[Orthostatic syndrome in old age. Frequency and therapy (author's transl)].

Largely confirmed Caird's figures. Using Schellong's test we recorded systolic pressure falls of 20 mm Hg and more in 25% of 160 residents in an old people's home in the age range 59-88 years. These cases of circulatory dysregulation were all manifestations of the hypodynamic form of the orthostatic syndrome. This means that only systolic blood pressure fell, diastolic pressure as well as heart rate being largely unaffected. The phenomenon appears to be due to age-related deterioration in circulatory regulation; at least no evidence has been found to suggest that it has a specific aetiology. In a double-blind cross-over study the effect of dihydroergotamine (DHE) was compared to placebo in 40 patients. Half the patients were given 2 mg DHE three times daily for 14 days and the other half placebo tablets; following one week's wash-out, the treatments were then reversed for a further 2-week period. Schellong's test (measurement of blood pressure and pulse after 10 minutes in the supine position and then after 1, 2, 5 and 10 minutes standing) was performed at the beginning and end of each treatment period. The average falls in upright systolic pressure for all 40 patients were as follows: before treatment 20 mm Hg, end of placebo periods 19 mm Hg, end of DHE periods 7 mm Hg. The differences between the values recorded after treatment with DHE and the other 2 values are statistically significant.

[Anterior pituitary regulation of testicular function in the elderly (author's transl)].

Pituitary gonadotrophin regulation was examined with the LH-RH test in 15 men (aged between 55 and 90 years), five of them aged between 71 and 90 years. Compared to normal mature males, the plasma testosterone in these subjects was reduced or was in the lower range of normal, while basal LH levels were increased in half the cases. However, basal FSH values were increased in three only. In most of these subjects with increased basal values of LH, the LH-RH stimulation caused a paradoxical temporary reduction in LH release which did not occur with FSH. This paradoxical response also did not occur when basal LH values were normal.

[Complications of selective coronary angiography and the choice of contrast media].

The incidence of side effects of two contrast media containing different amounts of sodium (Urografin 76 and Conray 70) is compared in two groups of patients (totalling 133) under-going diagnostic examination by means of a cardiac catheter and selective angiocardiography. The following results were obtained: 1. Transient ischaemic reactions were observed in 4 patients of the Urografin group. Cerebral circulation was temporarily restricted in 2 patients, but there were no sequelae. One patient with a history of repeated cardiac infarction in whom the examination was the last hope with a view to coronary surgery died immediately after the angiocardiography. 2. In the Conray 70 group, ventricular fibrillation occurred 32 times in 22 patients after a total of 155 injections. Protracted asystole was observed in 2 patients. The explanation offered for the divergent findings from the use of the two contrast media is not the different anions--the iodine hosts--, but rather the difference in the amount of sodium contained in the preparations. It is concluded from the studies that contrast media which deviate to any great extent in their sodium content from the physiological range of the blood sodium concentration are unsuitable for angiocardiography.

[Corticofugal influences of somatosensory areas of the cortex on signal transmission in the posterior ventral nucleus].

Acute experiments were conducted in cats under nembutal anesthesia. A study was made of responses in the thalamo-cortical fibers coursing to the first somato-sensory area of the cortex to the electric stimulation of the fibers of the medial lemniscus. Application of penicillin to the somato-sensory cortex and development of epileptoid activity considerably diminished the amplitude of the responses in the thalamo-cortical fibers. The first somato-sensory zone proved to be much more effective than the second one. The appearance of the epileptoid activity in the projection cortex considerably diminished the inhibitory process in the posterior ventral nucleus; this was revealed by giving paired stimuli to the medial lemniscus.

[Clinical experiences with a new ready-made lidocain-electrolyte solution for intravenous permanent therapy of ventricular arrhythmias].

Efficacy, tolerability and adverse effects of a ready-for-use lidocain-electrolyte infusion solution for the therapy and prophylaxis of ventricular arrhythmias were tested in routine clinical use. The solution proved highly efficacious and was well tolerated. Even in long-term administration (mean 5 days), no disturbances of the water and electrolyte homeostasis or any other complications were observed.

[Changes in the acid-base equilibrium in hypokalemic patients during potassium infusions with various anions].

Based on the parameters of the acid-base status, the effect of different anions in potassium and magnesium substitution was studied. A mixture of chloride with the metabolisable anions acetate and malate does not cause any changes in the acid-base balance. The exclusive supply of metabolisable anions in the form of aspartate always results in a change of blood pH values towards an alkalosis.
