A serological comparison of 4 Japanese isolates of porcine enteroviruses with the international reference strains.

The reference strains of four serotypes (J6, J8, J9 and J10) of porcine enterovirus isolated in Japan were compared with the international reference strains of 11 serotypes (W1 to W11) by cross neutralization tests. No cross reactions were observed between the two groups, although there were minor one-way crosses between W10 and J10, W11 and J9, and W4 and J9. This leads to our conclusion that all 4 Japanese serotypes can be newly added to 11 international serotypes. J9 virus produced type 1 of CPE (CPE I), J6 and J8 did CPE II, and J10 did CPE III. J10 grew in Vero, HeLa cells, and the primate cells, but J6, J8 and J9 did not.

Relation of serological- and CPE- classification of porcine enteroviruses to the classification by immunoperoxidase (IP) staining, and observation of CPE by IP staining method.

Characteristics of CPE produced by porcine enteroviruses (PEV) were examined by Immunoperoxidase (IP) staining method. Viral antigens were detected earlier than appearance of CPE. Distinctive characteristics of the three CPE types were clearly showed by the method. In cross reactions by IP staining, the titers of the staining were high (1:6,400 to 1:25,600), even though neutralizing titers of PEV with CPE II or III were low (1:200 to 1:400). The very close relationship was detected between PEV with the same CPE type, but the very low relationships were detected between PEV with the different CPE type. The relationships in CPE I group were various. The results by IP staining were more relative to CPE type than the serotype. Thus, IP staining is one method to classify PEV clearly.

Sero- and CPE-types of porcine enteroviruses isolated from healthy and diarrheal pigs: possible association of CPE type II with diarrhea.

Porcine enteroviruses (PEV) were isolated from feces of healthy and diarrheal domestic Japanese pigs, and examined for their serotypes. Each virus of the isolates corresponded not only to the Japanese version of the reference serotype, but also to its corresponding world version of the reference serotype. Twenty-two samples (75.9%) of 29 isolates from the feces of healthy pigs exhibited a characteristic of CPE I. On the other hand, 69.2% (27/39) of all diarrheal pigs showed a characteristic of CPE II. This would suggest the PEV with CPE II has a closer association with the enteric pathogenicity than that with CPE I.

[Two different spinal tumors (meningioma and schwannoma) with von Recklinghausen's disease in a case].

Von Recklinghausen's neurofibromatosis is a congenital anomaly due to maldevelopment of the neurectoderm and mesoderm. The disease is known to be commonly associated with other tumors of the neural system. We experienced a case in which two different types of spinal tumors (meningioma and schwannoma) appeared together with Von Recklinghausen's disease. The patient was a 54 year-old female. Her chief complaint was slowly progressive lumbar back pain of about a 10-year duration. She was admitted to our hospital after developing such clinical symptoms as paraparesis, bladder and rectal incontinence, and pain in the soles of her feet. The CT study demonstrated a massive tumor at the TH12-L2 levels accompanied with marked scalloping of the posterior vertebral body. The tumor removed was, histologically, meningioma. During surgery, another small bean-size tumor was incidentally found originating from the dorsal root, and it was identified by histological study as typical schwannoma. It has been reported that the incidence rate of spinal tumors in von Recklinghausen's disease is approximately 4%. Although a case has been reported in which different types of tumors developed in both the cranium and the spinal canal, our case as described above is considered extremely rare as far as we can find in reference literature. The pathogenic mechanism of vertebral scalloping as encountered in our case was thought to be based on interactions between hypoplasia of the supporting tissue and pressure generated by the tumor and CSF.

[A case of synchronous multiple primary cancers of the stomach and kidney].

In line with an increase in the incidence of multiple primary cancers, we have encountered a case of synchronous multiple primary cancers of the stomach and the left kidney. The patient, a 69-year-old male, visited our hospital after experiencing epigastric discomfort for three months. An advanced gastric cancer, Borrmann III type, was detected by endoscopic examination. Preoperative abdominal computed tomography also revealed a large low density mass occupying the upper part of the left kidney. On angiography, the left kidney showed a hypervascular mass, showing pooling and tumoral stains, thereby suggesting a renal cell carcinoma. The patient thus underwent a subtotal gastrectomy with an R2 lymph node dissection and a left radical nephrectomy. Histologically, the gastric lesion was a poorly-differentiated adenocarcinoma and the left renal lesion was a renal cell carcinoma of the clear cell type.

