Indirect IF has been performed also with antibodies eluted from circulating lymphoid cells. Besides confirming the data in the literature on the positivity of the basal zone, anticollagen antibodies were found, and eluted antinuclear antibodies were detected against nuclei of cells in the epidermis and dermis, namely, scattered dermal, endothelial and eccrine duct cells.
