[The case of Wegener's disease of the nose, paranasal sinusses and ears].

The Wegener's granulomatosis is an autoimmunologic disease with still unknown etiopathogenesis. A rare case of Wegener's disease in 53-year-old female was described, located in her nose, paranasal sinuses and ears. We have also presented the diagnostic difficulties with this rare disease.

[The case of carcinoma adenoides cysticum of the tongue, the trachea and the thyroid gland].

We present a case of a 65-year-old female patient who was for the first time admitted to the clinic in 1997 due to a tuber of the tongue root. The removed tuber turned out to be histopatologically a polymorphic adenoma. The patient was re-admitted to the Clinic in 2001 due to a tuber of the tongue and of the oral cavity bottom. The tuber was removed entirely through a central incision, and an apart hypertrophic change has been found on the posterior pharynx wall and in the scar after the tracheostomy carried out during the previous surgery. In all cases carcinoma adenoides cysticum has been found, as well as metastasis into the thyroid gland and the lungs. The patient was qualified for chemotherapy in the Institute of Oncology, which she has been going through periodically every two weeks until now. In 2002 the patient was operated on a small tuber located hypodermically in the scar after the tracheotomy, which was removed. Carcinoma adenoides cysticum was found. In November 2002, during the surgery a tumorous infiltration of the thyroid gland was found comprehending trachea and reaching the mediastenum. In February 2003 the patient was re-admitted to the Clinic due to dyspnoea caused by a significant contraction of the trachea which occurred as a result of a focus of carcinoma adenoides cysticum and significantly enlarged lymph glands near the trachea. The patient was qualified for stent placement in the Institute of Pulmonary Disease and Tuberculosis (Instytut Chorób Płuc i Gruźlicy). The presented case describes an exceptionally aggressive and polyfocal regrowth and transformation of a polymorphic adenoma into cancer.

[The large tumour of the nasal cavity and paranasal sinuses--a case report].

The authors described the case of the large tumour of the nasal cavity and paranasal sinuses caused total occlusion of the right nasal cavity. The greater part of the tumour was outside of the nasal cavity. Computed tomography showed: a mass, which filled nasal cavity, maxillary sinus, ethmoid, frontal and translocated the septum. The rhinotomy was performed and the large tumour was removed. In histopathological test was found: polypus fibrovascularis oedematicus in statu inflammationis chronicae.

[Efficacy and safety assessment program of fusafungin in the treatment of acute upper respiratory tract infections in adults (PROFIL): multicenter study in Poland].

The aim of the study was evaluation of efficacy and safety of new form of fusafungine in the treatment of upper respiratory tract infections in adults. The study comprised 1639 patients treated in 288 centres in Poland. Fusafungine (Bioparox) was administered 4 applications to the pharynx and/or to the nose every 4 hours during the day. The result of treatment were observed after 5 days. Signs and symptoms of infections were observed both by the physician and by the patients. In 1188 patients regression of signs and symptoms was observed after 5 days of treatment both in bacterial and viral infections. Fusafungine in a new (without freon) form is efficacy and safety drugs in the treatment of upper respiratory tract infections in adults.

[Sinus barotrauma and the functional endoscopic sinus surgery].

Sinus barotrauma is quite common illness, which affects the passengers of the planes and the air staff. The authors describe possibilities of the course of treatment these patients with special regard to the functional endoscopic sinus surgery.

Implication of immunological abnormalities after adenotonsillotomy.

The adenoids and tonsils are thought to be essential parts of the system protecting organism against pathogens invading the upper respiratory tracts. Human adenoids and tonsils are known to be immunologically reactive lymphoid organs, which manifest specific antibodies and B and T cells activity in response to variety of antigens carrying out the functions of humoral and cellular immunity. The purpose of the study was to observe the changes in systemic immunity in children with hypertrophy of adenoids and tonsils treated in Department of Laryngology, Children's Hospital in Warsaw in period 1994-1999. The study comprised 80 patients (33 girls and 47 boys, aged from 3 to 14 years, mean age 6.8 years) with diagnosed hypertrophy of adenoids and tonsils. The diagnosis of hypertrophy of adenoids and tonsils was based on characteristic history and laryngological examination. All patients were scheduled for adenotonsillotomy. The control group comprised 40 people (14 girls and 26 boys, aged from 3 to 15 years, mean age 7.6 years) without history of the recurrent upper tract infections. In all patients we carried out following examinations: serum levels of immunoglobulins A, G, M (humoral immunity); percentage of T lymphocytes (CD3); percentage of T helper (CD4) and T cytotoxic (CD8) lymphocytes (cellular immunity) and delayed cutaneous hypersensitivity-Multitest CMI (cell mediated immunity). Our study demonstrate that in children with hypertrophy of adenoids and tonsils exist changes in the immunological parameters. The observed changes appear not only locally but also generally. In the early period after adenotonsillotomy there was statistically significant decrease of the values of humoral and cellular immunity parameters. However, 6 months after operation we observed normalization of examined immunological parameters. We think that the examinations of parameters of immunological system (humoral and cellular) are necessary before planned adenotonsillotomy.

[Usefulness of neuron specific enolase in patients with head and neck malignant tumors].

The authors present results of evaluation of neuron specific enolase (NSE) in 146 persons. We analyzed a concentration of NSE in preliminary study and in monitoring depending on clinical advances of tumour, mass of primary tumour and metastases to lymph nodes. Neuron specific enolase was excluded because the number of positive results was the same both in the main and control group. Our results indicate that evaluation of NSE have not height value in monitoring patients with head and neck carcinoma.
