Recovery of prostaglandin production associated with reopening of the ductus arteriosus after indomethacin treatment in preterm infants with respiratory distress syndrome.

Increased PGE production has been demonstrated in 9 of 17 preterm infants with patent ductus arteriosus (PDA) associated with respiratory distress syndrome (RDS). Inhibition of PGE production in eight preterm infants with PDA and RDS was associated with marked improvement in the respiratory and circulatory function of all of them. However, in six of them this effect was only transient. In the posttreatment period of five and a half days reopening of the ductus arteriosus was frequently associated with increased PGE production and a drop of indomethacin serum levels. Three of these six infants were transferred for surgical ligation whereas the other three were successfully treated with a second course of indomethacin. However, the margin between closure of the ductus arteriosus and the deterioration of kidney function in preterm infants treated with a presently recommended indomethacin dosage was narrow. In conclusion, until an acceptable therapeutic serum level of indomethacin for ductal closure in preterm infants has been established and the duration of effective prostaglandin synthesis inhibition is known, it is too early for a general recommendation of a dosage regime of indomethacin for the pharmacological closure of PDA in infants with RDS.

Beau's lines. A case report.

Beau's lines are a depression of the nail surface associated with a severe disturbance which temporarily interferes with the rate of nail growth. They appear several weeks or more after incurrence of the damage and continue to extend during nail growth. These lines were observed soon after birth in all twenty fingernails of an infant girl. The depression extended through the whole thickness of the nails. This would appear to be the first reported case in which Beau's lines have developed so early in life, probably as a result of intrauterine distress.

Alterations in antigenic properties of normal epidermis adjacent to basal cell carcinomas.

The expression of U-CYT antigens, tissue-specific cytoplasmic antigens of keratinocytes that are markers of the terminal differentiation of these cells, was studied in normal skin adjacent to malignant epidermal tumors. Ten basal-cell carcinomas were studied. In all cases, it was found that the expression of U-CYT antigens in normal epidermis gradually decreased with increasing proximity to the tumors and finally disappeared completely within the tumor tissue itself. This finding suggests that basal-cell carcinoma can alter the normal differentiation of adjacent nonmalignant cells.

[Lipoma of bronchus (author's transl)].

Lipomas of bronchus have a special clinical importance because of their predominantly endobronchial growth. Chronic obstruction and stenosis result in irreversible changes of lung. The clinical symptomatology is untypical. The X-ray examination points to the right diagnosis. The tumour can be localized bronchoscopically and recognized macromorphologically. The histological examination of a biopsy results in the exact diagnosis. This is a report of two cases.

Necrotizing myelopathy associated with acute lymphoblastic leukemia. Case report and review of literature.

The clinico-pathological findings are reported of a 16-year-old boy with acute lymphoblastic leukemia who during chemotherapy which included intrathecal Methotrexate developed a transverse cord lesion 5 months before death. Autopsy revealed necrotizing myelopathy of the T8--T10 segments and moderate lymphoblastic infiltration of the cerebrospinal meninges and spinal nerve roots. Neither parenchymal infiltration nor vascular lesions were found. The pathogenesis of this rare lesion associated with acute lymphoblastic leukemia remains obscure.

[Epidemiologic studies on the occurrence of hepatitis B in dentists and their assistants in Bavaria].

A study of 586 dentists and 553 dental assistants revealed that the contamination of dentists with hepatitis B was three times higher than in the comparable control group. No elevated hepatitis B morbidity rate however could be determined for the dental assistants. The occupation-related risk of hapatitis B infection was one case for every 100 dentists per year. No serologic differences from the normal control group could be determined for dentists and dental assistants in regard to contamination with hepatitis A. The evaluation of the questionaire showed that most dentists and their assistants were not aware of their hepatitis infection. Protective epidemiologic and hygienic measures, for the most part, were not observed.

[Atrial tachycardia with atrio-ventricular block in infancy and childhood (author's transl)].

Atrial tachycardia with atrio-ventricular block is a very rare arrhythmia in infancy and childhood. We saw ten patients between 3 days and fifteen years old. Six of them had a congenital heart disease, in four cases we could'nt find any cardiac or extracardiac illness. In every case digitalis intoxication was excluded. Clinical findings, electrocardiographic features, therapy and prognosis are reported.

[Testing of the "retinal acuity" with the laser interference apparatus as a routine clinical method].

The retinal function may be tested by laser-interference-fringes formed on the retina independently of the refraction status of the eye. The perception of these fringes is a subjective test for the retinal function even in case of moderate opacities of the optical media. Experiences with a new apparatus described here showed that the determination of this "retinal acuity" is very simple to perform. The comparison of retinal acuity and visula acuity of several hundred cataratous eyes and the measurement before and after cataract extraction in about 100 cases are discussed. The method gives useful prognostic and diagnostic information to the ophthalmologist.

[Congenital tracheal stenosis due to pulmonary artery sling (author's transl)].

Congenital stridor was observed in a newborn due to left pulmonary artery sling. Clinical signs and the course of the disease were determined by an extreme tracheal stenosis. In newborns and infants with apneic episodes, inspiratory and expiratory wheezing and cyanosis a pulmonary sling should be considered. Unilateral hyperexpansion of the lung and an anterior indentation of the esophagus are helpful diagnostic signs. Surgical treatment for pulmonary sling is possible but remains unsatisfactory. Patients with associated tracheobronchial stenosis show a high over-all mortality rate.
