Mechanism of action of estrogen on intramembranous bone formation: regulation of osteoblast differentiation and activity.

Dynamic bone histomorphometry, [3H]thymidine radioautography, and Northern analysis for bone matrix proteins and insulin-like growth factor-I (IGF-I) were performed in calvariae of ovariectomized (OVX) and estrogen-treated OVX rats. Treatment of OVX rats with diethylstilbestrol (DES) for 2 weeks reduced the periosteal mineral apposition rate, osteoblast number, and osteoblast size in calvarial periosteum. DES treatment also reduced the number of preosteoblasts in the S phase of the cell cycle, suggesting that the decrease in osteoblast number was due in part to inhibition of proliferation of osteoprogenitor cells. One week after ovariectomy, there were small increases in mRNA levels for pre pro-alpha 2 (I) subunit of type I collagen (collagen), osteocalcin, and osteonectin and a large increase in the mRNA level for IGF-I. DES treatment resulted in rapid decreases (3 h) in the mRNA levels for osteonectin, osteocalcin, and IGF-I. In contrast, mRNA levels for collagen were virtually unchanged after short term DES treatment. Uterus and liver served as positive and negative control tissues, respectively, for the effects of DES on IGF-I mRNA levels in OVX rats; mRNA levels were increased in uterus and decreased in liver after hormone treatment. We conclude from these studies that estrogen reduces periosteal bone formation by inhibiting both the differentiation and activity of osteoblasts. Furthermore, down-regulation of mRNA levels for IGF-I and bone matrix proteins precedes the changes in dynamic bone histomorphometry.

The effect of PDGF on the healing of full thickness wounds in hairless guinea pigs.

1. Platelet-derived growth factor (PDGF), administered daily for 5 days and every other day for 9 additional days or the day of wounding and the following day, caused dose dependent (0.2, 1.0 or 5.0 micrograms PDGF/wound) increased granulation tissue. 2. The two day 5.0 micrograms treatment resulted in a 73% increase and the multi-day treatment resulted in a 52% increase of alkaline phosphatase activity three days after wounding. 3. Multi-day treatment resulted in significant increases in protein synthesis (132%), vimentin content (160%) and excised wound weight (76%) three days after wounding. 4. These results indicate that limited administration of PDGF alters wound healing although multiple applications provoke a more dramatic response.

Brain lesions and cognitive function in late-life psychosis.

Twenty-four patients who developed their first psychotic episode after the age of 45 were studied with MRI and comprehensive neuropsychological testing and compared with 72 healthy elderly subjects. The patients demonstrated more clinical abnormalities on MRI, were more likely to have large white-matter lesions or metabolic illness, and did more poorly on many neuropsychological tests, particularly those testing frontal-lobe and memory abilities. We conclude that structural brain injury is commonly associated with the late onset of psychosis. Neuroimaging investigations are a valuable component in the evaluation of this patient group.

The use of a membrane feeding technique to determine the infection rate of Culicoides imicola (Diptera, Ceratopogonidae) for 2 bluetongue virus serotypes in South Africa.

Culicoides spp. in the Lowveld of the northern Transvaal, Republic of South Africa, were fed bluetongue virus serotypes 3 and 6 and African horsesickness virus serotype 1 through latex and chicken skin membranes. After an incubation period of 10 days at 25-27 degrees C, the infection rate of C. imicola for bluetongue virus serotypes 3 and 6 was established at 31% and 24% respectively. No African horsesickness virus could be recovered. The membrane feeding technique and handling procedures proved to be suitable for field studies.

Penicillinase-producing Neisseria gonorrhoeae in a subset of nonemergent patients: I. A trend in the Washington, DC area.

A significant increase in penicillinase-producing isolates of Neisseria gonorrhoeae has been detected and documented in the District of Columbia. Currently, the prevalence of beta-lactamase isolates is approaching 50%, with the greatest number of cases being detected in the 18- to 23-year-old age group, in both males and females. Indicators suggest that this trend, which was first noted in 1987, will become more pronounced in the future. Ceftriaxone is now the first-line drug of choice in treating suspected gonococcal infections.

Comparison of CAGE questionnaire and computer-assisted laboratory profiles in screening for covert alcoholism.

