[Effects of PCMV4-hTGF beta 1 as nucleic acid vaccine on II* burn wound healing and postburn scarring in rats].

OBJECTIVE: To investigate the gene expression of PCMV4-hTGF beta 1 as nucleic acid vaccine regulation and the effects on burn wound healing and scarring in rats and study the feasibility of gene therapy in burns. METHODS: Sixty wistar rats were divided into three groups: 1. burn + nucleic acid vaccine; 2. nucleic acid vaccine only(control); 3. burn only(control). ELISA was used to determine the dynamic changes in anti-TGF beta 1 neutralizing antibody in serum; determination of the quantities of DNA retained in muscles by Southern blot and in situ hybridization; observation of the ratio in collagen I/III during the wound healing by special stain method. RESULTS: The level of anti-TGF beta 1 antibody in serum reached the peak at the third week after naked DNA was injected, and a slight drop in the 4th week. TGF beta 1 plasmid DNA could be detected 5 minute after injection, and lasted 3 hours. In situ hybridization showed a positive staining in muscle fibers 5 days after injection. During the day 0-day 9, the wound healing speed in vaccine group was faster than control, and after day 10, no significant difference was found between groups, but the ratio of collagen I/III was reduced remarkably in vaccine group. CONCLUSION: Injection of PCMV4-hTGF beta 1 in rats can really cause general immune response reaction. It was showed that PCMV4-hTGF beta 1 was similar to TGF beta 1, and it had the effect of stimulating of epidermic cells to accelerate wound healing at early stage, and at later stage, it was similar to anti-TGF beta 1 neutralizing antibody, having the effect of inhibiting hyperplasia of scar. So it is confirmed that PCMV4-hTGF beta 1 as nucleic acid vaccine has the effect of promoting healing of burn wound and controlling the formation of scar.

[Comparison between copper needle insertion and stainless steel needle insertion into rabbit ear central veins].

OBJECTIVE: The clinical application of copper needle insertion has been proceeded satisfactorily for peripheral vascular disorders. This work was to study the mechanism of the copper needle insertion on a pathological basis. METHODS: Sixty rabbits were used in the experiments. The copper needle or the stainless steel needle was inserted into the ear central vein of the animal. Then the specimens of the central vein including its surrounding tissue were taken for light and electron microscopic examinations. RESULTS: At 7 days after copper needle insertion, acute inflammation reaction of the venous wall and vascular thrombosis were revealed in the specimens. At 14 days, giant cells granuloma and destruction of the venous wall were seen. After 28 days to 2 months, granuloma and the residual destroyed venous wall were gradually absorbed and fibrosis resulted. The vascular inflammation and thrombosis caused by stainless steel needle insertion was slight and slow. CONCLUSION: Copper needle treatment for vascular disorders is superior to the stainless steel needle.

[The application of external skin expansion in the soft tissue donor site].

OBJECTIVE: To overcome the insufficiency of donor tissue for repairing a large postburn defect. METHODS: The donor site was pre-expanded with an external expansion device in 54 clinical cases. RESULTS: The expansion process was uneventful. The external expansion had delay effect that enabled the flap to be transferred with greater length to width ratio. After external expansion, the donor site of a flap or skin graft could be closed directly. The recipient and donor sites healed simultaneously. CONCLUSION: External expansion is a practical method to provide 'extra' skin for reconstruction of a large skin defect.

Resistant mechanisms of Candida albicans to fluconazole.

OBJECTIVE: To detect the resistant mechanisms of Candida albicans to fluconazole (FCZ) at molecular biology level, since the resistance mechanisms of azole antifungal agents have been the focus of attention these years. METHODS: Thirty-two FCZ-resistant C. albicans were selected as our test strains (MICs > or = 64 micrograms/ml). With 14-alpha-demethylase gene (ERG16 gene, target enzyme encoding gene of azoles) as our object, we chose six sets of primers from the ERG16 gene to amplify the interested fragments, and conducted Southern blot hybridization, restriction fragment length polymorphism (RFLP), and single strand conformation polymorphism (SSCP) analysis for the fragments which were amplified by the six sets of primers, and pre-resistant sensitive strains were used as controls. Three representative fragments, A66, D66 and E78, were selected to be cloned and sequenced. RESULTS: The polymerase chain reaction (PCR) amplification showed that several tested strains were negative for some primers. However, our Southern blot analysis reminded that their resistance did not result from the lack of target enzyme coding gene. SSCP analysis showed that differences were noted between the resistant and sensitive strains and inter-resistant strains. Statistical analysis showed that the most variable sequence lied in the amplifier of the sixth pair of primer, and all the tested 32 strains showed positive results. In the 11 mutation points we found, five resulted in amino acid alterations. It is likely that one or more mutational alterations (alone or in combination) might lead to the expression of an enzyme highly resistant to the inhibitory action of FCZ which in turn is responsible for the FCZ resistant trait in these strains. CONCLUSION: One or more mutational alterations might lead to the azole resistant trait in this strains.

