A study of 100 consecutive admissions to a psychogeriatric unit.

A series of 100 consecutive admissions to a psychogeriatric unit in a large public psychiatric hospital is described. Fifty-four of these were readmissions. Most of the patients presented with moderate to severe acute psychiatric disorders, and many of these were admitted under detention orders. Their disorders were often readily treatable and most patients returned to their local communities in less than three weeks. Unexpectedly, only one-third of patients had a primary diagnosis of organic cerebral disorder, and few had received a diagnosis from the referring doctor. The principal reason for the referral of patients was that they were obviously severely disturbed and disturbing within their community. Patients who do not cause a disturbance may thus be underdiagnosed and their treatment unnecessarily compromised. The traditional system of health care seems to be working well and in the interests of these psychogeriatric patients and there is no evidence that radical changes are needed.

[Fatal massive intravascular coagulation during phlebography. Responsibility of epsilon-aminocaproic acid?].

The occurrence during phlebography of massive intravascular coagulation involving the inferior vena cava, the right cardiac cavities and the entire pulmonary arterial network suggested that epsilon-aminocaproic acid administered before phlebography was responsible for this complication. The mechanisms of reactions to contrast media are still poorly understood, but these reactions are known to include, in some cases, disseminated intravascular coagulation. This phenomenon was histologically demonstrated in our patient, and it may be suggested that its strong enhancement by the antifibrinolytic agent had resulted in the fatal accident. In view of the possibility of such lethal reactions, the advisability of administering epsilon-aminocaproic acid--a drug widely used in France for the prevention and treatment of contrast media reactions--should be reconsidered.

Changes in psychological diagnosis and prescription in a practice employing a counsellor.

This study investigated the effect on consultation rates and psychotropic drug prescribing of employing a counsellor in a general practice. No major change was detected in the number of prescriptions for psychotropic drugs or the consultation rates during the years that counselling was available. An increase in the number of psychotropic drug prescriptions in the year after counselling was accounted for by a very small number of patients. Three years after counselling more than three-quarters of the patients who had been counselled were receiving no medical attention for psychiatric problems. This may either reflect the effect of counselling over a longer period or the natural history of the problems.

Assessing the needs of the elderly using unsolicited visits by health visitors.

Thirty-five percent of patients over 70 years old who received an unsolicited visit by a health visitor were found to have previously unidentified needs. In all, 71% of the 110 patients surveyed were found to have problems involving mobility, communication or caring for themselves. Most of their needs were social ones. It was concluded that unsolicited visiting by health visitors was efficient use of time by members of the primary health care team. Changes in practice administration are suggested to improve access to medical care for elderly patients.

[HPLC studies on the distribution behavior of guanyl- and N-phenylguanylhydrazone derivatives].

The article in hand presents first HPLC studies on the distribution behaviour of guanylic and N-phenylguanylic hydrazone derivates. The chromatographic parameters of compounds, which had been separated at NH2-columns by 0.06 mol X 1(-1) Na2HPO4 buffer/methanole (30 : 70), could be identified, and the k'/rates could be utilized for the calculation of the coefficient of distribution. As a result of performed calculations of the correlation, it could be proved that the log. P rates which had been ascertained by the HPLC method, been determined by the shaked flash method and been calculated acc. to Leo, correlate to the 1% significant level.
