[Ketoconazole. 3 years' experience with a new orally active broadspectrum antimycotic].

Ketoconazole is an orally high effective broad-spectrum antimycotic of the imidazole series. It is very well tolerated and stands out by its long-lasting therapeutic serum levels, lack of development of resistant fungi, simple administration and low toxicity. Own experience has shown its high efficacy in pityriasis versicolor, mycoses due to dermatophytes, skin and mucous membrane candidoses and chronic mucocutaneous candidosis syndromes. Its effect in systemic mycoses is promising. Consistent follow-up of laboratory data did not suggest any changes due to ketoconazole even after more than one year of treatment.

Administration, Oral↗

[Surgery of the nail region].

Operations of the nail apparatus are done under sterile conditions and regional anaesthesia and in local anaemia. The knowledge of the complicated anatomy and physiology is the prerequisite for good surgical results. Nail extractions are commonly done too frequently. Subungual haematomas require a different treatment depending on the severity. Removal of the proximal third of the nail plate quickly clears acute paronychia resistent to antibiotics. The treatment of choice for the ingrown toenail is the surgical narrowing of the matrix. Kind and size of subungual tumours determine their surgical treatment. Diagnostic nail biopsies are best performed laterally.

Basal Cell Carcinoma↗

[Subungual tumours (author's transl)].

The differential diagnosis of subungual tumours is often very difficult, since the same tumor located in another area looks entirely different. Former infections or traumata may further mask the tumour. Pain is usually not characteristic. Infection and the history of a trauma may further mask this tumour. Pain is usually not characteristic. both malignant and benign tumours may develop from all tissues present in the distal phalanx. Subungual melanomas are the most common malignant subungual tumours, whereas a variety of benign tumours occurs in the subungual region, the most characteristic of them is the glomus tumour.

Bowen's Disease↗

[Differential diagnosis and therapy of calluses, corns and plantar warts (author's transl)].

Calluses and corns arise from persistent continuous or intermittent pressure where there is an insufficient amount of soft tissue between skin and the underlying bone. Pain from corns results from their hard conical corn pointing to the bone. Foot deformation is the most common cause. Warts may develop as myrmecia, mosaic and vulgar warts. Surgical treatment is usually not indicated since the rate of recurrences is as high as with conservative therapy, and the resulting scars may lead to the same complaints.

Ultrastructure of blister formation in epidermolysis bullosa hereditaria: V. Epidermolysis bullosa simplex localisata type Weber-Cockayne.

The ultrastructure of epidermolysis bullosa simplex type Weber-Cockayne has as yet not been studied systematically. Therefore biopsies of blisters freshly produced by friction after a hot bath were investigated by electron microscopy in 8 subjects of 6 families. Blister formation was found always to start in the basal layer of the epidermis. At first the cytoplasm with the cell organelles appears to become diluted, then holes develop in the cytoplasm which merge and finally form cytolytic blisters between the dermo-epidermal junction and the nucleus. The cell organelles remain remarkably intact. The cause of blister formation is still unknown, however, no structural abnormality was observed.

Closure of wedge excisions of the lip.

By a simple modification of the mucosal suture described in many textbooks the disadvantages of the technique can be lip, the tunica propria is sutured with catgut from the inside without penetrating the mucous epithelium.

Humans↗

[Glossodynia].

Glossodynia is characterized by burning, pain and/or other dysaesthesias of the tongue and oral mucosa. Usually, pathological alterations of the oral mucosa are lacking. Latent depressions are the major cause, however other psychic disturbances may also occur. Local changes and systemic diseases are extremely rare causes of glossodynia. Treatment must be aetiological. Non-indicated substitution therapy of suspected but unproved deficiency states must be avoided.

Depression↗

[Levamisole treatment of acne fulminans (author's transl)].

In two young patients levamisole 150 mg twice weekly led to a rapid and long-lasting improvement of their acne fulminans. The sudden deterioration of acne vulgaris leading to acne fulminans is possibly due to a temporary immunodepression.

Acne Vulgaris↗

[Changes of the lip salivary glands in glossodynia. Histologic and morphometric analysis (author's transl)].

The minor salivary glands of the lower lips were analyzed histologically and morphometrically from biopsy specimens taken from 29 patients with glossodynia and 12 normal controls. The causes of glossodynia included Sjögrens syndrome [5], stomatological disorders such as diabetes mellitus, prosthesis allergy, etc. [5], psychiatric depressions [6], and idiopathic [13]. Using semi-thin tissue sections, the following results were obtained: 1. The control group had an average acinous area of 70.2%. This area was moderately reduced in the older patient. In patients with glossdynia, the acinous area was reduced to a mean value of 38.9%. Some severe cases showed values of 10% and less. 2. The area of duct structures was 9.3% in control tissues, and increased to 13.1% in patients with glossodynia. Values to 26% were rarely observed. 3. The interstitial tissue of the control group was inconspicuous and contained only few inflammatory cells. In glossodynia, the reduction of glandular acini could be positively correlated with the proliferation of the connective tissue and to stromal infiltration by lymphocytes and plasma cells. In diffuse lymphocytic sialadenitis, the mean value of the acinous area was 46%. An additional focal lymphocytic infiltration resulted in a further reduction to 27.3%. 4. The most severe cases of sialadenitis were found in glossodynia associated with Sjögren's syndrome. As such, the lip biopsy was found to be a useful method for diagnosis of the syndrome.
