Local transrectal hyperthermia (LTH) was used to treat 139 patients with prostatic adenoma aged 54-86 if they did not demand urgent surgery. LTH mechanism of action was studied by blood rheology, immunity characteristics, prostatic tissue gentamycin concentrations, morphological alterations after hyperthermia followed by TUR and adenomectomy. Clinical evaluation covered dysuria dynamics, uroflowmetry values, quantitation of residual urine and measurement of the prostate. The patients combined adenoma with chronic prostatitis, acute urine retention, cystostomy fistula (43, 22 and 16 patients, respectively). A microwave electromagnetic hyperthermia unit "Yakhta-4M" made in Russia (434 MHz, 200 W) heated the prostate to 43-44 degrees C. Two procedures a week of 60 min duration were performed within 3-5 weeks. LTH results in reduced blood viscosity, has no effect on blood coagulation, enhances neutrophil phagocytic function inhibiting their metabolic activity without affecting humoral immunity, raises 2-fold gentamycin concentrations in the prostatic tissue compared to blood and urine levels. Histologically, LTH does not alter prostatic parenchyma, but induces structural shifts in the muscular and connective tissue of the stroma producing stabilizing action on acinar epithelium. Clinical picture was characterized by subjective improvement in 72% of those treated, by urination recovery in 70% of the patients. 73% of the latter experienced cystostomy drainage which rid them of the fistula without operation. In general, mechanism of LTH action is brought to improvement of microcirculation, enhancement of cellular phagocytosis with a tendency to prostatic tissue sclerosing and stabilization of growth of the glandular tissue.
The causes, diagnosis, prevention and management of complications arising in insertion of internal ureteral stents have been analyzed basing on the results of 289 procedures in 272 patients. Dangers and complications related to drainage of upper urinary tracts (UUT) may be divided into technological (arising when inserting or removing the stent) and clinical (arising within the time of the stent location in the UUT). Technological complications emerged in 25 cases (8.7%). Of them, iatrogenic ureteral perforation occurred most frequently (9 cases, 3.1%). Clinical complications were registered in 41 patients (14.2%). Most common of them were: attacks of pyelonephritis due to vesicoureteral reflux and stent impassibility (19 and 9 patients, 6.6% and 3.1%, respectively). To ascertain the causes of the stent impassibility for urine, a complex of x-ray and ultrasound investigations was applied. The reflux is often provoked by the bladder drainage with urethral catheter. In the stent obstruction or concrement formation on it, the removal and changes of the stent were combined with extracorporeal or contact lithotripsy. Spontaneous distal migration of the stent (8 cases) necessitated its transurethral removal. Insertion of inner ureteral stents is rarely complicated by spontaneous migration of the drainage above the ureteral ostium (1 case) and by development of arterial-ureteral fistula (1 case). Thorough roentgenoendoscopical control over the stent insertion and staying in the UUT assists prevention and treatment of complications consequent to UUT internal drainage.

Ho-YAG laser (Medoptotech) operates in impulse regimen with transmission of radiation along the fiber 400-600 microns in diameter (mean power 12 W, impulse rate 1-15 Hz, energy of 1 impulse 0.7-1.5 J, its duration 300 microseconds. These parameters provide the effect of intensive biotissue evaporation, dissection of contact tissue, disintegration of concrements, hemostasis. Ho-YAG laser was employed for treatment of 8 patients with urethral polyp, 4 with stricture of the bladder neck and urethra, 5 with prostatic adenoma, 12 with bladder tumor, 10 with ureterolith, 5 with uroliths, 4 with penile lesions, 2 with ureteral stricture, 4 with ureterocele. Ho-YAG laser was used primarily for endoscopic interventions. Its use for open surgery is also promising. Further practical experience with Ho-YAG laser will provide additional information on its indications in urological diseases.
37 patients with cancer of the urinary bladder have undergone surgery and radiation with Ho-YAG laser. In 11 patients transurethral resection was followed by bladder wall ablation by impulse laser radiation (0.5-1.0 W, 3-10 Hz). Pilot results were promising: good coagulation effects, absence of secondary hemorrhage and infectious-inflammatory complications due to shorter postoperative drainage of the bladder and safety of ablation effect in exposure of the tumor base.
30 patients with strictures of the urethra (n = 19) and bladder cervix (n = 11) received combined treatment including surgical endoscopic correction of the stricture with Ho:YAG laser, pre- and postoperative endourethral radiation with low-energy laser, instillations of collagenase and dimexide solution into the urinary tract. The operation was made by Ho:YAG laser (2.09) microns, 4 J, 15 Hz, 300 mc). Persistent response for 10-12 months was achieved in 93.3% of patients.

One of the clinical and experimental investigations performed in the urological clinic of the Moscow Medical University and Research Institute of Urology aimed at elucidation of Ho-YAG-laser potential in endoscopic lithotripsy (EL). Russian Ho-YAG laser surgical units CTH-10 and LLT-3 with wave length 2.09 mu were employed. The studies showed that the developed laser units satisfied relevant medical and technical requirements. Low depth of laser impulse penetration (0.4 mm) obtained at EL warrants safety of the adjacent tissues which is essential in crushing fixed ureteroliths in the presence of severe inflammation in the ureteral wall. The conclusion is made that Ho-YAG laser EL is an effective treatment of ureteroliths. Further tests for EL efficacy in affections of the upper urinary tracts are advocated to define a proper place of EL in endoscopic urology.
The study of the PSA complexes with the protease inhibitors has confirmed the opinion of possible existence the complexes of the PSA with a2-macroglobulins and a1-antihymotripsines in blood serum. The immunoenzyme analysis of the PSA and its complexes has made it possible to establish the normal level of the PSA corresponding complexes in donors blood serum. There are reasons to suppose, that the quantitative analysis of the PSA and its complexes with the protease inhibitors can be used for the early diagnosis of PC and its differential diagnosis with the prostate benign hyperplasia. In our opinion, the effect of the complex approach can be more informative than the ordinary PSA detection.