Identification of mutation in a candidate gene for hereditary multiple exostoses type II.

OBJECTIVES: To identify possible mutations in our previously cloned candidate gene for hereditary multiple exostoses type II (EXT2) in affected members of EXT families so as to confirm that it is the disease-causing gene. METHODS: The mutation was detected first by single strand conformational polymorphism (SSCP) of all coding exons of the candidate gene and then by sequencing analysis. RESULTS: After analyzing 37 patients from 20 Chinese EXT families by SSCP and DNA sequencing analysis, one 2-bp insertion mutation was identified in this candidate gene in affected members of an EXT family. This mutation resulted in the frameshift and generated a truncated gene product consisting of 105 amino acids. CONCLUSIONS: The identification of the mutation in the candidate gene indicates that this novel gene is responsible for EXT2 (one of the disease-causing gene of EXT).

[Study on electrochemical behavior of HL-60 cells during the etoposide-inducing apoptosis].

OBJECTIVE: To study the electrochemical behavior of apoptotic HL-60 cells induced with etoposide. METHODS: Electrochemical cytosensor was used to monitor HL-60 cells treated with etoposide. RESULTS: The peak current and electron transfer rate decreased as etoposide dosage increased. The decrease appeared at 2 micrograms/ml etoposide treatment and was remarkable at 20 micrograms/ml, which occurred before the typical changes of cell morphology and DNA contents for apoptosis. FCM-DNA analysis showed that the apoptotic cells were of 10.5%-20% while the peak current decreased from 1.8 microA to 0.4 microA after 2.3 and 4 hours treatment with 200 micrograms/ml etoposide, and the peak current after 4 hours treatment was 5 times lower than that before induction. CONCLUSION: The changes of electrochemical behavior during the early stage of apoptosis occurred earlier and could be accumulated.

In vitro activities of five antifungal agents against pathogenic Exophiala species.

OBJECTIVES: To examine the in vitro activities of five antifungal agents against common pathogenic Exophiala species; to detect the minimum inhibitory concentration (MIC) discrepancies among different drugs and different species; to evaluate the role of the influence factors in MIC determination; and to establish a standard method for the antifungal susceptibility testing of Exophiala spp. METHODS: MICs of itraconazole (ICZ), fluconazole, ketoconazole, amphotericin B (AmB) and 5-flucytosine against 5 species (57 strains) of Exophiala spp were determined by modified NCCLS M27-A broth microdilution method. Two inoculum sizes (0.5 x 10(3)-2.5 x 10(3) CFU/ml and 0.5 x 10(4)-2.5 x 10(4) CFU/ml) and three incubation times (4, 7 and 10 days) were evaluated. The minimum fungicidal concentrations (MFC) of itraconazole against 20 Exophiala strains were also detected. RESULTS: All the tested Exophiala species were sensitive to AmB and ICZ. MICs varied among different species and drugs. Resistant strains to different drugs existed in this genus. The proper inoculum density was about 0.5 x 10(3) CFU/ml to 2.5 x 10(3) CFU/ml, and the 4-day incubation time was suitable for more than 90% of the strains. CONCLUSIONS: AmB or ICZ may serve as the first choice in treatment of phaeohyphomycosis. A standard microdilution procedure for MIC detection of Exophiala species is established in this experiment. The inoculum size and incubation time are the essential factors in the standardization of antifungal susceptibility testing of filamentous fungi. We hope this experiment may correlate well with the clinical treatment of phaeohyphomycosis caused by Exophiala species.

[Follow-up study of lung function changes for 10 years in healthy elder of ex-smokers].

OBJECTIVE: To observe changes of lung function for 10 years in healthy elderly ex-smokers who have quitted smoking for more than 10 years. METHODS: 51 healthy elderly ex-smokers were selected and routine lung function tests were performed, 12 subjects, who have never smoked and all other criteria were similar, were chosen as controls. Their routine lung function tests have also been followed up for 10 years. RESULTS: The decrease of VC, FVC, FEV1 and PEF of ex-smokers were significantly lower than non-smokers, the increase of RV and RV/TLC were significantly higher than non-smokers. FEV1/FVC, MVV, MMEF, FRC and TLC were also different, however, the difference has not reached statistically significant. CONCLUSIONS: The influence of smoking could be prolonged, even the subject has stopped smoking for years, so we suggested to stop smoking the early the better.

[Clinical observation on treatment of endometriosis by tonifying qi and promoting blood circulation to remove stasis and purgation principle].

