Therapeutic embolization of renal cell carcinoma.

The technique of embolization of renal cell carcinoma is assessed on the basis of the authors' experience. The procedure is usually envisaged in case of hypervascularized extracapsular tumors, with the purpose of obtaining the preoperative occlusion of the renal artery. Nevertheless, it cannot replace surgery, which can accomplish it with much more safety. The technique of embolization itself, which should be as selective as possible, is also discussed and its clinicobiological (immunological) implications are introduced.

Tubulovasostomy in the rat. A new experimental model.

Tubular patency is the primary concern in the evaluation of procedures for epididymovasostomy. The epididymal lumen easily becomes occluded or obstructed, with the resulting recurrence of azoospermia. Recently, two-layer direct anastomosis has been advocated. The aim of the present study was to present an experimental model of tubulovasostomy in the rat. Microsurgery was performed in 18 rats (16-40 X magnification). Suture material used was 11/0 Vycryl or nylon for the internal layer of the anastomosis and 10/0 Ethilon for the external layer. After 6 and 12 weeks, semen samples were obtained by electroejaculation and evaluated for sperm count and motility.

Monolateral tubulovasostomy. Report of four cases.

After conventional vasoepididymostony, the epididymal lumen may become occluded or obstructed, resulting in recurring azoospermia. Recently, two-layer direct anastomosis has been advocated on the basis of experimental models of tubulovasostomy performed in rats. Monolateral tubulovasostomy was performed in four cases under 16-40 times magnification. Nylon or 11/0 Vycril was used as suture material for the internal layer of the anastomosis, and 10/0 Ethilon was used for the external layer. After three and six months, semen samples were evaluated for sperm count and motility. The presence of spermatozoa in all the samples confirmed 100% patency of the duct. Three patients had a number of spermatozoa compatible with fertility, and one pregnancy occurred.

Value of magnetic resonance imaging in the evaluation of sex-reassignment surgery in male-to-female transsexuals.

BACKGROUND: We investigated the value of magnetic resonance imaging (MRI) in the evaluation of sex-reassignment surgery in male-to-female transsexual patients. METHODS: Ten male-to-female transsexual patients who underwent sex-reassignment surgery with inversion of combined penile and scrotal skin flaps for vaginoplasty were examined after surgery with MRI. Turbo spin-echo T2-weighted and spin-echo T1-weighted images were obtained in sagittal, coronal, and axial planes with a 1.5-T superconductive magnet. Images were acquired with and without an inflatable silicon vaginal tutor. The following parameters were evaluated: neovaginal depth, neovaginal inclination in the sagittal plane, presence of remnants of the corpus spongiosum and corpora cavernosa, and thickness of the rectovaginal septum. RESULTS: The average neovaginal depth was 7.9 cm (range = 5-10 cm). The neovagina had a correct oblique inclination in the sagittal plane in four patients, no inclination in five, and an incorrect inclination in one. In seven patients, MRI showed remnants of the corpora cavernosa and/or of the corpus spongiosum; in three patients, no remnants were detected. The average thickness of the rectovaginal septum was 4 mm (range = 3-6 mm). CONCLUSION: MRI allows a detailed assessment of the pelvic anatomy after genital reconfiguration and provides information that can help the surgeon to adopt the most correct surgical approach.

[Bilateral abdominal testicles in the adult. Microsurgical and laparoscopic therapy].

B. L. a 27 year old bilaterally cryptorchid patient underwent right testicular autotransplantation in the presence of a quite normal testis. After one year the patency of microsurgical anastomosis was confirmed by means of Doppler flowmetry and scrotal echography demonstrated the presence in the scrotum of a testis provided will normal echogenicity. Left laparoscopic orchiectomy was planned. A small semilunar skin incision was made just below the rim of the umbilicus. Veress needle was introduced: as soon as the needle pierced the parietal peritoneum, its spring mechanism was released allowing the sharp needle point to retract leaving only the blunt tip protruding. Carbon dioxide gas was insufflated through the side part of the Veress needle until adequate abdominal distension was achieved. After having removed the Veress needle, the laparoscope on its sharp-pointed trocar was introduced into the peritoneal cavity and left testis was easily localized. Four trocars were introduced up to proceed to laparoscopic orchiectomy. The patient was discharged two days after. In our opinion in the presence of a bilateral cryptorchism in the adult, is better to plan a monolateral autotransplantation. After having verified the long-term result of microsurgery we can decide if a contralateral orchiectomy has to be planned.

[Bulbocavernosus reflex and somatosensory evoked potential of the pudendal nerve in diabetic impotence].

The present study investigated the sensitivity of the bulvocavernosus reflex (BCR) and the somatosensory evoked potential (SEP) of the pudendal nerve in neurogenic impotence, the study comprised 22 males with diabetes type II. Of these, 11 had neurogenic impotence. The SEP was found to be normal in all cases, whereas BCR changes were observed in 36.4% of the patients who were not impotent and in 63.6% of the impotent males. This difference, however, is not statistically significant. The results show that in diabetic pelvic neuropathy compromise of the vegetative fibers does not necessarily mean injury to the somatic.

[Renal cancer embolization with isobutyl-2-cyanoacrylate. Experimental study and first clinical applications (author's transl)].

Isobutyl-2-cyanoacrylate (IBC) is an acrylic monomer which quickly polymerizes in contact with blood. It has been rarely used as an embolic material and never in embolization of renal cancer. The authors report their experience on the use of this substance as embolic material, based on an experimental study in the rat and on two human cases of renal cancer, where a preoperative transcatheter embolization was performed. IBC revealed as a substance which can produce a very efficient and lasting embolization. No complications were observed.

[Treatment of bladder hemorrhages due to inoperable pelvic cancers by embolization of the hypogastric arteries (author's transl)].

Transcatheter embolization of the hypogastric arteries has become more and more widely used in the management of bladder hemorrages due to inoperable pelvic cancers. It is a not shocking technique for the patient, without major complications. However, if its immediate results can be defined as very good, its long-term effects are not well known. The purpose of this paper is to describe the Authors' experience on this matter, which is based on 9 cases of transcatheter embolization of the hypogastric arteries. The patients were followed for at least 1 year or, in some cases, until their death.
