Suppression of ultraviolet erythema by topical corticosteroids.

The effect of three commercially available corticosteroid preparations on ultraviolet (UV) erythema reaction was studied in healthy volunteers. Four series of circles were marked on the skin of their backs. One half of the circle of each series received 0.01 ml of 0.05% betamethasone valerate, 0.1% triamcinolone acetonide and 0.01% hydrocortisone 17-butyrate dissolved in isopropyl alcohol or ethanol and was covered with impermeable plastic. 24 h later, the sites were irradiated with UV light from a Kromayer lamp during 2, 4, 8, 16 or 32 sec. The reactions were evaluated 24 h afterwards. The suppression effect of corticosteroids was expressed as the blanching score (0-3). In a second series of experiments, the reversed sequence was tested: irradiation with UV light was followed by the application of corticosteroids (betamethasone and hydrocortisone). In both series of experiments the blanching effect of the corticosteroids was found to decrease with increasing UV dose. Corticosteroids applied after irradiation were found to have stronger blanching effects. With UV doses of 1 minimum erythema dose (MED), mean blanching scores of 2.3-3.0 were registered, while with doses of 4 MED or more, only small effects were noticed. The three corticosteroid preparations showed comparable suppression effects.

Immune reactivity of women on hormonal contraceptives: dinitrochlorobenzene sensitization test and skin reactivity to irritants.

To evaluate the influence of sex steroids on immunity in 87 women on hormonal contraceptives, sensitization tests were performed with the contact allergen, 2,4-dinitrochlorobenzene (DNCB). Forty-five women were taking oral contraceptives of combined oestrogen/progestogen of the same brand (low oestrogenic and middle range progestogenic activity); 27 had received intramuscular injections of medroxyprogesterone acetate and 15 women were using a sequential pill. The results were compared with those of 44 women not taking contraceptive steroids and matched for age. In women on the combined pill and those who had received intramuscular progesterone, the mean DNCB reactivity was significantly increased (0.02 less than p less than 0.05 and 0.001 less than p less than 0.01, respectively), whereas women using a sequential pill proved to show a decreased skin reactivity in the oestrogenic phase of this pill. (p = 0.05). The changes observed proved to be specific, since no statistically significant differences in overall skin reactivity to irritants between the four groups of women could be observed. The clinical implications of these results have been considered.

Allergic contact sensitization to the fungicide Maneb.

Three cases of contact dermatitis caused by Maneb, manganous-ethylene-bis (dithiocarbamate), are described. Maneb is used as an agricultural fungicide, and is also used in spraying solutions in horticulture and in flower shops. Patch testing with Maneb-related compounds revealed no regular pattern of cross sensitization.

Contact urticarial skin responses to cinnamaldehyde.

A patient suffering from an eczema of the right hand showed after routine patch testing an immediate urticarial reaction to cinnamaldehyde. The same reaction could be noted in three out of eight control persons. Laboratory investigations revealed cinnamaldehyde to have a histamine liberating effect, a fact not previously mentioned in reports on untoward reactions to this compound.

Contact hypersensitivity to sesquiterpene lactones in Chyrsanthemum dermatitis.

In six persons with contact allergy to chrysanthemum of florists, patch tests were performed with 25 sequiterpene lactones. Positive responses were observed to 15 lactones. The most frequently encountered positive responses were to alantolactone (positive in all cases) and to arbusculin A, 8-deoxycumambrin, ambrosin, damsin and psilostachynin. The findings corroborate the assumption that an alpha-methylene group attached to the gamma-lactone ring is an essential prerequisite for allergenic activity of sesquiterpene lactones. Pyrethrum produced a positive reaction in one of the patients, pyrethrins gave negative reactions in all. Some of the chrysanthemum patients were also hypersensitive to turpentine and colophony and to garlic, primin and tars.

Investigative studies of the dermatitis caused by the larva of the brown-tail moth (Euproctis chrysorrhoea Linn.) I. Clinical and experimental findings.

