[Reconstructive possibilities after burns of the female breast].

Reconstruction of the adolescent female breast after full-thickness burns differs from that of the mature female breast. Whereas a final correction is aspired to in the adult, the operative procedure should be adjusted to the evolution of the breast in the adolescent. Our therapeutic guidelines as well as the advantages and disadvantages of the different procedures are discussed.

[Corrective possibilities of burn contractures of the large joints].

Major joint movement is frequently limited following full-thickness burns of the extremities due to scar contractures. After operative correction of the contractures physical therapy is absolutely necessary. Skin contractures are treated with Z-plasty, skin graft, local flaps, pedicled flaps and free tissue transfer. The advantages and disadvantages of those procedures are described.

[Autologous injection of fatty tissue following liposuction--not a method for breast augmentation].

In recent years autologous fat transplantation employing liposuction has become an established method for correction of small soft tissue defects and irregularities of skin contour. Only a small amount of fat may be used because fat cells will be nourished by perifusion alone until neovascularisation occurs. Transplantation of a large amount of fat will result in fat necrosis. The inappropriate use of fat for breast augmentation after liposuction is described in a case report.

[Post-traumatic soft tissue defects of the heel].

Trauma induced soft tissue defects of the heel are likely to develop recurrent ulcerations along with osteitis of the calcaneus even after years of conservative treatment. Biomechanical as well as plastic surgical, traumatological and orthopedical shoe device problems which must be taken into consideration to manage the functional demands, are discussed.

[Multi-sided free transplants for closure of multifocal tissue defects].

Recently microsurgical tissue transfer--especially to the distal lower leg--for coverage of big soft tissue defects has been found to yield better results than conservative methods. However, there is still a problem providing coverage for multiple defects with only one tissue transfer. One solution is to employ multiple transplants with only one nourishing pedicle. Six patients have been treated successfully with such transplants. The operative procedure and the postoperative results are presented with a case report.

[Primary reconstruction in extremity saving resection of the lower extremity].

Limb sparing procedures have become possible in many cases of tumor resections of the lower extremity by using the new free-tissue transplantation technique. In some cases the stump can be elongated. Forty-four patients were treated for soft tissue sarcoma of the lower extremity. In only one case conventional amputation was necessary. In all other cases the limb was saved by primary reconstruction. In four cases a distalisation of the stump was achieved.

[Primary management and secondary reconstructive surgery of severe combined hand injuries].

Primary treatment and secondary reconstructive surgery of severe mutilating hand injuries are associated with challenging problems. Conventional and microneurovascular reconstructive methods must be integrated into the therapeutical rationale. The technical and tactical principles essential to obtain an optimal rehabilitational result are discussed. Simple sensitive grip tools will provide better functional long time results than modern prosthetic devices.

[Limb-sparing surgery in recurrent soft tissue sarcomas?].

Limb sparing resection in combination with radio-chemotherapy is recommended in cases of locally recurrent soft tissue sarcomas of the extremities. The clinical results of 41 patients, who underwent radical resection and additional plastic surgical reconstructive procedures for functional improvement are discussed. The integration of patients suffering from soft tissue sarcomas of the extremities into the treatment protocols of interdisciplinary tumor boards is mandatory.

Penile construction by the radial arm flap.

Penile construction in transsexuals has always been a challenge for plastic surgeons. After a long period of exploration with different techniques of penile construction, the author has found the free radial arm flap to provide the best results.

Catecholamines and tumour promoting phorbolesters inhibit insulin receptor kinase and induce insulin resistance in isolated human adipocytes.

The effect of the catecholamine isoprenaline (10(-5) mol/l) and of the tumour promoting phorbolester tetradecanoyl-beta-phorbol acetate (10(-9) mol/l) on insulin stimulated 3-O-methyl-glucose transport was studied in freshly isolated human adipocytes. Both substances reduced the maximal responsiveness of the glucose transport system to insulin by approximately 50%. To test if this is caused by inhibition of the insulin receptor kinase the receptor from phorbolester and isoprenaline treated cells was solubilized, partially purified and its kinase activity studied in vitro. Insulin stimulated 32P-incorporation into the beta-subunit of the insulin receptor of phorbolester or isoprenaline treated cells was reduced to 20-60% of the values found with receptor from control cells at insulin concentrations between 10(-10) mol/l and 10(-7) mol/l. This inhibition of kinase activity of receptor from phorbolester and isoprenaline treated cells was observed at nonsaturating adenosine triphosphate levels (5 mumol/l), and it could be overcome with higher concentrations of gamma-32P-adenosine triphosphate in the phosphorylation assay. A Lineweaver Burk analysis of the insulin stimulated receptor phosphorylation revealed that the Michaelis constant for adenosine triphosphate of the receptor kinase from phorbolester and isoprenaline treated cells was increased to greater than 100 mumol/l compared with less than 50 mumol/l for receptor from control cells. We conclude from the data that catecholamine and phorbolester treatment of human adipocytes modulates the kinase activity of the insulin receptor by increasing its Michaelis constant for adenosine-triphosphate, and propose that this modulation of receptor kinase is a mechanism that can contribute to the pathogenesis of insulin resistance in human fat cells.

[Management of acute facial injuries with microsurgical technics].

The indication of the microsurgical treatment of face injuries after amputation of the scalp, midface and ear is demonstrated by clinical cases. Since 1980-1987 we replanted five scalps, two midfaces and two ears. We could demonstrate good aesthetic and functional results.

[Late results in transplantation of free or pedicled radical flaps].

The free and pedicled forearm flap is well suited to cover large skin defects on the head, neck and extremities. It offers advantages in terms of size, shape, colour, skin thickness and a wide range of usage with combined tissue transfer. 31 patients with free or pedicled flaps were reexamined. The parameters of reexamination were blood flow, pulse, capillary return, and colour of skin. In half of the cases there was no restriction of blood flow; only one parameter showed signs of less blood flow. Every flap revealed pulse and good capillary return. Esthetic parameters were colour, hairgrowth and skin niveau differences. In 22 patients the results were esthetically satisfactory. The donor area was evaluated with respect to hand circulation in which the selected parameters were cold sensitivity, claudication, and the results of thermography.
