[Late results of microscopic nerve suturing and nerve transplantation].

20 patients with 30 damaged nerves have been reexamined after direct suture and nerve grafts respectively transplantation. The results obtained have been evaluated according to the principles of HIGHET and SEDDON as well as to those of CLEMENS. In 61% of the cases a good sensibility was retrieved. The results concerning motor function were found to be surprisingly satisfactory. The transplantation of short grafts showed especially good results. According to CLEMENS the reconstruction of the nerves was successful in 65% of the cases. The most satisfactory results were obtained at the median nerve.

[Asymmetrical macrodactyly].

A case of asymmetrical macrodactyly is reported with special regard to the conservation of the remaining function. It is shown that partial resection of the metacarpal bones, performed for functional reasons, leads to a satisfying result, while the amputation of the fingers, proposed for cosmetic reasons, is not recommended.

Experience with a two-staged pedicled flexor tendon graft.

A two-staged technique for flexor tendon grafting in Zone 2 is described using a silicone rubber rod which is replaced at the second stage by a somersaulted superficialis tendon graft sutured end-to-end to profundus tendon at the lumbrical level. The results are analysed.

[Indication for limb replantation (author's transl)].

The indication for replantation of traumatised limbs cannot be forced into any scheme. Each single case has to be critically considered as individual problem with main reference to age, sex, general condition, profession and motivation of patient and personal competence and qualification. A factor of central importance is the amputated part itself. The following facts are of special interest: which extremity is afflicted, the level of amputation, the extent of local tissue destruction, the duration of ischemia and measures of preservation.

[Results of 2-stage flexor tendon transplantation on zone 2].

For reconstruction of injuries of flexor tendons within zone 2, we used pedicled two-stage tendon grafts in about 50 cases. Proceeding from the concept that a pedicled tendon graft, at least partially vascularised, would tend less to adhesions, we combined the technique of PANEVA-HOLEVICH with the two-stage procedure. The results of 31 follow-up cases are reported and analysed by using the evaluation criteria of BUCK-GRAMCKO. Very good results were obtained in 17,4%, 30,4% were judged failures, good in 30,4% and satisfying results in 21,7%.

[Contribution to camptodactyly].

Camptodactyly occurs relatively often but it rarely needs therapy. For decades the genesis of camptodactyly was not clear until it was discovered that the deformations causing the contraction of the PIP joint are to be found in the area of the dorsal aponeurosis (MILLESI) and of the intrinsic muscles of the hand (WILHELM, KLEINSCHMIDT). At the Münster University anomalies of the lumbricals were found, on occasion of exploration of the palm, but not in all cases. Probably there exist several types of camptodactyly with different defects. The necessity for exploration of the palm is confirmed.

[The fixation of the flail chest with tension band wires (author's transl)].

A new technique of internal fixation of the flail chest was tested successfully in sheep. After reduction a 0.8 mm wire is passed twice intramedullary across the fracture and twisted under tension with two loops on the external side of the rib. This wire neutralizes bending and rotational forces and produces axial interfragmental compression. Combined with tension band wire fixation of an eventually fractures sternum it produced sufficient stability even in the very thin parasternal part of ribs. No respiratory support was necessary postoperatively. The method is simple and needs no special equipment.

[Model of intramedullary compression-osteosynthesis procedure for the humerus].

A method of osteosynthesis is being tested for its effectiveness by means of physican and experimental study and animal research. This could make it possible to stabilize fractures of the humerus diaphysis safely with pressure produced intramedullary on the interfragmentary, that is to say through "Kompressions-Bündelnagelung" (compression bundle nailing). Rating tests on skelleted animal bones have shown stability on quasi natural fractures surpassing the exercise load called for the upper arm. This procedure, however, does not prove rating stability by sole means of staying osteosynthesis with only one pair of nails on diagonal (sawed through) fractures of tibia diaphysis. Rating stability is gained however--even with unrelenting use--through "Kompressions-Bündelnagelung", that is an additional filling in the medulla ossium with a bundle of nails, the fracture having been braced already with the first pair of nails. To use this technique routinely a further simplification of process is necessary much like the one solving the distal guying. Also a reposition instrument is needed which would anable simple nailing. Such a repositor is now being tested at our clinics.

[Problems in replantation surgery].

The development of replantation centres presents problems with respect to availability of operating rooms and staff. Attention must be paid to improving facilities at such centres but care is required in selecting cases suitable for replantation. This involves consideration of distance between the patient and the centre, the level of amputation and the condition of the patient and the amputated part. Furthermore, age, occupation, pre-existing disease and concomitant injury must be taken into account. This not only will make inappropriate replantation of a single digit because of resultant impairment of the entire hand seldom but the problems of staffing and accommodation will also be minimized by correct decision.

[Ultrasonic welding of bones. Preliminary results].

Experiments with ultrasonic applied welding, a method first developed in the USSR are reported. In standardized isolated bone preparations stability values could be obtained of 10% [and in one case of 40% (!)] of the stability of the nonfractured bone. With these values the requirements of the method of osteosynthesis are met. Results of in vivo experiments are expected.