A description is given of both naturally occurring and experimentaly produced brown-tail moth caterpillar dermatitis. Epicutaneous test reactions were studied in a group of 45 persons, utilizing different setae preparations obtained from Euproctis chrysorrhoea L. About 70% of the individuals developed marked reactions upon contact with untreated nettling hairs. The early skin lesions were as a rule erythematous and wheal-like in character (urticarial), gradually becoming more infiltrated and developing into erythematous papules or vesicles. Treatment of the nettling hairs either by heating or saline extraction resulted in a partial loss only of their skin irritating properties; heat treatment being slightly more effective in this respect. Intracutaneous tests with a sline extract of nettling hairs elicited positive skin reactions in all individuals tested. Biochemical analysis of the crude extract revealed the presence of a potent esterolytic enzyme. Apart from their skin irritating action, the nettling hairs were found capable to induce the formation of spherocytes when mixed in vitro with human erythrocytes. The relevance of this phenomenon is discussed. On the basis of our findings and those of other it is suggested that the action of the nettling hairs upon the human skin is dependent both on a toxic and mechanical traumatic principle.

Dinitrochlorobenzene sensitisation test in women on hormonal contraceptives.

Cell-mediated immunity was measured in women on hormonal contraceptives using the 2,4-dinitrochlorobenzene (D.N.C.B.) sensitisation test. Three groups of women were studied: forty-eight women were taking oral contraceptives of combined oestrogen/progestogen (thirty-seven of them had used oral contraceptives for a year or longer); twelve women were using a sequential pill (ethinyloestradiol and megestrolacetate); and sixteen women had received intramuscular injections of medroxyprogesterone acetate. The results were compared with those of a control group of twenty-nine age-matched women not taking contraceptive steroids. In women on the combined contraceptive pill and those who had received intramuscular progesterone, mean D.N.C.B. reactivity was significantly increased. The results were even more striking in women who had used combined contraceptive pills for more than one year. These results do not accord with the previously reported decrease in cell-mediated immunity in women on hormonal contraceptives.

Susceptibility to primary irritants: age dependence and relation to contact allergic reactions.

Patch tests with three primary irritants were performed in 600 persons with eczema and in 33 healthy controls, The irritants assayed were: croton oil (20%) in mineral oil, thymoquinone (1%) in ethanol and crotonaldehyde (7.5%) plus sodium lauryl sulphate (4%) in aqua dest. The number of positive reactions to croton oil was found to decrease with age, while for thymoquinone and crotonaldehyde and for the total irritant score no age dependence was observed. No significant correlation was found between sensitization to common contact allergens and susceptibility to irritants. The incidence of positive reactions to common allergens proved to increase with age.

[Dermatologic aspects of glass-fiber production].

Workers in glassfiber industry are prone to develop skintroubles. Therefore an investigation was made to the factors responsible for symptoms. Patchtests were performed in 66 out of totally 160 workers on the spinfloor (spinners) with a standard series of common contact allergens and with raw materials and finishers used in glassfiber industry. In 36 (54%) persons positive tests were observed. Epoxy resin was found to be the most prominent allergen; 16 (or 24%) reacted to the substances. Positive reactions to finishers for the most part were caused by a hypersensitivity to this resin. Only 4 out of 56 spinners, intensively investigated for skintroubles, were found symptom-free. Besides burn and traumatic reactions to glassfiber the skintroubles ranged from a practically normal skin with some vesicles to extensive erythematous and papulous eruptions at the limbs, neck and face. Countings were made of the number of glassfibers present at the skin of the spinners after the end of the work. It was found that even after showering a great many glassfiber remain at the skin. The investigations indicate that skintroubles in workers in glassfiber industry may be elicited by two factors: an allergy to substances in the finishers, and traumatic (irritative) effects of glassfiber penetrating the skin.

Protease inhibitors in plasma of patients with chronic urticaria.

The hypothesis that deficiencies of plasma protease inhibitors might play a role in the pathogenesis of chronic urticaria was evaluated. Plasma levels were measured in patients with urticaria and a matched control group for alpha1-antitrypsin, alpha2-macroglobulin, total trypsin-inhibiting capacity, kallikrein-inhibiting capacity, and the complement factors C1 esterase inhibitor, C3, and C4. A total of 92 patients with chronic urticaria or more than three months' duration was studied. Patients with acquired cold urticaria had significantly decreased levels of alpha1-antitrypsin and total antitrypsin activity. In patients with acquired angioneurotic edema, alpha1-antitrypsin levels and antichymotrypsin activities were lowered, with less significant decreases in anti-trypsin and antikallikrein activities. Levels of C1 esterase inhibitor , C3, and C4 were normal in all groups. There was no correlation between the increased sensitivity to intracutaneously administered kallikrein injection and deficiencies of of protease inhibitors.
