Platelet adhesiveness: methodology for testing in humans.

Platelets play an important role in the pathogenesis of early thrombus formation and might even be involved in initiating the process of artherosclerosis. Various platelet adhesiveness test methods are discussed. Results of an in vitro test in about 600 patients with various diseases are given. The effect from anti-platelet drugs like dipyridamole and acetyl salicylic acid in preventing increase of platelet adhesiveness due to a biologic stressor are compared with those obtained with isoxsuprine. Platelets play an important role in the pathogenesis of early thrombus formation. Platelets might also be involved in initiating the process of atherosclerosis. Therefore, it is understandable that various test methods to measure platelet behavior has been developed.

[Quantitative laser-nephelometric detection of rheumatoid factor, using IgG coated latex particles (author's transl)].

The laser-nephelometric test (Latex RF test) detects rheumatoid factor activity, and is based on immune complex formation between IgG coated latex particles and rheumatoid factor. This test requires little time and gives qualitative results in good precision. The latex nephelometric test and the frequently used agglutination tests (Waaler-Rose, Latex agglutination test) give the same qualitative results. In the quantitative assay higher nephelometric results correlate on average with higher titers in the Waaler-Rose and Latex agglutination test. The high standard deviation could be explained by the fact that other factors (IgG rheumatoid factor, IgG and IgG-IGM immune complexes), in addition to the IgM rheumatoid factor concentration, influence the immune complex formation in vitro.

[Diagnosis of systemic lupus erythematosus (author's transl)].

The positive demonstration of ds-DNA in a particular concentration is a diagnostic criterium for systemic lupus erythematosus. The most commonly used test at present has the disadvantage that radioactively marked substances are required. Thus a test method was developed for the laser nephelometer which is simple and which detects the antigen-antibody complex directly. First measurements on sera from SLE patients showed a good correlation with radioimmuno assay.

[Effects on platelet functions].

Platelet adhesiveness was measured in a total of 589 healthy volunteers and patients. Patients suffered from heart failure, diabetes mellitus, myocardial infarction and deep vein thrombosis have a significant higher platelet adhesiveness as healthy volunteers. The effect of the socalled stressors on platelet adhesiveness was shown in vivo; the same values of platelet adhesiveness were seen as in patients. Therefore it can be concluded that stressors constitute a risk factor in patients with altered vessel walls.

Platelet kinetics in Crohn's disease.

The pathogenesis of thrombocytosis in Crohn's disease was analized in ten non-treated patients using the 51Cr labelling method. The data obtained were compared to those taken from a control group. Besides platelet survival time, platelet turnover, surface radioactivity, platelet adhesiveness and morphological aspects of bone marrow were investigated. Platelet survival time in patients with Crohn's disease proved to be significantly shortened (p less than 0.001), whereas platelet turnover appeared augmented. No additional radioactivity is encountered from the organs scanned. The relative platelet adhesiveness is diminished while thrombocytopoesis in bone marrow is augmented.

Human-pharmacological investigations on the platelet adhesiveness- and aggregation-inhibiting effect of EMD 26 644, an oxazolyl-thio-propionic acid derivate.

EMD 26 644, a compound of novel chemical structure and with platelet aggregation-inhibiting properties, was investigated in 2 controlled clinical pharmacological studies involving 42 normal volunteers. In the first study with 22 subjects exhibiting normal or spontaneously increased platelet aggregation the influence of 1 X 250 mg EMD 26 644 p.o. on the platelet aggregation and adhesiveness was examined ex vivo. In the second study 20 subjects with and without a premedication of 1 X 250 mg EMD 26 644 were exposed an artificial climate with special platelet-irritating properties. A single oral dose of 250 mg EMD 26 644 markedly and significantly inhibited the platelet aggregation and adhesiveness over a period of several days.

[The effect of antiaggregating agents - acetylsalicylic acid (ASA) and dipyridamol - on the rejection of allotransplanted canine kidneys in presensitized animals (author's transl)].

The effect of antiaggregating agents - acetylsalicyclic acid (ASA) and dipyridamol - on the rejection of allotransplanted canine kidneys in presensitized animals were studied. Blood flow dropped more markedly in the control group than in the group treated with ASA and dipyridamol. 24 hours after transplantation the blood flow in this group was significantly higher than in the control group (p 2,5 %) and in the third group treated with dipyridamol alone. The cellular rejection started earlier and was more pronounced in the control group. Only in this group vascular rejection and microthrombi were demonstrated. Urine output ceased 24 to 60 hours after transplantation. The immunosuppressive, antiphlogistic and platelet antiaggregating effect of the combined ASA and dipyridamol therapy is thought to be responsible for the better results in this group. In this experimental model the combined treatment of ASA and dipyridamol results in a later onset of cellular rejection, in better blood-flow of the kidney and later onset of anuria.

[Plasminogen estimation as a routine test (author's transl)].

A simple method for the routine determination of plasminogen using the coagulation time is described. 0.2 ml of plasminogen reagent and 0.1 ml of sample plasma, prediluted 1:200, are incubated for 3 minutes at 37 degrees C. 0.05 ml of diluted test thrombin are then added and the coagulation time is measured. It is converted into percent of normal using a standard curve. The test correlates with an immunological method in healthy probands (r = 0.96). Initial measurements in 160 patients show that knowledge of the plasminogen titre may also be of interest outside the problems of haemostasis.

Platelet adhesiveness and cancer cell stickiness: a comparative study.

Comparing platelet adhesiveness and cancer cell stickiness under the conditions of the so-called Fibre test showed that: 1. Platelets aggregate in the first few seconds only, whereas Yoshida sarcoma cells accumulate continuously. 2. Platelets show a characteristic velocity profile. Platelet adhesiveness shows a maximum at a velocity of 7 cm/sec. Yoshida sarcoma cells adhere better at low flow velocity. 3. An experimentally induced electric current (streaming electric current) of ca. 40 mV augments platelet adhesiveness to twice the basal value, whereas Yoshida sarcoma cells remain uninfluenced by the current. These results suggest an interaction between platelets and vessel wall, whereas Yoshida sarcoma cells adhere passively.

[The effect of acetyl-salicylic-acid and a pyrido-pyrimidine-derivate on the rejection of allotransplanted canine kidneys (author's transl)].

The effects of anti-thrombotic drugs-Acetyl-salicylic-acid and 2-Piperazinly-4-thiomorpholino-pyrido(3,2-d)-pyrimidin-sulfat-trihydrat-on rejection reactions of renal allografts in dogs have been studied. From the clinical point of view no effectiveness of the two drugs can be found, whereas the histological findings do clearly show diminished rejection reactions in the Acetylsalicylic-acid group.