To identify the most effective method of screening for covert alcoholism Ewing's CAGE questionnaire was compared with several computer-assisted laboratory data profiles in a prospectively gathered, random sample of 915 adults admitted to a general hospital. Whether a subject was alcohol dependent (n = 244) or not (n = 671), as defined by DSM-III-R, was determined on the basis of a structured interview. The CAGE questionnaire was highly sensitive (76%) and specific (94%) for recognition of alcohol dependence (positive predictive power 87%). None of the discriminant laboratory functions gave recognition rates greater than chance alone. Until the sensitivities, specificities, and positive predictive powers of computer-assisted methods improve, brief interview alone remains the best screening method for general hospital populations.

Changes in red blood cell choline and choline-bound lipids with oral lithium.

The influence of oral lithium on the concentration of red blood cell choline (Ch), lecithin, glycerophosphorylcholine (GPCh) and phosphorylcholine (PCh) was studied. The concentration of RBC Ch was significantly elevated and the concentration of lecithin, GPCh and PCh significantly depressed in 16 patients on oral lithium compared to 9 age-matched controls. We conclude that lithium markedly depletes the red blood cell of choline containing compounds including lecithin. These changes may be responsible for both the therapeutic efficacy and the toxicity of lithium.

Quality of life during chemotherapy for small cell lung cancer: assessment and use of a daily diary card in a randomized trial.

Fifty-three patients who were taking part in a randomized trial of chemotherapy in small cell lung cancer (SCLC) were entered into a study of quality of life measurement using a daily diary card. Patients received either four or eight cycles of initial chemotherapy and daily records were scored, using a four point scale of nausea, sickness, appetite, sleep, mood, pain, activity and general well being. Two hundred and fifty-six of a possible 379 cards were returned (68% compliance). The first 31 patients took part in an assessment of the diary card where comparison was made with nurse ratings using the card, the EORTC questionnaire and the Spitzer quality of life index. These comparisons showed appropriate convergent and divergent validity and demonstrated the sensitivity of the diary card to short term changes compared with the other measures. In the randomized trial the diary card demonstrated a worsening of sickness and related variables as treatment continued. This spilled over into mood and general well being although physical variables of pain, sleep and activity were largely unaffected. Prophylactic cranial irradiation was associated with a transient increase in sickness and vomiting. The study shows that the diary card is an instrument sensitive to short term changes in quality of life and thus especially useful for comparing effects during the period of treatment.

The attenuations of some hearing protectors as used in the workplace.

A sound field which conforms to the requirements of ISO 4869 (1981) has been created in an audiometric booth which has been installed in a mobile trailer. This trailer has visited industrial sites where protectors, which were being used in the workplace, were tested. Work people, selected at random, were tested in a procedure similar to that specified in ISO 4869. The occluded and unoccluded thresholds of hearing were measured and then used to estimate the protector attenuation. Results from a number of muffs, plugs and helmet-mounted muffs, tested in the workplace, show that, in most cases, the attenuations were less and the standard deviations were greater than those quoted by manufacturers from laboratory ISO 4869 tests. The implications of these results for the selection of hearing protectors are discussed.

Responses of harbor seal and pig heart to progressive and acute hypoxia.

Myocardial oxidative and glycolytic reserves were evaluated in four harbor seals, Phoca vitulina richardsi, and six domestic pigs, Sus scrofa (hematocrits: 58 +/- 5 and 34 +/- 1%., respectively). Progressive hypoxia was induced by lowering arterial hemoglobin-oxygen saturation in 10% decrements, each maintained for 10-min periods, until the onset of heart failure. Myocardial oxygen consumption rate (VO2), lactate release/uptake rate (L), and triple product, an index of myocardial energetic demand, were determined at each saturation level. Onset of L began in pigs at Sao2 = 57 +/- 5% and in seals at Sao2 = 35 +/- 4%. Cumulative oxygen consumption (VO2) during hypoxia, determined from the onset of cardiac lactate release, was 435 ml O2/100 g in seals and 172 ml O2/100 g in pigs. Cumulative lactate release during the same period was 14 mM/100 g in seals and 4.6 mM/100 g in pigs. The pigs' left ventricular contractile response (dP/dtmax) was greater than that of seals throughout the time of lactate release. Total myocardial energetic sources were higher in seals than in pigs, and seals were better able to tolerate myocardial hypoxia than were pigs. In a separate experiment, two seals and six pigs were made acutely hypoxic until cessation of cardiac output (seals, 17.5 min; pigs, 7.4 min) and were then reoxygenated. Both seals recovered promptly to control levels of cardiac mechanical function, whereas none of the pigs recovered. Additionally, five pigs were beta-blocked with 0.10 mg/kg of propranolol and were subjected to acute hypoxia. Tolerance to cardiac hypoxia in beta-blocked pigs was significantly increased compared with that of control animals.(ABSTRACT TRUNCATED AT 250 WORDS)

The epidemiology of mumps in southern Alberta 1980-1982.

