The combined effects or orally administered physostigmine and lecithin were assessed in a double-blind study of a single patient with posttraumatic amnesia. Treatment improved verbal recall but not verbal recognition, visual memory, or conceptual reasoning. Both storage and retrieval of words in verbal memory were facilitated. Greater improvement in learning of longer or semantically homogeneous word lists than shorter or semantically mixed lists may indicate that treatment reduced the effects of interstimulus interference. Greater divergence of recognition response biases for semantically homogeneous vs. mixed lists was observed under treatment, reflecting some enhancement of semantic appreciation. Absence of a treatment effect on visual nonverbal memory may be due in part to lateralization of the mesencephalic lesion to the left in this patient. Lack of improvement of encoding capacity, d', or conceptual reasoning may reflect a greater dependence of basal forebrain structures on catecholaminergic than cholinergic mechanisms.

Of the 4,529 women delivered of infants at The John Hopkins Hospital during 1976 and 1977, 212 (5%) had roentgenographic pelvimetry. An analysis of a subgroup of 142 single vertex pregnancies with radiographic pelvimetry during this two-year period is presented. The cesarean section rate was 40% among patients selected for roentgenographic pelvimetry whose pelvis was found to be adequate. Vaginal delivery in women with a pelvis radiographically shown to be small was accomplished in 27% of patients with failure to progress in labor and in 22% of the patients with a fetus in the floating vertex presentation. Patients with a small pelvis who delivered vaginally had significantly smaller infants than those delivered by cesarean section (2,785 +/- 399 gm vs 3,146 +/- 433 gm). The only significant difference in pelvic measurements between these two groups occurred in the AP diameter of the inlet (11.5 +/- 1.1 cm vs 10.5 +/- 1.0 cm). Roentgenographic pelvimetry of single pregnancies as evaluated in this study appears to have limited clinical usefulness.
M-mode echocardiography in a 68-year-old man with a permanent transvenous pacemaker revealed intermittent mitral valve prolapse. The mitral valve prolapse was present during normal sinus rhythm as well as during pacemaker beats either preceded by or not associated with any P wave activity. During pacemaker beats followed by retrograde P waves, the mitral valve prolapse was no longer seen. It is postulated that atrial contraction during ventricular systole produced a rise in left atrial pressure and prevented the mitral valve leaflets from prolapsing into the left atrium.
LDH-x is a unique form of lactate dehydrogenase found in germinal epithelium during spermatogenesis. 53 patients with germ cell tumors were screened for the presence of LDH-x in the serum. No patient had detectable levels of the enzyme. 31 of the patients had elevated total serum LDH levels, and the isozyme fraction type 1 and/or type 2 was responsible for the elevation in all cases. LDH is a useful marker in germ cell tumors, but LDH-x is not the responsible isozyme.

Hyaluronidase (hyaluronate 4-glycanohydrolase, EC 3.2.1.35) was purified from mouse testes by ion-exchange chromatography, Sephadex G-200 filtration and Con A-agarose affinity chromatography. The final preparation had 94-fold purity and 12.2 units spec. act. of the enzyme (unit of specific activity = mumol N-acetylglucosamine released/h per mg protein at 37 degrees C and pH 4.5). Hyaluronidase is relatively heat stable and loses 10-20% of its activity at 50-55 degrees C for 10 min. Ea for eat denaturation of enzyme is 42-45 kcal between 45 an 63 degrees C. The Michaelis constant of mouse testicular hyaluronidase is 1.1 mg/ml hyaluronic acid. Antibodies to the purified enzyme were produced in rabbits and showed a single precipitin line by Ouchterlony gel diffusion. Antiserum to hyaluronidase inhibited enzyme activity by 25%. Immunologically, mouse testicular hyaluronidase is species specific. Tissue extracts of mouse vital organs, except testes and epididymis did not react with the antisera, though nonspecific precipitation occurred between intestinal extracts and anti-hyaluronidase serum. Hyaluronidase was localized in testis sections by indirect immunofluorescence. A specific dark green fluorescence was localized on cell boundaries extending from spermatogonia to spermatids and appeared on the sperm acrosome. Cytoplasm of spermatogonia and spermatocytes showed light green fluorescence, whereas interstitial tissue was devoid of fluorescence.