Age-dependent decrease in the computed tomographic numbers of parotid and submandibular salivary glands.

The computed tomographic (CT) numbers of 182 normal parotid glands were measured in patients ranging in age from 1 to 81 years, and of 172 normal submandibular glands with an age range of 1-85 years. The mean CT number (+/- s.d.) of the parotid and the submandibular glands was -10 +/- 24 Hounsfield units (HU) and 41 +/- is HU respectively. Sex and intra-individual differences in the CT numbers were small, but significant linear decreases were found with age in both glands. The mean annual decrease was 0.5 HU.

A comparison of polypeptides and restriction endonuclease sites of BHV-1 isolates and identification of IPV virus in Japan.

Bovine herpesvirus 1 (BHV-1) isolates from respiratory tract and from vagina of bovine in Japan were analyzed by SDS-polyacrylamide gel electrophoresis (SDS-PAGE) and the DNA restriction endonuclease cleavage pattern, and compared with European BHV-1 strains. Both protein profile and DNA cleavaged pattern of BHV-1 isolates from respiratory tract were the same as those of European infectious bovine rhinotracheitis (IBR) virus, whereas the protein profile and DNA cleavage patterns of one isolate (M1) from vagina was the same as those of the European infectious pustular vulvovaginitis (IPV) virus. The facts indicate that IPV virus has existed in Japan.

[Thoracic and lumbo-sacral spinal lipoma in an infant].

We reported a case of thoracic and lumbosacral spinal lipoma in an 8 month-old boy. He was born in asphyxia. Six months after birth, spastic paraparesis developed gradually at both lower extremities and secondary funnel chest due to shallow thoracic movement and scoliosis in the thoracic region was observed. MRI was then performed where tumor mass demonstrated by the high signals in both T1 and T2 in ages was confirmed in the thoracic and lumbo-sacral regions. Surgery was performed only for the thoracic lesion which was considered primary. The tumor in the Th3-4 level had developed subdurally, while the tumor in the Th5-10 extradurally. Only partial removal could be done for the subdural tumor because its boundary with the spinal cord was not clearly identified. Postoperative histological study revealed it as lipoma. During the following 6 months only slight recovery from paraparesis was observed. Surgical intervention against the mass in the lumbosacral region was not performed for the conus medullaris was found located in its normal position and no concomitant symptoms could be observed. In summary, it is considered that spinal lipoma is rare (1% of total spinal tumor) and commonly associated with other types of congenital anomalies (in 31% of spinal lipoma). Particularly in infants, spinal lipoma in the thoracic region as experienced in our case is extremely rare and any medical treatment started after clinical symptoms once developed seems ineffective.

[Attempt of intracranial IC bypass for giant aneurysm in the cavernous portion of the carotid artery].

We report here, a novel method for internal carotid (IC) bypass between C2 and C5, by using the autologous saphenous vein. The patient was a 58 year-old female, complaining of pain and motor disturbance in her right ocular region. She suddenly developed left-sided hemiparesis during her stay in our hospital. The following CT examination demonstrated a high density homogenous round mass, which occupied the right side of the parasellar region with no perifocal edema. The contrast enhancement CT, however, suggested there was still a certain degree of blood flow in the mass. On the other hand, carotid angiography showed an occlusion of blood flow at the C5 level (interosseous portion), which seemed to indicate either giant aneurysm or cavernous angioma. The mass was identified as a giant aneurysm during surgery, and the decompression procedure by removing the intra-aneurysmal blood clot was attempted first. However, this trial failed due to an accidental severe bleeding. Because of this, we then decided to perform the grafting of autologous saphenous for a length of 10 cm between C2 and C5. Intraoperative heavy bleeding mentioned above seemed to have occurred due to release of compression by the aneurysm against the internal carotid artery through which there was restricted blood flow. Postoperative angiography demonstrated that the graft remained patent enough to be approximately equal in size to the original artery. From our experiences in this case, two important points are, we suggest, to be kept in mind while applying this method; (1) to carefully expose the C5 segment, (2) to avoid damage of the surrounding structures such as the Eustachian tube, facial nerve and cochlear nerves, and so forth. In conclusion, we strongly recommend this IC bypass technique for other types of tumors in the cavernous portion as well.