OBJECTIVE: To explore the therapeutic mechanism of endometriosis by the treatment of Yiqi Huoxue Huayu Tongfu Principle (YQHXHYTFP, tonifying Qi and promoting blood circulation to remove stasis and purgation therapy) with Neiyi 1+ pill. METHODS: Forty-five cases with endometriosis were divided randomly into TCM group (30 cases, treated with Neiyi 1+ pill) and the control group (15 cases, treated with tamoxifen). The activity of natural killer cell of peripheral blood, T lymphocyte subsets, secreting interleukin-II and the EmAb were determined. RESULTS: Endometriosis is associated with immunologic disturbance. After treatment, the activity of NK cells and the count of T-suppressor cells were significantly increased (P < 0.01), the count of T-helper cells, the secretion of interleukin-II and TH/TS ratio were significantly reduced (P < 0.01). Over 50% of the cases whose EmAb was positive eventually turned to negative. The effective rate of this therapy was 90%. There was no significant difference between TCM group and the control group. CONCLUSION: YQHXHYTF therapy could modulate the immunologic disturbance in women with endometriosis, improve the clinical symptoms and signs obviously. It is a safe and effective therapy.

[Clinical observation and experimental study on treatment of malignant tumor with bailong tablet].

OBJECTIVE: To observe the clinical efficacy of treating moderate and advanced malignant tumors with the compound Chinese herbal medicine, Bailong tablet (BLT) and to explore its mechanism. METHODS: Therapeutic effect of BLT in treating 34 cancer patients was analyzed according to the criteria of therapeutic effect for solid tumor, and changes on their living quality and immune function were observed clinically. In the experimental studies, the effect of BLT on human lymphocyte, celiac macrophage of mice and human gastric cancer cells in vitro were observed. RESULTS: The total remission rate was 11.8%, the remission-stationary rate was 82.3%, the living quality improved and immune function of organism enhanced, the difference between before and after treatment was significant (P < 0.05 or P < 0.01). Experimental study revealed that comparing with the control group, BLT can induce the activated human lymphocyte to kill target cell, promote the proliferation of T-lymphocyte, activate the phagocytosis of celiac macrophage in vitro(P < 0.05 or P < 0.01). CONCLUSIONS: BLT has definite therapeutic effect for moderate and advanced cancer, its anti-tumor effect was related to a series of cell biologic mechanisms, such as the immune function enhancement, T-lymphocyte proliferation and macrophage activation.

[Effect of total flavonoids of Astragalus on nitroxide in ischemia reperfusion injury].

OBJECTIVE: To study the protective effect of total flavonoids of Astragalus (TFA) on ischemia/reperfusion injury. METHODS: Change of nitrite (NO2-), the terminal product of nitric oxide (NO) metabolism, and the effect of TFA and chloroquine (a phospholipase A2 inhibitor) on it were observed with hemorrhagic shock/reperfusion injury (S/R) rabbit model. RESULTS: Plasma NO2- content was positively correlated with blood pH and total carbon dioxide content lowering. TFA and chloroquine could block the decrease of NO in certain degree and might have some effect on maintaining acid-base balance of body. CONCLUSION: TFA has protective effect on ischemia/reperfusion injury.

[A study of mitoxantrone with other chemical agents in treating 126 cases of adult acute myeloid leukemia].

OBJECTIVE: To estimate the effect of mitoxantrone with other chemical agents in treating adult acute myeloid leukemia. METHODS: 126 inpatients diagnosed as acute myeloid leukemia between January 1993 to December 1997 were treated with mitoxantrone with other chemical agents. 85 had not been treated previously, (untreated) while 51 were relapsing or resistant to previous treatment. RESULTS: The complete response rate among untreated and the relapsing/resistant groups were 44.7% and 27.5%; while the partial response rate were 20.0%, 9.8% respectively. 4 patients were in the untreated group died early. The main side-effect of mitoxantrone is bone marrow depression which led to fever and bleeding with rates of 40.4%, 24.3%. The non-bone marrow side-effects were tolerable. Damage to the liver was a prominent occurrence 31.8%. CONCLUSION: mitoxantrone was effective as the first line anti-tumor drug with other chemical agents in treating adult acute myeloid leukemia, its side-effects were mild.

[Minimally invasive approach for heart valve replacement: report of 73 cases].

OBJECTIVE: To report a new minimally invasive approach for heart valve operation. METHODS: Through right minimal vertical infra-axillary thoracotomy incision (by intercostal space), 68 patients with rheumatic heart disease, 2 patients with subacute endocarditis, and 3 patients with congenital heart disease underwent heart valve replacement from March 1997 to Spring 1998. RESULTS: 30 double valve replacements, 33 mitral valve replacements, 10 aortic valve replacements and 14 DeVega tricuspid annuloplasty were performed. The length of the incision averaged (8.5 +/- 0.9) cm, arrest times averaged (88.6 +/- 33.8) min, cardiopulmonary bypass times averaged (117.0 +/- 45.9) min. Five mitral valve replacements were performed with ventricular fibrillation. Ventilator periods averaged (9.6 +/- 3.6) h, chest tube drainage averaged (264.5 +/- 184.3) ml. One patient died of cerebral bleeding in hospital. Two patients suffered SBE and one of them died. The results of follow up for other patients were fine. CONCLUSIONS: This minimally incisive valve operation is a new safe, small wounded and cosmetic approach for heart valve replacement.