To determine the optimal noninvasive method for the demonstration of endocarditic vegetations, 35 consecutive episodes of clinically diagnosed endocarditis in 33 patients were studied with M mode and two dimensional echocardiography, and with gallium-67 citrate and technetium-99m stannous pyrophosphate cardiac scanning. Clinical criteria for the diagnosis of endocarditis were: temperature higher than 38 degrees C; sustained bacteremia with at least three positive blood cultures; no extracardiac focus of bacteremia; and known underlying heart disease, a new or changing murmur or a history of intravenous drug abuse with radiologic evidence of septic pulmonary emboli. M mode echocardiography detected 18 vegetations in 17 of the 35 episodes of endocarditis studied (49 percent positive); two dimensional echocardiography detected 30 vegetations in 28 of the 35 episodes studied (80 percent positive). In contrast, no vegetations were detected with technetium-99m stannous pyrophosphate scanning,, and only two gallium-67 citrate scans were positive. The advantage of the two dimensional echocardiographic technique over all others tested was particularly notable for the identification of aortic and tricuspid valve vegetations.

Immunization of female rabbits and mice with the sperm-specific isozyme of lactate dehydrogenase, LDH-C4, significantly reduced their fertility. Similar studies have been extended to nonhuman primates. Two female baboons, immunized with human LDH-C4, produced low antibody titers. These titers were markedly enhanced by booster injections of murine LDH-C4. An additional seven female baboons responded with relatively high antibody titers after receiving murine LDH-C4 as both priming and booster dosages. All nine females received injections of murine LDH-C4 at varying times determined by serum titer levels during fertility studies. These antisera reacted with human, mouse, and baboon LDH-C4. In a series of breeding experiments, 22 of 30 matings, or 73%, were infertile as compared with 28% in control matings. This contraceptive effect of the vaccine containing LDH-C4 was related to antibody titer and was reversible. Normal pregnancies ensued in animals in which the titer declined after termination of booster injections of vaccine.
1. 3-Methylbutanal is a normal constituent of human plasma and is elevated in patients with hepatic encephalopathy. Therefore these studies examined the possible source and site of synthesis of 3-methylbutanal and its effect on the central nervous system of adult rats. 2. 3-Methylbutanal was found to be a normal constituent of rat plasma and increased two to five times when leucine comprised 5% of the diet. Neomycin in the diet prevented the leucine-induced rise in plasma 3-methylbutanal. When this was injected at a dose of 120 mg/kg abnormal EEG patterns and sleep-like behaviour occurred, whereas smaller amounts (30 mg/kg) increased brain serotonin concentrations. 3. 3-Methylbutanal is a normal component of rat plasma: it may be derived in part from colonic bacterial breakdown of leucine and may influence central nervous system function. A possible relationship of 3-methylbutanal to the pathogenesis of hepatic encephalopathy is suggested.

Fourteen patients with aortic valve endocarditis were evaluated. Twelve patients had vegetations detected by two-dimensional echocardiography and two were diagnosed anatomically. M-mode echocardiography was positive in eight patients. Two-dimensional echocardiography was superior to M-mode in determining size, shape, and mobility of vegetations. The following three morphologic types of vegetative lesions were seen on two-dimensional echocardiogram: globular polypoid masses (seven); irregular, elongated lesions with chaotic movement (four); and a cord-like structure (one). Serial two-dimensional echocardiograms performed after completion of antibiotic therapy in seven patients revealed no change in appearance in five and complete disappearance in two patients. Among the 12 patients with vegetations visualized on the two-dimensional echocardiogram, seven responded to medical therapy, four required aortic valve replacement, and one patient died. One of the patients who initially responded to medical therapy eventually required aortic valve replacement following a second episode of aortic endocarditis. In those patients with negative two-dimensional echocardiograms, the vegetations were 3 mm in diameter or less at surgery or autopsy. Vegetations that were visualized on two-dimensional echocardiography were found to be at least 5 mm in diameter at the time of operation. Two-dimensional echocardiography is a valuable noninvasive tool in the evaluation of patients with aortic valve endocarditis.
