[Subacute lupus erythematosus of the skin--clinical aspects and laboratory findings].

UNLABELLED: The data of 20 patients with subacute cutaneous lupus erythematosus (SCLE) were evaluated according to clinical and laboratory criteria. We found the following laboratory deviations: elevated BSR, leucopenia, anti-dsDNA, anti-SSA/Ro and anti-SSB/La antibodies, positive LE cells, and elevated levels of IgE and immune complex. Aside from the characteristic skin eruptions in SCLE, the patients showed arthralgia, myalgia, and pronounced photosensitivity. THERAPY: corticoids, chloroquine, and light protection. Regular follow-up examinations are extremely important in these patients.

Plasmid analysis of clinical isolates of Bacteroides fragilis group strains.

Cryptic plasmids were isolated from 26 of 52 Bacteroides fragilis group strains derived from severe infections. Four strains harboured two plasmids, one three plasmids and one five plasmids with different molecular weights. The same molecular weight plasmid (3.7 Md) was isolated from 17 of the 26 plasmid-containing B. fragilis group strains. No correlation was found between plasmid-harbouring and resistance against ten antibiotics and different heavy metal ions. No curing of the strains from the plasmids was achieved with ethidium bromide and acridine orange.

[Discoid lupus erythematosus beginning on the eyelids].

We report on a 20-year-old women, who suffered from discoid lupus erythematosus (DLE) beginning on her eyelids with dyschromia and cutaneous infiltration. Similar signs appeared on the top of her nose, her nostrils, and her lower lip. Simultaneously with these alterations, we found a superficial exulceration on her hard palate, which histologically proved typical for DLE. After therapy with chloroquine for 3 weeks, the alterations disappeared.

Incidence of rhythm disorders in hyperthyrosis with special respect of old age form.

Sinus tachycardia and atrial fibrillation are frequent features in hyperthyrosis while sinus node dysfunction is regarded as a rare complication. Bradycardia may cause diagnostic problems mainly in atypical hyperthyrosis of the old age. The authors analysed distribution and age related association of the rhythm disorders in hyperthyrosis. In case of the appearance of Sick Sinus Syndrome (SSS), parameters representing the function of sinus node were studied by electrophysiological investigations. Above the age of 50 years incidences of atrial fibrillation and SSS were significantly increased. The abnormal sinus node function proved to be reversible in a portion of the cases. In old age, in case of occurrence of the symptoms of SSS, possibility of hyperthyrosis also should be considered, especially when indication of permanent pacemaker is established.

Expression in Escherichia coli of the major outer capsid protein of infectious pancreatic necrosis virus.

The outer capsid polypeptide, VP2, represents the major neutralizing antigen of infectious pancreatic necrosis virus (IPNV). A 926-bp viral cDNA, encoding an N-terminal truncated VP2, was cloned into the pWR590 expression plasmid family resulting in a C-terminal extension of a truncated Escherichia coli beta-galactosidase (beta Gal) under the control of the lac promoter. When cells transformed by in-phase hybrid plasmids were induced by isopropylthiogalactoside, high levels of the 100-kDa beta Gal-VP2 fusion protein accumulated within 4 h after induction. The fusion protein reacted in Western blots both with rabbit anti-beta Gal and with neutralizing mouse anti-VP2 monoclonal antibody. Sera of rabbits immunized with semipurified fusion protein reacted with the VP2 polypeptide in Western blots and with intact purified virus in ELISA and also neutralized IPNV infectivity in a plaque-reduction assay. Out-of-phase hybrid plasmids did not produce the fusion protein but expressed a small amount of structurally discrete VP2-specific sequences probably by internal initiation of translation at an in-phase AUG codon near the 5' end of the VP2 gene.

[Forme fruste of Hunter's disease].

We report on a 19-year-old girl with hepatosplenomegaly and possible hematological disease. We suspected Gaucher's disease on account of histological and biochemical evidence found in specimens from the liver, spleen, and bone marrow. 18 months later, pebbled skin developed on her neck and upper back. Histological examination revealed large amounts of mucous material between the collagen bundles deep in the dermis, which proved to be dermatan sulfate. The clinical and histological symptoms are characteristic for Hunter's disease.

Pituitary dependence of bone marrow function.

The anaemia, leucopenia, thrombocytopenia and impaired DNA and RNA synthesis in the bone marrow of hypophysectomized rats could be restored by syngeneic pituitary grafts placed under the kidney capsule, or by treatments with ovine or bovine prolactin or growth hormone. Treatment with ACTH, FSH, LH and TSH had no effect in this respect. These results indicate that bone marrow function is regulated by the pituitary gland.

Routine prenatal screening policy of fetal malformations by both maternal serum alpha-fetoprotein and ultrasound in eastern Hungary.

Since January 1979, at our Genetic Counselling Unit in Debrecen all pregnancies at high risk for any reason have been screened sequentially by serum alpha-fetoprotein estimation (at the 16th week) and by ultrasound examination (18th week). This screening policy covered 1200-1500 consultations per year. From July 1983, the same prenatal screening policy has been extended to cover the whole pregnant population of two large counties in Eastern Hungary. In a six-year period over 300 fetal malformations were diagnosed. Details of the screening policy and the main groups of malformations diagnosed prenatally are demonstrated.

Experience with low osmolar contrast media a clinical and experimental study.

Low-osmolar contrast media were used in 546 patients: 371 with ioxaglate, 80 with iopamidol, 95 with iohexol. The image quality was good in all cases. Heat was observed in one patient (iopamidol) and pain was observed in 7 patients (ioxaglate: 4, iopamidol: 1, iohexol: 2). Blood pressure, heart rate as well as the parameters, BUN, SGOT and serum bilirubin showed no significant changes. Our experimental investigations were performed on 101 dog kidneys. Selective renal angiography was performed with 10 ml of contrast media metrizamide, ioxaglate, iopamidol, or iohexol. Comparative preparations were the high-osmolar contrast media iodamide and metrizoate. Untreated kidneys and kidneys given saline constituted baseline controls. Histological examination of tissues 3 and 24 hours after the injections showed moderate reversible changes (metrizamide 15/21, ioxaglate 5/17, iopamidol 2/18, iohexol 2/26). Electronmicroscopy (6 iohexol) showed no significant changes.
