[Primary malignant mesenchymoma of the lung].

A case of a primary malignant mesenchymoma of the lung with elements of rhabdomyo-, leiomyo-, osteo-, fibro- and lipo-sarcoma is described. The tumor developed in a man of 58 with a long history of smoking, complaints of chest pains and cough with scanty sputum expectoration. The neoplastic process involved the upper lobe of the left lung and microscopically contained smooth muscle, endothelial, fibroblast-like cells, multinuclear giant cells resembling osteoclasts, strips of osteoid-like hyalinized connective tissue. Electron-microscopic examination revealed myoid-type cells with clusters of myofilaments and Z-type material, cells resembling fibroblasts, osteoclasts and lipocytes. It is suggested that there should be a common histogenesis of primary lung sarcomas arising from a stem cell precursor of mesenchymal origin in lung stroma, bronchial and vascular walls, pleura.

[Renal cell cancer in a polycystic kidney].

A case report of a renal (mixed) cell carcinoma found in a 35-year-old male with acquired renal polycystosis who had died from chronic renal insufficiency treated with chronic hemodialysis is presented. The morphogenesis and pathogenesis of acquired polycystic transformation of the kidneys in patients with chronic renal failure and the possible relation of the former to renal cell carcinoma are discussed. A preblastomogenic role is suggested for sclerotic changes arising in polycystic kidneys as well as for hepatitis B virus which may act as an oncogene.

[Peripheral cancer of the lung and tuberculosis].

Morphological analysis of 90 observations with a clinical diagnosis of a small peripheral lung carcinoma is performed. The examination of an operation material confirmed diagnosis in 71 cases. Tuberculomas are found in 19 cases. The peripheral lung carcinoma was found to develop in 91.5% (65 cases) against the background of preexisting scars which in 73.8% (48 cases) had a post-tuberculosis and 26.2% (17) post-pneumonia origin. Scars were most frequently related to the healed forms of a focal secondary tuberculosis (30 cases) and sclerotic changes around tuberculomas (8). Post-tuberculosis scars provoking sclerosis and deformation of vessels, bronchi, bronchioles and alveoles with the development of an epithelial dysplasia, are one of the risk factors in the development of a peripheral lung carcinoma. The degradation and fibrinoid changes of a scar tissue infiltrated by a tumour and followed by destruction of scars are observed in peripheral lung carcinomas with a diameter more than 3 cm.

[Criteria of organ specificity of a tumor].

On the basis of literature the markers of organ and tissue tumour specificity are reviewed. Signs of specific cell differentiation, similarity of tumour cells to the homologous cells of normal tissues are found electron-microscopically, histochemically (immunohistochemically and histoenzymochemically) in many organ-nonspecific tumours. Carcinoma cells with the ultrastructural features of organospecificity can be considered to be the cells-markers of the organ appurtenance of the tumor. Biological tumour markers (oncofetal antigens, ectopic hormones and enzymes) revealed until now in the serum and secretes of a tumour-bearing host, are not specific for a tumour of a single type. The search of criteria of tumour organospecificity (so-called biological, ultrastructural and histochemical markers) is necessary for precise tumour verification, determination of its histo- and cytogenesis and its primary location.

[Comparative histological and cytological characteristics of peripheral lung cancer].

Peripheral pulmonary carcinoma was analysed retrospectively histologically, histochemically, and cytologically. The examination material (smears taken at percutaneous thoracic aspirational biopsy, resected lung or lung lobe) had been obtained from 113 patients with PPC among whom 85 patients underwent pulmon- or lobe-ectomy. Cytological examination of specimens from percutaneous thoracic biopsy allowed the diagnosis of PPC to be established in 90.6% of cases and to determine correctly the histological type of cancer in 85.7%. No regular correlation between the levels of tissue and cellular differentiation in squamous cell carcinoma and adenocarcinoma was found. Argyrophilic granules (Grimelius reaction) were detected in cells of combined oat cell carcinoma and small cell carcinoma of transitional type. Because of the existence of combined tumors in the lungs consisting of cells of different types of differentiation it is suggested that complex histological types of pulmonary carcinoma can develop from a common polypotent precursor cell.

[Association of small peripheral lung cancer and early stomach cancer].

A combination of peripheral lung oat-cell carcinoma and early gastric highly differentiated adenocarcinoma in a man of 52 with long-term chronic peptic ulcer existing for 20 years is described. Electron microscopic and histochemical examinations of pulmonary carcinoma revealed neurosecretory granules and keratohyalin in the tumor cells. The gastric tumor on the ultrastructural level had cells differentiating in the direction of the glandular and intestinal epithelium. The presence of two independent malignant tumors in the patient was proven by the use of several morphological methods (cytologic, histologic, histochemical and electron microscopic).

