Segmental membranous glomerulonephritis.

Each of the four stages of membranous glomerulonephritis (MGN) has a characteristic ultrastructural appearance. This report described four children with another distinctive pattern of MGN, which we propose to call segmental membranous glomerulonephritis (SMGN). In all cases there were segmentally distributed subepithelial or intramembranous electron-dense or electron-lucent deposits and prominent spikes. Less than 50% of capillary loops per glomerulus were affected. Granula deposits of IgG were identified by immunofluorescence microscopy, but deposits were either absent or less intensely fluorescent in repeated biopsies. All four patients had proteinuria. Three patients have normal creatinine clearances with either diminished proteinuria (one patient) or no proteinuria (two patients) after a mean follow-up of ten years. The fourth patient died of an unrelated cause. The evolution of MGN and SMGN is discussed and it is proposed that SMGN represents partial resolution of MGN.

Membranous glomerulonephritis: clinical significance of glomerular hypercellularity and parietal epithelial abnormalities.

Quantitative and semi-quantitative morphological analysis of 41 cases of idiopathic membranous glomerulonephritis revealed glomerular hypercellularity in 24 cases and abnormal parietal epithelium in 25 cases. Abnormal parietal epithelium included crescents and prominent parietal epithelium (at least ten enlarged cells with round or ovoid nuclei lining Bowman's capsule of two or more glomeruli per biopsy). The clinical course (mean follow-up = 5.1 years) was correlated with biopsy findings. 8 to 17 patients with normocellular glomeruli, but only 3 of 24 patients with hypercellular glomeruli achieved complete clinical remission. 10 of 16 patients with normal parietal epithelium, but only 1 of 25 patients with abnormal parietal epithelium achieved complete remission. Of 9 urine patients, all but 1 had abnormal parietal epithelium in their biopsies. Glomerular cellularity and parietal epithelium may be useful prognostic indices in idiopathic membranous glomerulonephritis.

Evaluation of chronic renal failure by digital gray-scale ultrasound.

Forty-two patients presenting with chronic renal failure (serum creatinine 1.5--22.2 mg/dl) were examined with digital gray-scale ultrasound using high-frequency transducers. In 41 patients both kidneys were diffusely hyperechoic. Corticomedullary boundary could be defined in only a few patients. In one patient with focal glomerulopathy and mildly reduced renal function, the kidneys were ultrasonographically normal. The increased echogenicity of kidneys was not specific to any renal parenchymal diseases, and there was no definite correlation between the echogenicity of the kidneys, kidney size, and the degree of decrease of renal function.

Thyroid antigen associated immune complex glomerulonephritis in Graves' disease.

A 60 year old hyperthyroid black woman with long-standing Graves' disease treated with methimazole presented with anasarca and congestive heart failure. She was found to have the nephrotic syndrome with a urinary protein excretion of 32 g/day. Light and electron microscopy revealed a stage II membranous glomerulopathy. Direct and indirect immunofluorescence demonstrated immunoglobulin G (IgG), immunoglobulin M (IgM), third component of complement and thyroglobulin in a granular diffuse pattern consistent with an immune complex glomerulonephritis. Total thyroidectomy led to a decrease in proteinuria with little change in glomerular filtration rate during an 11 month follow-up period. We believe this to be the first report of immune complex glomerulonephritis associated with thyroid antigen in Graves' disease.

Systemic amyloidosis and hypogammaglobulinemia.

Small deposits of amyloid were found at autopsy in the kidneys, liver, spleen, and blood vessels of the intestines of a patient known to have primary acquired hypogammaglobulinemia. Previously described cases of hypogammaglobulinemia-associated amyloid are reviewed and possible pathogenetic mechanisms are discussed. The pathogenesis of amyloidosis in this patient is not known.

Endometrioid carcinoma of the fallopian tube.

A case of endometrioid carcinoma arising in endometriosis of the fallopian tube is described. One case of tubal endometrioid carcinoma has been reported previously, but the entity has not hitherto been recognized among primary fallopian tube tumors. The clinical presentations, histopathology, and pathogenesis of this rare tumor are discussed.

Colorized digital transmission electron microscopic images for teaching pathology.

We describe how electron microscopic images can be selectively colorized to accentuate ultrastructural detail for teaching purposes. Images are either captured by a CCD camera attached to the electron microscope or are scanned into a computer as electron micrographs. The computer's digital images are then manipulated using image-editing and colorizing software to highlight specific ultrastructural details, and subsequently printed or converted into 35-mm slides to be used for teaching.

Sarcomas of the female genital tract and pelvic soft tissues. A cluster of 14 cases in one year.

We report ten uterine and four extrauterine sarcomas encountered in a twelve month period (1989-90). The seven new cases of uterine sarcomas represent an estimated one third of all cases of malignancy of the corpus uteri, which were diagnosed at the gynaecology departments of St James's and Adelaide hospitals in one year. Overall, the prognosis was poor in the uterine sarcoma group-only one patient in clinically tumour free and five have succumbed to their disease. Delay in diagnosis especially in younger women with fibroid uterus contributed to this outcome.
