An analysis of 12 early pacemaker failures in a series of 128 transvenous implants is reported. Spontaneous recovery, maintained during long-term follow-up of 3 patients with isolated transient sensing failure, suggested the possibility of a transient loss of sensing voltage amplitude, or slew rate (dv/dt), or both. Conservative management is frequently indicated in these cases. In 62 patients in whom a "tined" lead was used, there were no early pacemaker failures.
The prevalence of methane production in an adult population of 256 subjects was 41%, but it was significantly higher in females (49%), than males (33%). When the population was subdivided into ethnic groups. Caucasians (48%) and Black (45%) had significantly more methane producers than Orientals (24%) and Indians (32%). when the ethnic groups were analysed by sex, female Caucasians had the highers prevalence (58%), significantly more than Caucasian males, Oriental males, and females and Indian males. In contrast with previous studies, a single dose of lactulose was found to significantly increase breath methane concentrations in six out of 12 methane producers, but not in 25 non-methane producers from the population study. In conclusion, any studies on breath methane must take into consideration the ethnic origin of the subjects and, contrary to previous advice, substrate intake, especially undigestible carbohydrates. Furthermore, a single breath sample may miss up to one-fifth of methane producers.
M-mode and two-dimensional echocardiograms were recorded in 12 narcotic addicts who had right-sided infective endocarditis. The two-dimensional echocardiogram showed vegetations in 10 patients (nine tricuspid and one pulmonic), while the M-mode echocardiogram was positive in six (five tricuspid and one pulmonic). The use of multiple transducer positions resulted in better visualization of the valves and appeared to be an important reason for the large number of positive two-dimensional echocardiograms. Echocardiographic findings were also available after completion of antibiotic therapy in seven of 10 patients. Of these seven patients, the vegetation appeared unchanged in three, diminished in size in three and was no longer visualized in one. No patient required valve replacement. Two-dimensional echocardiography using a wide-angle sector scanner appears to offer distinct advantages over the standard M-mode technique in evaluating patients with right-sided infective endocarditis.

Female Swiss Webster mice were immunized intraperitoneally with mouse epididymal spermatozoa or with phosphate buffered saline (PBS) and their fertility was compared by (1) incidence and size of litters, (2) number of uterine implantation sites, and (3) incidence and number of fertilized eggs in the oviducts. Statistically significant reduction in fertility was noted following two courses of injections of spermatozoa; 12% of mice injected with spermatozoa had litters compared with 80% of mice injected with PBS. The infertility did not seem to be related to a failure in fertilization since the two groups of mice had a similar incidence and number of fertilized eggs in the oviducts. All female mice were found to have a "natural' anti-acrosomal antibody. Following immunization with spermatozoa, antibodies to "postacrosomal' region, the main piece and the midpiece of the tail, as well as cytotoxic antisperm antibodies, appeared. Anti-LDH-X antibody was not detected. However, correlation was not found between infertility and antisperm antibodies or sperm granulomata that developed in the peritoneal cavities. It is concluded that female mice receiving repeated i.p, injections of mouse spermatozoa become infertile and that the infertility is related to interference with events after fertilization.

Idiopathic hypertrophic subaortic stenosis (IHSS) occurs more commonly in the elderly than is generally believed, and is often unsuspected. In 26 patients above the age of 60, the diagnosis was recognized in only 7 (27 percent) prior to echocardiography. Symptoms included dyspnea in 17, chest pain in 16, and dizziness or syncope in 8 patients. In 10 patients, establishing the correct diagnosis led to therapy with propranolol, with or without discontinuation of digitalis; in 7 of these, the chest pain was significantly reduced. An accurate diagnosis is particularly important because drugs that are useful in other forms of heart disease may have adverse effects in IHSS. Echocardiography is the diagnostic procedure of choice and is indicated in the presence of an unexplained systolic murmur, especially when it is associated with chest pain, syncope or left ventricular hypertrophy.

The sperm-specific isozyme of murine lactate dehydrogenase (LDH-C4) was injected into female mice of various strains. Two regulatory phenotypes characterize the resultant immunity to LDH-C4: one is manifested by high, intermediate or low levels of response, the other by the immediate or delayed maturation of peak titer. The response of several strains can be classified as high (SWR, SJL, BABL/c, C3H/He) and intermediate to low (A, CBA, DBA/2, DBA/1, C57BL/6) according to the level of antibody production and cell mediated immunity. BALB/c, SJL and SWR strains are immediate responders while DBA/2 and C3H/He mice are clearly delayed responders. Maturation and magnitude of response do not appear to be related. Both the antibody and cell mediated responses are T-dependent, but are not obviously associated with Ig allotype or H-2 regulation.

A 72-year-old man with calcific aortic stenosis and complete heart block was noted to have an intermittent diastolic murmur. Using combined echocardiography and phonocardiography, it was demonstrated that the murmur occurred while the mitral valve was closing. This suggested that antegrade flow across a closing mitral orifice is of etiologic importance in the production of the murmur. Atrial sounds were also recorded and probably have a similar etiology. It is postulated that reduced ventricular compliance resulting in a more rapid rate of mitral valve closure was of etiologic importance in the genesis of the murmur.
Clinical and electrocardiographic findings in 171 patients with pericardial effusion diagnosed by echocardiographic studies were reviewed. In 70 patients the effusion was unsuspected. There were 87 small, 50 moderate, and 31 large effusions. Cardiac tamponade was present in three patients. Congestive heart failure was the most common cause of pericardial effusion and occurred in 37 patients. Other frequently noted conditions included cardiac disease without congestive heart failure, neoplasms, acute nonspecific pericarditis, renal failure, and acute myocardial infarction. A pericardial friction rub was present in 23 patients, two-thirds of whom had moderate or large effusions. Atrial arrhythmias were common. Low voltage occurred in 31 of 136 patients and was more common with large effusions. The ability to distinguish between a small effusion and the quantity of pericardial fluid present normally is a problem requiring further clarification.
Spermatogenesis is a programmed developmental process characterized by the inactivation of certain genes and the activation of other, testis-specific genes. Synthesis of unique gene products such as LDH-C4 and cytochrome ct occurs only at precise stage of germ cell formation. The developmental sequences of gene activation for these proteins was observed by immunohistochemical procedures. LDH-C4 is first detectable during mid-pachytene of the primary spermatocyte. The C subunits appear to be uniformly distributed throughout the cytoplasm of the spermatocyte. The mid-pachytene stage also marks the first appearance of cytochrome ct. The association of this electron transport protein with spermatocyte mitochondria is reflected in a granular fluorescence of specific antibody-treated sections of testis. Neither the C subunit of LDH, nor cytochrome ct appear in leptotene or early pachytene primary spermatocytes. These analyses indicate that there is stage-specific protein synthesis in the primary spermatocyte which is characterized by differential activation of the LDH-C locus and of the gene coding for cytochrome ct.
Mouse testes contain a unique form of cytochrome c. As demonstrated by the indirect immunofluorescence technique, the testis-specific cytochrome c is detectable in the primary spermatocyte and in cell types comprising the later stages of spermatogenesis. Interstitial cells, Sertoli cells, and spermatogonia contain the somatic form of cytochrome c, as does heart muscle.

An unusual pattern of LDH isozymes was observed by gel electrophoresis of an extract of a human testis. This isozyme composition is consistent with an allelic variant of Ldh-c.
