Photoinactivation and recovery in skin fibroblasts after formation of mono- and bifunctional adducts by furocoumarins-plus-UVA.

Cultured skin fibroblasts from young male guinea pigs were irradiated with UVA light in the presence of 8-methoxypsoralen (8-MOP) or angelicin. As compared to 8-MOP 30 times higher concentrations of angelicin were needed to obtain comparative inhibition rates of DNA-synthesis. Complete cellular recovery could be observed when the cell cultures were treated with angelicin-plus-UVA (320-400 nm) or 8-MOP-plus-395 nm. Both treatment schedules are known to cause monofunctional photoreactions. In contrast to this, bifunctional photoreactions caused by 8-MOP-plus-365 nm produced an inhibition of DNA synthesis which lasted more than four days. Also, UVA (320-400 nm) applied to cells treated with 3H-labeled 8-MOP resulted in a dose-dependent binding of 8-MOP molecules again lasting several days. Application of 8-MOP-plus-UVA (320-400 nm) to cells growing in log-phase showed a characteristic change in morphology. An increasing number of polynuclear and hyperchromatic cells appeared with time after treatment. In this subpopulation of cells DNA synthesis continued without division as revealed by DNA measurements and autoradiography. It is concluded that monofunctional adducts caused by angelicin-plus-UVA as well as 8-MOP-plus-395 nm permit cellular recovery whereas bifunctional photoadducts remained without recovery. In the latter case semiconservative DNA synthesis continued leading to hyperchromatic cells which could serve as a parameter for the presence of cross-linked nuclear DNA strands.

Polymorphonuclear leukocyte chemotaxis in psoriasis: enhancement by self-activated serum.

The chemotactic activity of polymorphonuclear leukocytes (PMNs) directed against serum was determined in a modified Boyden chamber assay. In a total of 176 experiments PMNs and sera from healthy controls were compared with PMNs and sera from patients with psoriasis. When PMNs from patients with psoriasis were confronted with psoriatic serum greatly enhanced chemotaxis was demonstrated. Non-psoriatic PMNs in the presence of psoriatic serum showed no enhancement. However, psoriatic PMNs in the presence of normal serum were chemotactically more active compared to non-psoriatic PMNs. Heat inactivation (56 degrees C, 30 min) reduced the chemotactic activity of all sera by nearly 50%. However in psoriatic sera the enhancement of chemotaxis was still present after heat treatment. The results indicate the presence of a functional abnormality of chemotaxis in psoriasis. This is likely to be caused by the in situ generation of chemotactically active fragments of complement. Experiments showing increased chemotactic activity of sera exposed to migrating PMNs support this concept.

Dominant sex-linked inherited chondrodysplasia punctata: a distinct type of chondrodysplasia punctata.

This paper suggests that there is probably a dominant, sex-linked type of chondrodysplasia punctata. Clinical data are reported for three girls with such a disorder. Two of their mothers showed a mild form of cicatricial alopecia. The pathognomonic dermatological findings in the children include erythematous skin changes and striated ichthyosiform hyperkeratosis during the first months of life. Later on, patterned ichthyosis, follicular atrophoderma, coarse, lusterless hair and cicatricial alopecia become evident. It is assumed that about one fourth of all cases with chondrodysplasia punctata reported in the literature belong to the dominant sex-linked type.

Therapy of psoriasis with retinoid plus PUVA: clinical and histologic data.

In a group of 40 patients suffering from wide-spread psoriasis oral administration of a retinoid (Ro 10-9359) was followed by PUVA therapy. The clearance rate was increased by 30% as compared to PUVA alone. Except for cheilitis no side effects were seen. Histological analysis in 20 patients before, during and after therapy revealed an intensification of psoriatic tissue changes after retinoid treatment. Loss of corneal layers, massive exoserosis, and neutrophil migration were prominent features. Mitotic counts were not increased by the pretreatment. The increased susceptibility of diseased skin to PUVA as produced by this drug appears to be based on several factors related to the tissue changes revealed by histology.

Thrombin and fibrin-induced growth of fibroblasts: role in wound repair and thrombus organization.