[A case of male acromegaly with galactorrhea].

A 43-year-old man was admitted to our clinic with complaints of headache and nasal obstruction. He has noted increasing hand and foot size with decreasing libido and pollakiuria for ten years. On admission, he showed galactorrhea. His visual symptoms were slightly decreased. Endocrinological evaluation revealed high serum levels of GH and prolactin. A plain skull X-ray showed acromegalic features with remarkable destruction of dorsum sellae. A contrast enhanced CT demonstrated an intrasellar high density mass extending to the sphenoid sinus and the right ethmoidal sinus. A cerebral angiogram was nor.al. Surgery was then performed with the transsphenoidal approach. A soft reddish brown mass was found in the sphenoid sinus and the bilateral cavernous sinus extending from the sella turcica. Histologically the tumor was eosinophilic adenoma. There were numerous cells exhibiting immunostaining for both GH and PRL in the immunocytochemical study. Postoperative course was uneventful. His visual impairment improved soon after the operation. Serum GH and PRL levels decreased to 38 and 130 ng/ml. He was treated with conventional irradiation (500 rads), so remained galactorrhea and hyperhidrosis. One year after the operation, there is no regrowth of the residual tumor.

[A comparison between stereotaxic operation and conservative therapy for thalamic hemorrhage].

The subjects studied in this report included 17 surgical cases which underwent evacuation of hematoma by means of BRW CT stereotaxic system approximately 14 days after its onset, and 11 non-operated cases. The average age was 65.5 years for the operated and 68.7 years for the non-operated groups. The neurological grades on admission were Grade III or above (slight or mild disturbance of consciousness). The evaluation of the cases was made according to (1) laterality of the hematoma on the left or right, (2) neurological grading, (3) maximum anterio-posterior, lateral diameters, and maximum depth of the hematoma, (4) CT classification (Kanaya, 1981) and ADL (5 grades) at 4.5 months after onset. No correlation was found between ADL and the laterality of the lesion in both groups. As for neurological grading I and ADL on admission, 6/8 cases in the operated and 3/8 in the nonoperated groups recovered to ADL grade 2 or above and 2 bed-ridden cases were included in the latter group. In regard to the extent of the hematoma, the mean ADL was 1.83 in the operated group. While 2.75 in the non-operated group where the hematoma was 25 mm or less in its maximum anterio-posterior diameter. On CT, there existed a significant difference in ADL at IIb and 4/5 of the operated and 1/4 of the non-operated groups appeared to be ADL 2 or above. Assessment of the activities (mental change, willingness) as well as the muscle strength was performed within 3 days after surgery in operated group and 88.2% and 35.3% improvements were observed, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[The penetration of cefoperazone into the cerebrospinal fluid in patients on acute or chronic stage].

Cefoperazone (CPZ) was administered to 10 patients with cerebrovascular disturbances at acute and chronic phases to investigate its passage into the cerebrospinal fluid. The antibiotic was administered at dosages of 1 g and 2 g to patients in the acute phase to examine the dose-dependency. The results obtained are summarized as follows: 1. Serum concentrations Peak values of CPZ were 46.9 +/- 4.42 microgram/ml in acute phase patients in 1 g dosage group (acute group 1), 253.1 +/- 63.2 micrograms/ml in acute phase patients in 2 g dosage group (acute group 2), and 197.9 +/- 15.7 micrograms/ml in chronic phase patients in 2 g dosage group (chronic group 2) at 1 hour after CPZ administration. Concentrations of CPZ varied about 5-fold between the 1 g dosage group and the 2 g dosage groups. The acute group 2 showed generally higher values of CPZ concentrations than the chronic group 2. 2. Cerebrospinal fluid concentrations. Peak values were 0.96 +/- 0.30 microgram/ml (at 3 hours) in acute group 1, 4.55 +/- 3.41 micrograms/ml (at 1 hour, except 1 case) in acute group 2, and 1.29 +/- 1.28 micrograms/ml (at 1 hour) in chronic group 2. Acute group 2 showed generally higher values than chronic group 2. 3. CPZ was considered useful for the prevention of postoperative infections in the field of brain surgery.

