[Phage typing and plasmid characterization studies done on Salmonella typhimurium strains isolated in Ankara epidemic].

During our work which covers a period of 9 consecutive years, in the laboratories of two pediatric hospitals in Ankara Salmonella strains isolated mostly came out multiple resistant S. typhimurium strains. Phage typing and plasmid characterization studies showed that the cause of this situation was due to the presence of a FIme group antibiotic resistance plasmid. S. typhimurium strains carrying the same plasmid was shown to be present in many Middle Eastern countries located near by Turkey. The clinical problem is the resistance of this bacteria, which cause the epidemic, against the conventional drugs such as Chloromycetin and ampicillin used for the treatment of Salmonellosis.

[Salmonella meningitis].

Cultures were done for 14.838 cerebro spinal fluid specimens (CSF) during 1977-1980. Positive results were obtained from 950 specimens. These were identified as 352 Klebsiella-Enterobacter (37.0%), 133 Pseudomonas (14.%), 96 Salmonella (10.1%), 65 Pneumococcus (6,9%), 51 Escherichia coli 5,4%), 50 Meningococcus (.3%), Haemophilus influenzae type B (1.0%) and 127 various miscellaneous bacteria. During the same period of time, total of Salmonella isolations from various sources were 865. These specimens were 488 stool (57.0%), 160 blood (18.6% 7) 96 CSF (11.2%) so forth. The majority of these Salmonella strains were of typhimurium serotype carrying a known antibiotic resistence plasmid. This plasmid, belonging to the fime compatibility group, present in the Salmonella typhimurium strains isolated in Middle-East countries like Iran, Israel and others, conferred resistance to the carrying strain against antibiotics like ampicillin, kanamycin, chloramphenicol, streptomycin, sulphamide and tetracycline. As started to show up in Ankara too, addition of trimethoprim and gentamicin resistences to this markers were reported. These bacteria were also causing epidemics in pediatric wards of some South America countries which develops with hing rate of bacteremia and meningitis complications as it is in our hospital.

[Meningococcic meningitis epidemic in Ankara].

The meningococcic meningitis cases which appeared in Ankara in 1973 had ceased by 1976. The monthly distribution of the cases conforms well with the distribution schemes of other countries, where the peak months were the late winter and the early spring months. Beside the peak months some cases were seen throughout the year. Although the peaks disappeared after 1977, a few cases continued to be seen again. In the identification of the aetiological agent, the serogroup B was the predominant (70 cases, 45.5%) and the serogroup A was the next in prevalence (41 cases, 26.6%). When the aetiological agent was considered, no correlation could be established between the epidemic which had occurred in same periods in Ankara and in Istanbul.

[S. typhimurium strains which are H2S negative in TSI medium].

During an epidemic in the premature ward of Ankara Pediatric Hospital which lasted for almost 25 days, 15 cases were diagnosed and H2S (TSI medium) negative S. typhimurium strains were isolated 23 times on the specimens obtained from these patients. Specimens consisted of mainly stools but two were blood samples, one was a lung aspirate and two were pathologic materials obtained from the elbow and hip joints of the same patient. When these strains were transferred to Braun-Ozek B Medium, they were able to produce H2S which was detected because of the strong blackening on the lead acetate paper. Component replacement studies were performed to show which were necessary for the production of H2S in this second media. According to the results of this studies Pepton witte + meat extract broth + Na2S2O3 . 5H2O were found essential for the production of H2S in Braun-Ozek B. Medium. When this medium was prepared with the incorporation of ferrous ammonium sulfate instead of Na2S2O3, 5H2O, the production of H2S decreased and delayed.

[Three strains of Salmonella arizona isolated in microbiology laboratory of Hacettepe children's hospital].

We isolated three strains of Salmonella arizona during our routine daily laboratory work. Those isolations were made on patients who originally came from different parts of the country. Two isolations were made from the hospital four months after each other. Third isolation was made on a patient who was examined in outpatient clinic. This communication indicates first isolations and the presence of Salmonella arizona in Turkey.

[Aminoglycoside antibiotics: chemistry, modifying enzymes, present therapeutic values and recent developments].

This review article outlines the chemical structures and the location of the enzymatic inactivation sites which are sensitive to bacterial enzymes of aminoglycoside antibiotics. The distribution of these enzymes in different bacterial genera are also discussed. It is also noted that ineffectiveness of a new antibiotic may be due to the presence of preexisting inactivating enzymes. The great importance of these must be taken into an account before deciding to introduce a new drug into the therapy of local pathogens. Some new methods, like chemical alterations, are now available which renders aminoglycoside antibiotics to be resistant against these enzymes.

[A meningococcal epidemic in Ankara].

