Assessment of immunological mechanism in infertility of the rat after experimental testicular torsion.

A necrotic torted testis left in the scrotum affects the contralateral testis, resulting in depressed fertility. It was suggested that testicular torsion provoked a systemic response of an immunologic nature. In order to prove this theory, 56 rats were operated. Ten had a sham operation, in 17 a testis was torted and excised after 24 h, and in 29 a testis was torted and excised after 24 h, and in 29 a testis was torted but left in situ till the sacrifice. Antisperm-antibodies were not found in any phase of the experiment in the sham group. Using indirect immunofluorescence, antisperm-antibodies were found in the sera of 8/17 rats of the orchiectomy group and in 24/29 of the detorted group. In the direct immunofluorescence study, all the antibodies were localized in the tail of the spermatozoid. The antisperm-antibodies disappeared after 3 to 6 months.

[Sarcomatoid carcinoma of the kidney].

Sarcomatoid renal carcinoma is an uncommon renal malignancy with an aggressive biological behaviour and very poor survival rate. We treated lately 4 patients, three of which died shortly after the operation. The case reports are presented and the histology and the clinical implications are discussed.

[Treatment of ureteral obstruction by a double pig-tail catheter. Our experience].

The authors' experience is based upon 39 patients treated for ureteric obstruction between March 1980 and March 1984. The obstruction was of neoplastic origin in 7 cases. In 5 cases the double pig tail catheter was inserted peroperatively as a urinary drainage method. The catheter was inserted endoscopically in 10 cases and percutaneously in 9 cases. In 15 cases a temporary or permanent nephrostomy was necessary. Indications and complications of the method are analysed and the results compared with those in the literature.

Computerized tomography in xanthogranulomatous pyelonephritis.

Computerized tomography offers the means to recognize changes in the kidney associated with xanthogranulomatous pyelonephritis. The preoperative extent of the inflammatory process, reaction of adjacent parenchymal structures, and extension to the muscles and abdominal wall are identifiable on computerized tomography. The presence of obstructing renal stones, thickening of the perirenal fascia and inability of the kidney to handle intravenously injected contrast medium can be assessed with computerized tomography. Low density renal areas measured on computerized tomography may indicate lipid infiltration of the kidney or incorporation of the fat of the renal bed into the tumefactive mass.

[Persistent gluteal pain after embolization of the hypogastric arteries. An unexpected complication].

Ligation or embolization of the hypogastric arteries for control of intractable hemorrhage from the bladder has been used in the past with equal results and without ill effect. Gluteal pain following hypogastric embolization is common and was reported to be resolved in less than 48 hours. A 57 year old man in our ward underwent a bilateral hypogastric embolization. The bleeding decreased considerably but a severe gluteal pain and claudication ensued. The pain persisted over a month necessitating frequent Morphine injections. We propose to use superselective catheterization technique in the future sparing the superior gluteal artery.

[Tomodensitometry in the diagnosis of perinephritic phlegmon].

Twenty-nine patients with perinephric abscess were treated during the last 15 years. Twenty-seven abscesses were cured by drainage performed as soon as the diagnosis has been made. One patient died from septicemia in spite of the treatment. One patient died because the correct diagnosis was not revealed until the autopsy. Ten patients had a nephrectomy performed on them later on. Staphylococcus and gram negative bacilli were equally isolated. Stones were present in twenty patients. Eleven patients were diabetic. The clinical picture is not always clear, and diagnosis might be difficult. The different radiological signs help to reach a correct and prompt diagnosis. To these classical signs we can add during these last years Gallium-67 radionuclide studies, ultrasound and computerized tomography, each of these having its indication and usefulness.

Assessment of fertility after testicular torsion: an experimental study.

The spermatic cord was ligated in Charles River adult rats producing viable reversible and non viable damaged testes. Weight of the rats, weight of the testes, weight of the epididymis, sperm motility, sperm concentration and fertility were the different parameters assessed. The results showed that there is a high percentage of subsequent atrophy in the ligated testes, and that there is a definite correlation between the length of the occlusion and the amount of testicular injury sustained. Leaving a ligated testis in situ for three months provoked a severe reduction in fertility and some lesser reduction in the sperm motility in the contralateral testis. This damage was not observed in the rats where the ligated testes were removed.

[Scrotal leiomyosarcoma].

A case of leiomyosarcoma of the scrotum in a 65 year old man is presented. It is, we believe, the sixth case reported in the literature. A hemiscrotectomy was performed with preservation of both testes. Three years after the operation the patient is well and free of metastases. The electron microscopy study is reported and the features are discussed.

Leukaemic infiltration of the prostate.

A biopsy of the prostate should be done in lymphoma/leukaemic patients suffering from urinary obstruction. Patients with a lymphomatous infiltration of the prostate should be irradiated and not subjected to surgery. Two such cases successfully irradiated are described and the present literature is reviewed.

Osteitis pubis: treatment by heparinisation.

Three cases of osteitis pubis following prostatectomy, herniorrhaphy and combined prostatectomy and herniorrhaphy are presented. Each case was successfully treated by intravenous heparin after other treatments had failed.

Infarction of renal tumors using isobutyl-2 cyanoacrylate and lipiodol.

Percutaneous transcatheter embolization of the renal artery in clear cell carcinoma was performed in nine patients. The occlusive material used was a suspension of 1 ml of isobutyl-2-cyanoacrylate in 3 ml of Lipiodol. Six patients were operated on 2-5 days after the embolization, and in three patients the procedure was palliative. In one of the latter, follow-up arteriography 4 months later showed that the artery was still occluded. There were no complications attributed to the glue or to oil emboli in this series.

Histoacryl in sutureless uretero-ureteral anastomosis: an experimental study.

Uretero-ureteral anastomosis can be compromised by tissue reaction to the suture material, extravasation of urine, and periureteral inflammatory reaction at the anastomotic site, resulting in structures. A sutureless, urine-tight anastomosis, therefore, would be the ideal solution, n-butyl 2-cyanoacrylate was used in the sutureless uretero-ureteral anastomosis in 10 dogs and resulted in a good anastomosis with no untoward reactions.

[Blind-ending branch of bifid ureter (author's transl)].

A bifid ureter with a blind-ending branch is rare. There is only six published cases in which was the exact diagnosis established on the basis of excretory urography alone. In all other cases the diagnosis was established by retrograde pyelography, at an operation or at autopsy. Asymptomatic cases may go undetected. Symptoms, if present, may be non-specific (vomiting, fever, pain, dysuria, hematuria) arising from stasis within the blind lumen. Almost all reported cases arose from the distal third of the ureter. In our present case, a woman aged 56, reflux of the contrast material injected intravenously outlined the entire course of the blind-ending branch. This blind-ending branch was resected resulting in the cure of the patient.