Skin fibroblast cultures were treated with various components of the blood clotting system (thrombin, fibrinogen and fibrin) during the logarithmic growth phase. Fibrin as well as thrombin showed dose-dependent growth promoting activities as revealed by cell counting and 3H-thymidine uptake. No effect was seen with fibrinogen. After entrapping in polymerizing fibrin enriched by complete culture medium the cells elongated, multiplied and formed net-like interconnecting cell strands throughout the clots. Nutritional deprivation appeared as a limiting factor for eventual growth cessation. The results demonstrate active growth of fibroblasts in fibrin clots such as present in healing wounds and thrombi. The production of thrombin by the coagulation cascade does not only result in the conversion of fibrinogen to fibrin but has also a long-lasting hormone-like effect on fibroblast proliferation which is of essential importance in wound healing, thrombus organization and progression of chronic atherosclerotic lesions.

Dose-effects of 8-methoxypsoralen and long wave UV-light in 3T3 cells: evaluation of phototoxic index.

3T3 cells were cultured until confluency and treated with various doses of 8-methoxypsoralen followed by long wave UV light irradiation. The inhibition of 3H-thymidine incorporation was dose-dependent for both, psoralen and light. A phototoxic index (PTI) was established demonstrating that a constant correlation between psoralen and UVA light exists for the photoinactivation in living cells.

Growth characteristics of skin fibroblasts and 3T3 cells entrapped by polymerizing fibrin.

Skin fibroblasts as well as 3T3 cells were cultured after entrapping freshly prepared cells in medium containing polymerizing fibrin. In contrast to cells grown on plastic substratum, fibrin-clot-cultured cells became highly elongated forming strands of cells. The strands interconnected by lateral cellular protrusions so that horizontal networks of cells were present throughout the clots. Cell growth as well as stretching were dependent upon the concentrations of fibrin. Highest growth rates were obtained with low fibrin concentrations (0.3 mg fibrinogen per ml). As shown by deprivation experiments nutritional limitations appear to be responsible for differences in growth rates observed in fibrin clots of higher density. In this system the fibrin meshwork serves as substratum for adhesion, elongation and multiplication of fibroblasts. The method makes it possible to study single cells in culture and the effects of persistent microenvironmental influences.

Photoinactivation of skin fibroblasts by fractionated treatment with 8-methoxypsoralen and UVA.

Guinea pig skin fibroblasts treated with low doses of 8-methoxypsoralen (8-MOP) and long-wave ultraviolet light (UVA) showed a dose-dependent inhibition of 3H-Thymidine incorporation as determined by liquid scintillation counting. The minimum incubation time necessary to obtain constant inhibition rates was 60 min. By washing the drug was removed from the reactive sites within 30 min. Repeated light exposure at a constant concentration of 8-MOP caused a cumulative inhibition of DNA synthesis. Irradiation of 8-MOP-plus-UVA treated cells, from which the drug was removed, produced a small increase in photoinhibition. Split dose treatment at various time intervals (ranging from 1--48 hr) revealed inhibitory rates, which correspond to the total amount of UVA applied. No recovery effects were seen in cultures treated by single or multiple applications of 8-MOP-plus-UVA.

Leiomyomatosis cutis et uteri.

A 32 year old female patient showed multiple plaque-like leiomyomata cutis and an uterus myomatosous of early onset. In the skin large areas of plaque-like confluent erythematous nodules consisting of smooth muscle tissue are formed. The patient suffered from frequent attacks of severe pain elicited by temperature changes or trauma. Treatment with nitroglycerine and nifedipine together with phenoxybenzamine completely suppressed the symptoms.

Photoinactivation of cultured skin fibroblasts by sublethal doses of 8-methoxypsoralen and long wave ultraviolet light.