[Biomechanical evaluation of five fixation techniques for the lower cervical spine].

OBJECTIVE: This study biomechanically investigated the three-dimensional motion stability of five reconstruction methods in the cervical spine, in order to provide the biomechanical basis for the clinical selection of fixation methods. METHODS: With eight adult cervical spine fresh specimens, the three-column injury was produced at C(4 - 5) level. The spinal constructs, reconstructed by various techniques including anterior titanium locking screw plate (TLSP), posterior interspinous wiring (IW), combined fixation with the TLSP and IW (TLSP + IW), Roy-Camille plate (RP), and transpedicular screw plate (TP), were tested under six loading modes-flexion, extension, right/left lateral bending, and right/left axial rotation. RESULTS: The three-dimensional motion stability of either TLSP or IW was less than that of intact cervical spine. The TLSP + IW and RP provided increased stability compared with the intact spine. The stabilizing capabilities of transpedicular screw plate fixation was the best in all loading modes. CONCLUSIONS: In three-column instability of cervical spine injury, exclusive use of the anterior plate or the posterior interspinous wiring was not supported by the results. The stabilizing capabilities provided by combined anterior and posterior instrumentation and posterior plate were good. The three-column fixation for the cervical spine using transpedicular screw plate fixation offers increased stability significantly over that of other conventional cervical fixation systems.

[Expression and regulation of p16 in osteosarcoma].

OBJECTIVE: To study the expression and regulation of p16 and related factors in osteosarcoma. METHODS: The levels of p16 protein and related factors were explored by Western blot analysis in 12 cases of osteosarcoma tissues and in cells of different phases of osteosarcoma cell line OS732. The status of p16 protein in osteosarcoma cells was revealed by immunofluorescence analysis. RESULTS: p16 protein was detected in 10 of 12 osteosarcoma tissues. pRb was found in all of the specimens, but 8 of them were hyperphosphorylated. p16 protein was the least in G1 phase. Localized in cytoplasm during G1 phase, p16 protein gathered around nuclear envelope in S phase and entered the nucleus in G2 phase. CONCLUSIONS: The expression rate of p16 protein was 83.3% in osteosarcoma tissues. The expression of p16 protein was unable to inhibit the phosphorylation of pRb in most osteosarcomas; p16 protein was extremely low in G1 phase, presumably it was sufficient to inhibit the phosphorylation of pRb, causing abnormal proliferation of the cells.

[Refractory carotid-cavernous fistula: causes and countermeasures].

OBJECTIVE: To explore the causes and countermeasures of refractory carotid-cavernous fistula (CCF). METHODS: Twelve refractory cases from 123 cases of consecutive traumatic CCF during 12 years were reviewed. RESULTS: The main causes of refractory CCF were: small or large fistula's opening, or constrictive parent artery; inappropriate early treatments such as ligature of internal carotid artery or common carotid artery, balloon detachment of, and non-dense packing of coils inside the cavernous sinus; unexpected deflation or balloon displacement of resulting in fistula recurrence. Anatomical cure was achieved in 11 cases, and clinical care in 1 by using balloon or/and coil or/and NBCA (n-butal 2-cyanoacrylate) through arterial, venous or surgical approach. CONCLUSIONS: refractory CCF can be treated effectively skilled catheterization and embolization as well as appropriate approach and embolic material according to fistula structure and vascular route.

[Clinical analysis of orbital cysticercus cellulosae].

OBJECTIVE: To investigate the clinical features, laboratory tests, medical imaging manifestations, diagnosis and treatment of orbital cysticercus cellulosae. METHODS: 23 cases of orbital cysticercus cellulosae were analyzed from 1973 to 1997. RESULTS: Most cases had features of exophthalmos, lid swelling, limitation of ocular movement. Some of them with visual loss, optic papilledema and orbital mass. B-scan ultrasonography revealed a cystic tumor and a strong echo spot with spontaneous movement. On CT scanning, found was a well-defined hyperdensity mass in which there was a low density area. On MRI, the signal intensity of T(1)WI of cystic fluid was low or moderate and of T(2)WI was high, while the signal intensity of T(1)WI and T(2)WI of surrounding scar tissue was still low. CONCLUSION: We can make diagnosis by imaging tests and treat them with surgical operations.