The full spectrum of mumps in Southern Alberta was studied over the years 1980-1982. In the community, a random sample of 3,497 children was tested for prevalence of immunity to mumps. The 1,816 who were not immune were retested after one year. The incidence of new immunity was 16.5%. Most seroconverters had no illness suggestive of mumps. History of previous mumps or immunization were found to be unreliable predictors of serologic immune status. In the family study, information was obtained on 342 cases diagnosed as mumps by family physician recorders. The overall complication rate was 10.8% and the secondary attack rate within families was 11.7%. Hospital records of all 25 patients admitted in Calgary with mumps during the same period were analyzed. Estimated costs of unopposed mumps over 20 years compared with the estimated costs of vaccination showed that a vaccination program could give a benefit-cost ratio between 6.4 and 247. These studies show that mumps is a mild disease with relatively few serious complications or sequelae. Even so, there would be definite medical and economic benefit with immunization. A low-cost addition of mumps vaccine to the immunization program would be justified.

Brain white-matter lesions and psychosis.

In a prospective study of late-life onset psychosis, five of the first 27 patients studied had extensive white-matter lesions demonstrated by MRI and/or CT. None of 60 age-matched psychiatrically healthy controls demonstrated such lesions. All five patients had a mild dementia and a frontal behavioural syndrome. In addition, every patient performed poorly on neuropsychological tests of frontal function. Dysfunction of the frontal cortex associated with white-matter lesions appears to contribute to the clinical picture of some cases of late-life psychosis.

Liability insurance: can hospitals afford a roll of the dice?

The rise in hospital malpractice claims has greatly affected hospital liability insurance. Increasing premium costs and the shrinking availability of coverage have been some of the effects hospitals have felt. The two most promising approaches for dealing with this problem are increased risk management and changes in tort law. These solutions allow insurance companies to calculate the risk, leading to greater predictability.

Clinical comparison of a new automated infrared blood culture system with the BACTEC 460 system.

A new blood culture instrument, the BACTEC (Johnston Laboratories, Inc., Towson, Md.) NR-660, which utilizes infrared detection of carbon dioxide from microbial metabolism, was compared with the radiometric BACTEC 460 system. There were 1,554 isolates from 18,785 paired aerobic blood cultures. Of these isolates, 1,303 were isolated from the radiometric 6B medium, and 1,259 were isolated from the NR6A medium (P = 0.06). Analysis of the data indicated no significant differences in recovery when any individual species was considered. When organisms were considered as groups, there were no significant detection differences for gram-negative bacilli, yeasts, or anaerobes. For gram-positive cocci in aerobic medium, the BACTEC 460 detected 84.3% of the total isolates, and the BACTEC NR-660 detected 79.7% (P = 0.04). There were 891 isolates from 13,983 paired anaerobic blood cultures. Of these isolates, 725 were recovered from the radiometric 7D BACTEC medium, and 723 were recovered from the NR7A BACTEC medium (P greater than 0.9). In the anaerobic media there was no significant difference in detection of any organism group, including the gram-positive cocci. When the results of the aerobic and anaerobic media were combined, there was equivalence between the two systems for the detection of gram-positive cocci (P greater than 0.2) and other organism groups. When the ability to detect septic episodes was compared, there was no significant difference for any organism group (P = 0.12). For aerobic media, the mean times for detection were 30.5 and 29.5 h for the BACTEC 460 and NR-660, respectively. For anaerobic media, the mean times for detection were 39.8 and 41.6 h for the BACTEC 460 NR-660, respectively. Compared with the BACTEC 460, the BACTEC NR-660 system had a greater ease of operation, faster test cycle, computerized data base, and equally rapid detection of positive cultures.