The meningococcal meningitis epidemic has been observed in Ankara in the last 3 years. Cases were from all parts of the country. Most of the strains isolated were from group B, while many could not be classified, and some of them were from group C, A and D. Because of fewer cases from group A and more from group B and C, the epidemic was not the classical meningococcal meningitis, but an intermediate one. Many cases could not be classified with the classical A, B, C, and D antisera. This showed that different serological groups existed in the country and should be taken into consideration in laboratory work.

[Salmonella serotypes isolated recently in Ankara from children with enteritis (author's transl)].

The results of serotyping done on 859 Salmonellas isolated between 25/2/1975--6/4/1977 are reported. In the series presented here percentage of S. thyhimurium is very close to that reported by us (91.37%) previously (2) showing that it is still the predominating serotype; But the important observations of the present study are the rises of the new serotypes which took the second and third places namely S. montevideo and S. thompson, indicating that clinicians should be aware of their presence. Under the impression of these findings the authors suggest to include the reagents prepared by S. thyphimurium's (i), S. thompson's (k) and S. montevideo's (g, m, s) flagellar antigens in addition to the routine Widal test antigens in Ankara hospitals.

[Nasopharyngeal culture results in children with acute otitis media].

Nasopharyngeal cultures in 463 children, suffering from otitis media were examined in 1968-1970. In 45% of cases H. influenzae, D. pneumoniae, S. aureus and beta hemolytic streptococci were found. These are thought to be the causative organisms for acute otitis media, however their dominance may vary. In our findings H. influenzae came first. 102 of 463 children, that is, 21.9% showed this organism. D. pneumoniae was second with 10.7%, which is generally considered as the primary cause of bacterial otitis media in children.

[Classification of acute bacterial meningitis cases according to their etiology, seen in a children's hospital in Ankara from March 1973 to April 1974].

In a 14 months period, as from March 1973 to April 1974 468 cases of acute bacterial meningitis have been diagnosed by C.S.F. findings; namely by examining the protein and sugar content, by the cell counts, Gram's staining and culturing of the sediments of C.S.F. 188 cases were proven by culturing to be meningococcic meningitis. All these cases are from the families living in suburban areas of the city under crowded circumstances, which show the importance of population density in the epidemiology of this particular disease.

[106 cases of infantile diarrhea caused by enteropathogenic E. coli 0111:B4 serotype in a 1974 epidemic in Ankara].

106 Enteropathogenic E. coli (E.P.E.C.) 0111 : B4 were isolated from the stools of children with gastro enteritis. 101 of these came from different pediatric wards, 5. from outpatient clinics with no history of previous hospital contact. In 16 months different E.P.E.C. serotypes were isolated, these are shown table VI. In spite of the uniformity of the serotype isolated during 1974 epidemic various serotypes were isolated in subsequent years. Among these serotypes 0111 : B4 was most prevalent, consisting of 47.3% of all isolations.

[ 771 Shigella strains isolated in a 15-year period from Americans living in Ankara. Comparisons with Shigella strains isolated from Turkish natives].

The author had worked as a bacteriologist in Ankara American Air Force Hospital from 1958 to 1972. During this period of time he was able to isolate 771 shigella strains from the American patients of the hospital. The amounts and the percentages of the subgroups are listed below together with the two other Ankara City Pediatric hospital results (see article). american community lived in Ankara for 15 years and shared many aspects which were important in shigella epidemiology like using city's tap water, shopping from the same green groceries to by fruits and vegetables so on. For shigella cases Ankara had seasonal alterations in July and August being the peak months. Americans, disease followed that course exactly. The big difference was being the local people of the city had those epidemics with the flexneri strains but Americans had them with the sonnei strains. To show the possible sources which could infect the community time to time and make sonnei predominant yielded negative result. All the patients stools were cultured. Sensitivity studies employing "plate disk diffusion" techniques were done. All patients treated accordingly. Stools checked for routine carriership exams. None become a chronic carrier. American installations had their shops, clubs, mess halls. The food handlers of these places had checked routinely by the preventive medicine office. None found infected. Only at 1963 the amount of flexneri isolations were higher than sonnei. All the remaining years sonnei were predominant. Antibiotic sensitivity studies showed no differences between subtypes. Except streptomycin and sulphamide strains stayed fairly sensitive to other antibiotics. Resistance factors contained maximum 6 markers. The little Americans community in Ankara consisted a unique and highly socioeconomic-wise developed group of the city. Many attempts to find the answer for the question is "who is infecting who" was always fruitless. That gives the impression as if the Americans were more sensitive to the shigella infections caused by sonnei strains for some reason. In another words Americans were more resistant to the infections other than sonnei.

[Providencia isolated from the stools of patients with gastroenteritis].

92 providencia strains were isolated from the stools of patients with gastroenteritis. All specimens were negative for other pathogenes such as salmonella, shigella, enteropathogenic E. coli and vibrios. Viral etiology was not investigated. The distribution according to years and months is shown in the table. 17 isolations made during August, seemed rather high and gave the impression of a small epidemic.
