[Role of penile ultrasonography in erection deficit].

The presence of penile nodule, curvature, disorders in blood flow to and from the corpora cavernosa is often correlated with organic sexual impotence. Echography allows to obtain a clear imaging of penile structures: diameters of corpora cavernosa can be measured before and after injection of vasoactive drugs. In our study penile structure were evaluated using a Combison 310 and a Toshiba with 7.5 MHz probe. In a lot of cases echography was repeated after intracavernous injection of PGE 1 or papaverine. The equipment needed for this evaluation is expensive but echography is undoubtedly less invasive than other equivalent examinations like cavernosography. Although the sensitivity and specificity of penile echography have not yet been clearly established this test is generally considered to be an useful and objective one. Penile echography is particularly usefull in discriminating between echogenic and not-echogenic nodules in case of Peyronie disease.

Angiographic treatment of high-flow priapism.

Priapism can be successfully treated by unilateral or bilateral percutaneous transcatheter occlusion of the internal pudendal arteries. Occlusion should be reversible in order to avoid impotence. Embolization with an autologous clot satisfies this requirement because of clot lysis and consequent vessel recanalization. This treatment is the most specific therapy in 'high-flow' priapism. The authors describe extensively 5 cases which demonstrate the feasibility of the method by which sexual potency is preserved.

Microsurgical spermaticoepigastric anastomosis for treatment of varicocele.

For varicocele correction, the occlusion of the internal spermatic vein is commonly performed, leaving the testis without its major pathway. Microsurgery offers the possibility to create a new venous drainage to the testis by anastomosing the internal spermatic vein with the inferior epigastric vein. Both stumps of the epigastric vein can be alternatively used, according to the different pathogenic mechanism of the varicocele. Eight patients with left varicocele were operated on using this new technique, with satisfactory results.

A new type of alloplastic spermatocele: the bridge spermatocele.

A new type of artificial spermatocele is proposed. A Gore-tex cylinder is sutured to the tails of the epididymides making a bridge between the two testes. The advantages of this spermatocele are (1) use of a single puncture; (2) easy execution of the puncture because the bridge stabilizes and holds the testicles in place; (3) a greater quantity of aspirate and, therefore, easier conservation of sperm; (4) a better quality of aspirate; and (5) relative ease of the surgical procedure.

The role of venography and sclerotherapy in the management of varicocele.

Venography has recently been introduced in routine diagnostic evaluation of varicocele, and a pathogenetic classification based on venographic findings has been proposed. Transcatheter sclerotization of the internal spermatic vein completes the angiographic management of varicocele. After venographic classification of 56 cases, the patients with type-1 varicocele (reno-spermatic reflux) were treated by transcatheter sclerotization (group A, 25 patients) or high inguinal surgical ligation (group B, 15 patients). The results were quite similar, whereas the cost/benefit ratio was favorable to sclerotherapy.

Treatment of priapism by transcatheter embolization of internal pudendal arteries.

Priapism can be successfully treated by unilateral or bilateral percutaneous transcatheter occlusion of the internal pudendal arteries. Occlusion should be reversible in order to avoid impotence. Embolization with autologous clot satisfies this requirement because of the clot lysis and consequent vessel recanalization. Three cases are extensively described which demonstrate the feasibility of the method and the ability to preserve sexual potency. The rationale for using interventional angiography in priapism is also discussed.

Transcatheter embolization with autologous clot in the treatment of renal bleeding.

Six cases of renal bleeding which were successfully treated by transcatheter embolization with autologous clot are described and illustrated. The problems of clot lysis and consequent vessel recanalization are discussed both from an experimental and clinical point of view. It is concluded that clot embolization is suitable treatment for renal bleeding and, in selected cases, is preferable to open surgery.