Cultured guinea pig skin fibroblasts were treated with 8-methoxypsoralen (8-MOP) and UVA light. Determination of 3H-TdR-uptake as well as counting of the number of adherent cells was carried out 24 hr later. Incubation of fibroblasts with varying concentrations of 8-MOP (10(-4) to 10 microgram/ml) or 1 to 5 J/cm2 UVA alone showed no effect. When 8-MOP-photosensitization was followed by UVA a dose response was observed. This ranged over 3 orders of magnitude of the concentration of 8-MOP. Changes in irradiation energy produced a higher inhibition of 3H-TdR incorporation as compared to changes in 8-MOP concentrations. Using the same energy of UVA changes in which 3H-TdR uptake was inhibited by 50% showed no loss of plating activity. A fraction of these cells underwent DNA synthesis and division after reseeding. The results indicate that under the dose regimens currently used for the treatment of various skin disorders a proportion of the cells may become sublethally photoinactivated to undergo division. When reseeded these cells still are able to perform cellular functions such as spreading and attachment.

Dose-effects of 8-methoxypsoralen and UVA in cultured human lymphocytes.

Cultures of human lymphocytes were incubated with 8-methoxypsoralen (8-MOP) and irradiated with long wave ultraviolet light (UVA). Subsequently the mitogenic response to PHA was determined by 3H-thymidine incorporation. The minimal effective dosages of drug and light for the partial suppression of lymphocyte PHA-responsiveness were found to be 0.1 microgram/ml 8-MOP and 0.5 J/cm2 UVA. While an increase in the concentration of 8-MOP revealed a comparatively small rise in phototoxicity, a marked inhibition of 3H-thymidine uptake was obtained by increasing the UVA dosage. Cell viability of non-stimulated lymphocytes was similarly affected by the combined treatment.

Evaluation of PUVA, topical corticosteroids and the combination of both in the treatment of psoriasis.

A total of 90 patients with psoriasis were treated either by photochemotherapy (PUVA) or by topical steroids under occlusion (TOC). In a third group of patients a combination of both regimens was applied. All patients received approximately 15 treatments of either type. During therapy, biopsies were taken in all treatment groups and the epidermal thickness was monitored planimetrically. After clearing of the skin the patients were left without treatment and the time of onset of new lesions was recorded. While TOC produced rapid clearing of the skin this was followed by early relapses (in 50% of the patients after 3 weeks). PUVA reduced psoriatic hyperplasia more slowly; however, 50% of the patients remained free of psoriasis for approximately 10 weeks after clearance. Psoriasis treated with steroids under occlusion together with PUVA showed a significantly faster return of the skin to normal as compared to PUVA. As in PUVA-treated patients relapses occurred after approximately 12 weeks is this group. Therefore the combination of both treatments appears to be advantageous for rapid skin clearance and comparatively long remissions.

Leukocyte chemotaxis after in vitro treatment with 8-methoxypsoralen and UVA.

Isolated human polymorphonuclear leukocytes were submitted to long wave ultraviolet light (UVA) with and without preincubation of the cells with 8-methoxypsoralen (8-MOP). Leukocyte chemotaxis was determined in modified Boyden chambers using caseine as the attractant. Combined treatment (8-MOP + UVA) significantly inhibited the chemotactic activity with 8-MOP concentrations above 0.1 mug/ml and 2 J/cm(2). However, with high dosage of UVA (5 J/cm(2)) with and without 8-MOP still 25-30% cells migrated through the filters. Also, cell viability as determined by trypan blue exclusion was only moderately affected by combined treatment. The results indicate that these nonreplicating cells are comperatively insensitive to UVA and 8-MOP.

Effects of alkaline on horny layer cellularity.

The microscopical changes of plantar Str. corneum after immersion in alkaline were investigated. For this purpose a test system was used in which unexcised skin of guinea pig hind feet was exposed and subsequently analysed by the FITC-staining method. The effect of KOH on the Str. corneum consisted in an immediate swelling of the outer cell layers which was followed by a further increase in cell volume. Cells became separated layer by layer after maximal swelling. There is a high and constant rate of cell loss, when treated superficial cells are repeatedly removed as compared to permanent immersion. Thus, the buffering capacity of the treated outer cell layers has a rate limiting effect upon alkaline-induced cell loss in the Str. corneum.