A new instrument for noninvasive measurement of intracranial pressure through the anterior fontanel. I. Preliminary report.

A pressure sensor has been developed with which intracranial pressure of infants with open anterior fontanell can be measured. The sensor consists of a light, small strain-gauge transducer (Statham P-50, Gould, USA), supported by a thin phosphorus bronze sheet having a strong spring quality characteristic. The light and small assembly, which weighs only 2.55 g, allows lengthy, continuous measurement with little drift. It eliminates almost any gravitational component in the recorded pressure. The thermal influence is also minimal and the thermal equilibrium is reached within 10 min. The sensor is precise and accurate in a range from 0 to 500 mm H2O, with excellent linear response. This newly developed fontanel pressure sensor has been successfully applied without the need for surgical procedures previously required to measure intracranial pressure.

Rare neonatal intracerebral hemorrhage. Two cases in full-term infants.

Intracranial hemorrhage in neonates is often found in either the subependymal area or the subdural space. The former is observed particularly in premature infants and is attributable to damage of the germinal matrix layer. The latter usually occurs in the vicinity of the falx and tentorium cerebri of full-term neonates and is thought to be caused by birth injury. Two cases of intracerebral hemorrhage in full-term newborn babies are reported. In both the hematoma was located at the left frontal area under the coronal suture. Angiograms revealed no causative signs, such as arteriovenous malformation, aneurysm or angioma. Observations on CT scans and during surgery led to the conclusion that the hemorrhages were caused by the moulding which forced the frontal bone to slip under the parietal bone at the coronal suture and then press on the fragile cerebral vasculature of the neonates, thus causing contusion.

Intracellular localization and transport of three different bovine herpesvirus type 1 glycoproteins involved in neutralization.

Monoclonal antibodies against 3 different glycoproteins of bovine herpesvirus type 1 (BHV-1) involved in virus neutralization were used in indirect immunofluorescence (IIF) tests to characterize the appearance and transport to the plasma membrane of virus antigens in the infected cells. Antibodies against gp 117 and gp 71 glycoproteins first showed pronounced ring-like nuclear fluorescence at 4 hours post-infection (PI), followed by staining of the perinuclear region, presumably the Golgi apparatus. In contrast, antibody against gp 87 produced staining in cell-to-cell junctional areas at 3 hours PI before any staining close to the nucleus. The expression of the 3 glycoproteins at the surface of the infected cells was confirmed by the use of monoclonal antibodies having neutralizing activity, but not by non-neutralizing antibodies against gp 117 and gp 71. Non-neutralizing antibody against gp 87 detected the surface fluorescence only in those cells showing marked degeneration. Inhibition of glycosylation of the viral glycoproteins with tunicamycin (TM) was followed by interference with transport of gp 117 and gp 87 to the plasma membrane. On the other hand, gp 71 was incorporated into the plasma membrane despite the lack of N-linked glycosylation.

Bovine herpesvirus type 1 gp87 mediates both attachment of virions to susceptible cells and hemagglutination.

The 87,000-dalton glycoprotein (gp87) of bovine herpesvirus type 1 (BHV-1) was found to selectively attach to susceptible cells. The attachment of gp87 to the cells was markedly decreased by the prior adsorption of intact virions. Anti-gp87 (site Ia) monoclonal antibody, which inhibited BHV-1 adsorption to the cells and neutralized the virus without complement [Okazaki et al., Virology 250: 260-264], was effective in inhibiting the adsorption of gp87. Only the same antibody was able to inhibit the hemagglutination activity of BHV-1. Other monoclonal antibodies to the glycoproteins of BHV-1, including antibodies directed to sites Ib and Ic on gp87, were ineffective in inhibiting either virus adsorption or hemagglutination. The results of this study indicate that site Ia of gp87 molecule is the critical site of virus attachment for initiation of infection as well as the hemagglutination of BHV-1.