[Cytology of large cysts of the breast].

Cytological examinations of punctates of 200 large mammary gland cysts distinguished three variants thereof: cysts without signs of epithelium proliferation; cysts with signs of epithelium proliferation; and cysts with signs of epithelium proliferation and atypia. In 12 operated patients with the cyst of the second variant no cancer was detected. Among 9 operated patients with cysts of the third variant 3 were found to have intraductal cancer with initial invasion (including one case of malignized cystadenopapilloma). Detection in cytological examinations of cysts with signs of epithelium proliferation and atypia is sufficient for excision biopsy. Such patients should be considered as a higher risk group.

[Histo- and cytochemical enzymatic characteristics of breast cancer].

In most of the cases studied, histo- and cytoenzymochemical values of metabolism in cancer of the mammary gland correlate with the histological type, differentiation level and the degree of the tumor malignancy. As the histological degree of malignancy increases and the level of differentiation of the mammary cancer decreases, the activity of glycolysis and pentous shunt enzymes, lactate dehydrogenase and glucose-6-phosphate dehydrogenase, increases and that of the Krebs cycle enzymes, succinate and malate dehydrogenase, as well as the enzyme of glycerophosphate shuttle mechanism and oxidation of glycerol-L-glycerophosphate dehydrogenase decreases simultaneously. These data suggest that histo- and cytochemical methods of the enzyme activity determinations may be used for the specification of the prognosis and choosing of the auxiliary methods for the therapy of the mammary gland cancer based both on morphology and metabolism of the tumour.

[Morphologic and molecular-genetic characterization of lung cancer developing in people who have worked at nuclear facilities and who have lived in Russian territories polluted after the accident at the Chernobyl power plant].

Clinicomorphological analysis of 15 lung carcinomas of patients who had been exposed for a long time to the radiation after the Chernobyl accident was performed. The material consisted of 10 surgical and 5 autopsy cases and was studied at the light, electron microscopic and immunohistochemical level. There were 6 peripheral, 8 central carcinomas and one massive tumor. Fibrous areas with many dust particles were found in peripheral carcinomas. In central tumors chronic obstructive bronchitis with epithelial dysplasia and metaplasia was observed. Carcinoma was represented by various histologic types: small cell (4 cases), combined small cell with squamous differentiation (5 cases), adenocarcinoma (5 cases), adenosquamous type (1 case). Peculiar calcium deposits in both stroma and parenchyma were found in tumors with glandular differentiation. Morphogenesis of calcium microdeposits may be connected with dust radioactive particles elimination. Central carcinoma had, in the majority of cases, a neuroendocrine differentiation and can be related to some types of small cell carcinoma. Peripheral cancer was mostly of a glandular differentiation and was, as a rule, carcinoma in the scar. Lung carcinomas studied had peculiar molecular-genetic features: lack or low bcl-2 expression, low Ki-67 expression and a high degree of c-myc expression. Tumors were characterized by a low apoptosis index independently of a histologic type. Apoptosis was not complete: lack of apoptotic bodies phagocytosis this resulting in postapoptotic detritus formation.

[Molecular pathology of lung cancer and insulin-like growth factors].

Molecular pathology of lung cancer (LC) investigates molecular-genetic rearrangements initiating development and growth of the tumor. The system of insulin-like growth factors (IGF) and binding proteins (IGFBP) regulates cell proliferation in the majority of embryonal and tumor tissues of man and animals in the course of reparation processes and productive inflammatory reaction. A general property of all LC histological types is presence in their cells of various members of IGF-system. Content of IGFII in tumor cells correlated with IGFBP-1, IGFBP-2, IGFBP-5. Location of IGFII and IGFBP in LC was different. IGFBP-1, -2, and -5, blocking IGFII, are detected in large amounts in areas of cell death inducing apoptosis while IGFII accumulates in dividing cells and foci of keratinization. Nuclear deposits of IGFBP-3 in bronchioloalveolar LC create phenomenon of intranuclear inclusion of "owl's eye" type. Synthesis of the majority of IGF occurs in tumor cells. Stromal cells also produce and transport into the tumor some quantity of IGFII and IGFBP.

[Bronchoalveolar neoplasms].

This review concerns the development and progression of one of the forms of peripheral lung cancer--bronchiolo-alveolar carcinoma (BAC). The study is based on the comparison of roentgenological, histological, immunohistochemical examinations and results of surgical treatment of 482 patients with this tumor. Morphological and clinical variants of BAC are defined. It is confirmed that their biological behavior may vary within the forms and differ from common adenocarcinoma. A close correlation exists between clinicoroentgenological picture and histological form of BAC.
