Platelet aggregation studies generally are performed in platelet-rich plasma (PRP) by the turbidometric method. The authors compared this technic with the recently introduced impedance aggregometry in PRP and whole blood (WB). In healthy controls there was a good correlation between the two technics when aggregation was induced by ADP or collagen. As compared with PRP, platelets in WB were more sensitive to the aggregating effect of thrombin, ristocetin, and arachidonic acid. Platelet sensitivity to prostacyclin was increased in WB. The anti-platelet effect of a single oral dose of aspirin could be detected for a longer period in WB than in PRP. Platelet aggregation tests in WB from patients with platelet dysfunctions showed the same response pattern to different aggregating agents as in PRP. In contrast to turbidometry, the impedance method in PRP and WB enabled registration of platelet aggregation in a dose-dependent fashion in a sample from a patient with severe hyperlipoproteinemia. It is concluded that platelet aggregation can be studied conveniently with the impedance method in the more physiologic medium of WB. Providing the same information as the well-established turbidometry, the time-sparing impedance method needs less citrated blood. Moreover, our results show an increased sensitivity of the WB system to some aggregating and anti-platelet agents.

We report a 58 year old man suffering from recurrent epistaxis and high blood pressure. After removal of a subcutaneous tumour from the left neck the blood pressure and the pre-operatively elevated urinary excretion of adrenaline returned to normal. Histological examination of the tumour revealed a paraganglioma with immunocytochemical demonstration of serotonin.
Protein C, a potent anticoagulant, has been detected in large amounts in some prothrombin complex concentrates (PCC). We extended these findings to eight PCC commercially available in West Germany detecting 0.82 to 1.92 plasma equivalent units of protein C antigen (PC:Ag) per unit factor IX. Infusion of PCC in four patients on stable oral anticoagulation resulted in a recovery of 58.6 +/- 10.9%. Estimated from the decrease in plasma levels of PC:Ag half life was 7.9 +/- 0.6 h.
ITP is the most common autoimmune disease during pregnancy. Whereas the maternal risk due to the extreme thrombocytopenia can be calculated, the risk to the foetus is difficult to predict. As there is no correlation between maternal and foetal platelet count, no generalisations can be made as to the preferable method of delivery. The methods of treatment with special reference to pregnancy and the various procedures necessary during delivery are discussed.

Seven males with atherosclerotic disease received daily 1.6 g of the thromboxane antagonist BM 13.177 in two separate oral dosages over a period of four days. The drug significantly reduced elevated plasma levels of thromboxane B2, beta-thromboglobulin and platelet factor 4, whereas thromboxane B2 generation was only slightly depressed. BM 13.177 inhibited platelet aggregation by collagen, and to a minor degree the second wave of ADP induced aggregation. Platelet sensitivity to prostacyclin and aggregation by ristocetin were not altered. Bleeding time was prolonged. All effects disappeared within 24 hours after the last application of the drug. No side effects were noted. Thus BM 13.177 appears to be a safe and effective new antiplatelet drug.
The present intensification of cancer chemotherapy has resulted in increased toxicity. Nausea and vomiting are often severe and prolonged, rendering a patient unfit for further treatment. This review summarizes the current understanding of the physiology of vomiting and the pharmacology and efficacy of the antiemetics in clinical use. It also discusses new approaches with agents, such as cannobinoids, high-dosage metoclopramide, and high-dosage corticosteroids. The new agents are superior to standard antiemetic drugs as demonstrated in a number of trials.
There is evidence that polymorphonuclear granulocytes release neutral proteinases such as elastase (E) and cathepsin G in the course of acute leukemia. These proteinases may inactivate clotting factors by unspecific degradation before they are eliminated via complex formation with endogenous inhibitors, e.g. the alpha 1-proteinase inhibitor (alpha 1-PI). In this study it was attempted to correlate plasma levels of the E-alpha 1-PI complex with factor XIII and antithrombin III in acute leukemia. Using a newly developed, sensitive enzyme-linked immunoassay the concentration of E-alpha 1-PI in patients with various types of leukemia, malignant lymphoma or multiple myeloma was determined. Only patients with acute myelocytic or promyelocytic leukemia (AML, APL) and chronic myelocytic leukemia with and without blastic transformation (CML) showed moderate to high levels of E-alpha 1-PI (2- to 20-fold of normal). However, coagulation factor concentration observed in the different types of leukemia seemed to be independent of elastase liberation. Most of the AML-patients with elevated E-alpha 1-PI levels showed peroxidase positive blood cell smears.

Because of the high dosages which have to be given during extracorporal circulation it is often impossible to measure the end point of coagulation for both thrombin time and aPTT. A correct determination of the heparin level has become possible with synthetic substrates with the help of the "Protopath proteolytic enzyme detection system" (AHS, Munich). With a fluorogenic synthetic substrate in a one-way cuvette and calibrated fluorometer the heparin and AT III levels can easily be measured. Very high levels of heparin which averaged 6,9 U/ml +/- 1,0 10 minutes after heparinization and 4,4 U/ml +/- 1,0 60 minutes later were found in ten patients with open heart surgery during extracorporal circulation. AT III decreased to less than 50% possibly due to dilutional effects. Ten minutes after protamine neutralization heparin levels were less than 0,1 U/ml. After two hours AT III levels returned to 70%. Measurement of heparin blood levels adds to the quality of anticoagulant control in situations where high dosages of heparin are given.
Twenty patients with cancer previously unresponsive to antiemetic treatment of chemotherapy-induced nausea and vomiting were treated with the new tetrahydrocannabinoid Levonantradol. 15 patients experienced substantial relief and 10 of them preferred the drug for further courses. These observations suggest that Levonantradol can be beneficial to patients refractory to conventional antiemetic therapy.

Intracerebral manifestations of Hodgkin's disease are rare. They may occur as a primary or secondary manifestation and cause a variety of neurological symptoms. Frequently other organs as lungs, liver, bones and gastrointestinal tract are involved. Computer tomography and scintigraphy of the brain are valuable tools to help establish the diagnosis. Most patients have a poor prognosis. The treatment of choice is radiation of the brain. The literature of 34 reported cases is reviewed and two own cases are reported.
Essential features of the methods and results of an automated system-analytic investigation procedure with well-defined psychic load are described. This was developed with the aim of recognizing disturbances in the psychophysiological system behaviour as premorbid stages of the most important cardiovascular diseases. Results thus obtained show a.o. that healthy subjects and patients suffering from ischemic heart disease can be classified with high reliability according to their psychophysiological system behaviour. A differentiation could be obtained as well on the basis of personality characteristics. The results support the hypothesis underlying the development of this procedure.
