[Radiation-induced intracranial meningiomas].

Based on the analysis of 112 cases a clinical characterization has been done of intracranial meningiomas developed secondary to X-ray irradiation of the head for fungus disease of skull integuments. X-ray irradiation increases the risk of intracranial meningioma development up to 4-fold. Radiation-induced intracranial meningiomas are characterized by particular features of clinical manifestations, by preponderance of hyperdense forms, by being of large size, as well as by a high frequency of changes in the adjacent bone presenting as hyperostosis, usuration, and destruction. The above meningiomas are notable for a high specific weight of anaplastic varieties.

[The characteristics of the clinical course and diagnosis of traumatic intracranial hematomas with rare locations].

The clinical course was studied as were potentialities of certain methods of diagnosis in 91 patients with traumatic intracranial hematomas of rare localization: hematomas of the posterior cranial fossa, epidural hematomas of the anterior cranial fossa and occipital pole, and those seated above the upper longitudinal sinus, interhemispheric subdural hematomas and basal subdural hematomas. The authors' and reported data are summarized.

[The clinical manifestations of craniocerebral trauma in elderly patients].

It has been shown that over the recent 20 years the number of senile patients with a history of craniocerebral injury rose from 4.8 to 7.2% whereas in the group over 60, from 53 to 72%. In the course of "mild" craniocerebral injury, 21% of senile patients manifested secondary acute disorders of brain circulation that not infrequently eventuated in lethal outcomes. Of paramount importance in the clinical manifestation of craniocerebral injury in senile age is the growing deterioration of brain circulation rather than the rise of intracranial pressure.

[Differential treatment of focal traumatic brain injury].

On the basis of observation of 65 patients with focal traumatic injury to the brain (hematoma of mater encephali was noted in 24, intracerebral--in 27, a focus of type III brain contusion--in 24, type IV--in 11, multiple hematomas and foci on contusion-crush--in 21), the criteria for their non-surgical treatment have been substantiated. Choice of a method for the treatment depends on the sizes of a hematoma, focus of contusion-crush, location of an injury, degree of its influence on the brain structures (deformation, shift), value of intracerebral pressure, dynamics of the clinico-roentgenologic symptoms.

[Clinical characteristics of repeated cranio-cerebral injury].

The clinical manifestations of repeated craniocerebral injury indicate that repeated brain injury is a graver form of traumatic disease of the brain. As compared to the primary injury, patients with repeated craniocerebral injury manifest more pronounced general cerebral, focal hemispheric symptomatology. In such patients, stem symptoms are also encountered more often. Analysis of 163 cases has demonstrated that the clinical manifestations of repeated craniocerebral injury depend on the gravity of the primary and repeated brain injury, age of the victims and by the rate of repeated injuries.

[Clinical aspects, diagnosis and treatment of cranio-thoracic injuries in patients of different age].

The comparative analysis of the clinical manifestations and informative value of the adjuvant methods of investigation in 125 patients with ++cranio-thoracic injuries and 53 patients with isolated craniocerebral trauma was carried out. This permitted to define more precisely the possibilities of the methods of neurosurgical diagnosis and find the optimal methods for the treatment of patients of different age with ++cranio-thoracic injuries.

[The clinical manifestations of epidural hematomas in the anterior cranial fossa].

Epidural hematomas of the anterior cranial fossa are a relatively rare localization of intracranial hemorrhage. Hematomas of this localization show a peculiar course and clinical manifestation. By the character of clinical manifestations, course of the injury, results of surgical treatment two groups of patients were distinguished: patients with isolated hematomas of the frontal region characterized by a paucity of clinical manifestations and patients with epidural hematomas of the frontal region associated with other intracranial lesions and are characterized by a grave course and high lethality.

[Features of mild repeated cranio-cerebral injuries].

An analysis of 97 cases indicates that the clinical manifestations of mild repeated head injuries do not reflect the real severity of the traumatic brain disease which determines the necessity of complex clinico-instrumental examination of these patients. Objective clinical and instrumental methods suggest that complete cure does not occur in mild repeated head injury in patients of all age groups.

[Prevention of intracranial hemorrhage in the postoperative period in severe cranio-cerebral trauma].

An analysis of 463 patients operated upon for severe cranio-cerebral traumas has shown that one of the main factors improving results of treatment is using the methods of neurosurgical prophylactics of intracranial hemorrhages in the postoperative period. A considerably less incidence of intracranial hemorrhages can be achieved by using the inflow-outflow dialysis system, removing the crushed brain substance, suturing the dura mater to the periosteum at the borders of the bone defect. The ends of the ligature are taken out onto the skin through the holes in the center of the osseous flap.

Brain Injuries

[Clinical characteristics of craniocerebral injury in patients with arterial hypertension].

A comparative analysis of 408 cases of craniocerebral trauma (CCT) has revealed a considerable influence of hypertension on clinical manifestations of CCT. It has been found that in hypertensive patients the incidence of secondary acute impairments of the cerebral circulation predominantly of the ischemic type with less marked hypertensive and dislocation syndromes was 4-5 fold higher than in patients with normal blood pressure.
