Disturbances of platelet function in patients with multiple myeloma.

Coagulation disturbances in patients with multiple myeloma are presented. Platelet disfunction was especially observed, but only in patients with marked hyperproteinemia and a high level of immunoglobulins in plasma, whereas normal adhesion and aggregation were noticed in 3 out of 17 patients without protein disturbances. Chemotherapy as well as plasma exchange caused not only a decrease of the paraprotein level in plasma, but also a normalization of the failure previously observed in platelet function. Our experiments confirm the opinion that protein M in plasma is responsible for the disturbances observed in multiple myeloma.

Clinical investigations on long-term activation of fibrinolysis by anabolic steroids.

The influence of anabolic steroids plus phenformin on the course of disease was studied in patients suffering from occlusive vascular diseases. In arteriosclerosis obliterans of the lower limbs the combined therapy led to a higher percentage of improvement than the control group showed. In coronary heart disease there was also a beneficial effect of this treatment. The drugs were found to exert beneficial clinical effects especially in patients with thrombophlebitis migrans and superficial thrombophlebitis. From the results of this study it is concluded that this therapy may be of clinical value in the treatment of patients with occlusive vascular diseases.

A case of congenital factor XIII deficiency.

The case of a 53 years old woman was described in whom a congenital factor XIII deficiency was suspected because of deforming scars and hemorrhagic diathesis. A thromboelastographic declination of elasticity as well as decreased factor XIII level up to 5% of normal range were only found in all hemostatic examinations. In 2 children factor XIII decreased to half of its normal level, whereas in the youngest daughter that level was 25%. Sporadically the girl had mild diathesis. No changes in thromboelastograms were observed in members of the patient's family. The platelet function was unchanged in all examined cases.

The influence of cytostatic treatment on the initiation of plasma coagulation in patients with neoplastic diseases.

The above described changes in the haemostatic system in acute leukemias are well known and underlined by many authors [1, 5, 6, 9]. It should be stressed that the results of particular nonspecific hemostatic tests in some patients may be within the normal range in spite of significant alterations in the activity of some blood coagulation factors and the presence of hemorrhagic symptoms. In the observations of some authors the factor VIII level is distinctly increased in the majority of acute leukemic cases, whereas the vitamin K-dependent blood coagulation factors show a low activity in some patients [6, 9]. It is not easy to interpret the different behaviour of the factor XI and XII level especially before antileukemic treatment. In 3/4 of all studied cases the factor XI activity was low, whereas the factor XII level was high in 1/4 of patients above the normal range. It may be that there is a specific inhibitor against the factor XI that is produced in acute leukemia. It must be stressed that the level of factor XI shows normal values during hematological remission.

Internal, neurologic, psychiatric and psychological characteristics of 25 cases of haemophilia.

The medical, neurologic and psychiatric complications in 25 hemophiles were analysed. The medical examinations did not show any changes connected with frequent blood transfusions, and bleedings to different tissues. The only changes found out were the result of local bleedings to joints. The psychological examination of the two persons suggested the brain injury, despite the lack of deviations from the norm at the neurologic examination including electroencephalography.

Localized intravascular coagulation in the mesenteric region.

Coagulation and fibrinolysis were measured in the portal and peripheral circulation in 15 rabbits after ligation of the superior mesenteric artery. It was found that the local coagulation syndrome in the mesenteric region can be recognized by tests in blood samples taken from the peripheral circulation area.

[Adenomectomy in hemophilia A patients].

A patient with haemophilia A is described in whom a loading blood-loss after adenomectomy developed. The congenital haemorrhagical diathesis was diagnosed only after the surgical intervention, since there did not exist any clinical symptoms. Taking into consideration world literature the problem of adenomectomy in patients with haemophilic is discussed.
