[Endoscopic occlusion of the bronchial stump in bronchial insufficiency].

The results of endoscopic occlusion of the stump of the main bronchus in its insufficiency were studied in follow-up periods of up to 3 years in 27 patients. The indications for the method were developed and the techniques of the occlusion described. Endoscopic occlusion is most expedient and sparing in early insufficiency of the bronchial stump under conditions of acute pyothorax and the presence of complications in a solitary lung. Its application is not opposed to the other commonly used methods of treating bronchial fistulas after pulmonectomy.

[Surgical treatment of complicated esophageal diverticula].

In 1973-86 the authors encountered esophageal diverticula (ED) in 44 patients. The disease was clinically manifested in all cases. Concomitant diseases of the abdominal and thoracic organs were found in 27 patients; ED followed a complicated course in 23 patients. Progressive clinical signs due to increase in the size of the diverticulum and the presence of complications are considered to be indications for operation. The surgery was performed on 18 patients. Complications developed in 4 cases after diverticulectomy. There were no fatal outcomes.

[Clinical value of transesophageal electric stimulation of the heart in patients with occlusive diseases and aneurysms of the aorta and major arteries during preparation for surgery].

A total of 325 patients with occlusions and aneurysms of the aorta and its main branches were investigated preoperatively. The patients were divided into 2 groups; in one of those (32 patients), esophageal electrostimulation of the heart was used for the diagnosis of latent coronary insufficiency. In addition to the degree of myocardial ischemia, the efficiency of antianginal drugs and the results of preoperative medication were assessed here. Comparative analysis of the results demonstrated a higher sensitivity and diagnostic value of esophageal electrostimulation, as compared to routine lead tests, which, in combination with the test's simplicity and noninvasiveness as well as possible identification of diagnostically-meaningful criteria, makes it a method of choice.

[Interrelations of changes in liver tissue and chemical composition of the bile and biliary calculi].

The correlation between the degree of morphological alterations of the hepatic tissues, changes in the chemical composition of bile and bile stone structure was established after comparing clinico-laboratory findings, results of examinations of liver bioptates, chemical composition of the bile and bile stones in 216 patients with cholelithiasis. The formation of cholesterol and mixed stones in the gallbladder is in correspondence with mild alterations of the liver tissue and colloid stability of the hepatic bile. The formation of pigment stones in the gallbladder and common bile duct is considered to be a symptom of chronic hepatitis followed by deep alterations of the chemical structure of hepatic bile.

[Odontogenic mediastinitis].

An experience with the surgical treatment of 28 patients with odontogenous mediastinitis is generalized, 12 of them died. The clinical picture is described in detail. Active surgical tactics is given the decisive role in the improvement of results of the treatment.

[Benign tumors and cysts of the mediastinum].

Data of examination of 208 patients with benign tumors and cysts of mediastinum are presented. The specific clinico-roentgenological symptoms of the disease are described by groups. Diagnostic possibilities of special methods of examination and problems of choice of the operative access are discussed. The operative complications are characterized. Postoperative lethality was 1,1%.

[Thoracoscopy in nonspecific spontaneous and iatrogenic pneumothorax].

Thoracoscopy was used in 92 patients. The etiology of pneumothorax was established in 76 patients. In most of them the treatment was effective. Suture of the pleura and lung defect, pleurodesis, pleurectomy, coagulation of tissues were performed under the control of thoracoscope. Thoracoscopy allowed to avoid extensive thoracotomy, to shorten the period of treatment at the hospital.

[Duodenal injuries].

Experience with the treatment of 33 patients with injuries of the duodenum is presented: 17 patients with ruptures of the duodenum, 16 patients with wounds. Post-operative complications were developed in 21 patients, 13 patients died. The two-row suture of the wound and decompression by nasogastroduodenal probe are considered by the authors to be the adequate operation for most of the patients.
