High frequency loss of heterozygosity in the region of the DBH locus in bladder cancer.

OBJECTIVE: To determine the location of a putative tumor suppressor gene (TSG), and evaluate the frequency loss of heterozygosity (LOH) of the long arm of chromosome 9 (9q) in bladder cancer. METHODS: We analyzed 25 patients with bladder cancer for LOH of 9q using 25 high-density microsatellite markers. RESULTS: Twenty-three samples (92%) showed LOH at least at one locus on 9q. We identified that the commonly deleted region were at 9q12-q21, 9q22, and 9q34. The rate of LOH was 44.0%, 22.7%, 22.7%, 16.0%, 12.0% on DBH, D9S15, D9S1815, D9S1831, D9S176 locus, respectively, and was not significantly related with grades and stages of tumor. CONCLUSION: These data suggest that alteration of a TSG at DBH of 9q may play an important role in the development of bladder cancer.

CT virtual bronchoscopy: imaging method and clinical application.

OBJECTIVE: To evaluate the imaging method and clinical application of CT virtual bronchoscopy (CTVB). METHODS: Fifty-two patients with bronchial and pulmonary diseases were studied with CTVB. All patients underwent fiberoptic bronchoscopy and patients with lung cancers were confirmed pathologically. Lung cancer of center type was found in 46 patients, postoperative lung cancer in 4, tracheal adenoidcystic carcinoma in 1, and bronchial diverticulum in 1. CTVB was performed using Navigator Smooth software on the workstation (Advantage Windows 3.1, GE Medical Systems). Source images included slice thickness of 3 mm or 5 mm, pitch of 1.0 or 1.5, 1.0 mm or 2.0 mm interval (overlap more than 60%) reconstruction. RESULTS: CTVB could reveal vividly the tracheobronchial lumens, the cartilage rings, the carina and the left and right main bronchi, down to the fourth order of bronchial orifices, mimicing fiberoptic bronchoscopy. Among 46 patients with lung cancers of center type, fiberoptic bronchoscopy showed the masses in 45 patients and CTVB displayed the masses in 42. The sensitivity of CTVB was 93.3% and its accuracy was 93.5% (chi 2 = 1.33, 0.10 < P < 0.25). The tumors appeared as masses or nodules, causing bronchial stenosis (n = 35) or occlusion (n = 7). The bronchial rings near the masses were blurred, smooth or absent in contrast to the findings of fiberoptic endoscopy. Postoperative bronchial stump (n = 4) appeared to be smooth. Bronchial diverticulum exhibited a local concavity on CTVB and local protrusion on surface shadow display (SSD). CTVB could pass through the stenotic bronchi and detect the occlusive bronchi from the distal end. CONCLUSIONS: The sensitivity of CTVB in detecting bronchial masses was higher than that of fiberoptic bronchoscopy. Combined with multiplanar reconstruction (MPR) and CTVB can demonstrate the extraluminal extension of tumors. As a noninvasive examining method, however, CTVB is limited to observe mucosal abnormalities and to obtain histologic samples.

[The digestive functions of the stomach after esophagectomy with vagus nerve preserved or severed in esophageal cancer patients: a comparative study].

OBJECTIVE: To study the feasibility, indication, and clinical significance of preserving the vagus nerve during esophagectomy for patients with cancer of the esophagus. METHODS: The digestive functions of the stomach were studied and compared before and after esophagectomy in patients with vagus nerve intact (n = 11) or severed (n = 12). RESULTS: According to Angorn's grading system, patients with vagus nerve preserved (VNP) had less symptoms after operation than those with vagus nerve severed (VNS). The gastric emptying time of the intrathoracic stomach in patients with VNS was much prolonged compared with that in patients with VNP. There was no significant difference in the preoperative and postoperative mean basal gastric acid output and the 24-hour pH monitoring in patients with VNP, while in VNS patients they were significantly decreased after operation. Fasting serum gastrin level in VNS patients was significantly elevated but this was not observed in VNP patients. Fibroptic endoscopic examination revealed higher incidence of postoperative atrophic gastritis in VNS than in VNP patients. CONCLUSION: Preservation of the vagal trunks during surgical resection for cancer of the esophagus is beneficial to keep the postoperative digestive functions of the stomach in a better condition.

[Correction of dento-maxillofacial deformities by using both of orthodontics and orthognathic surgery].

OBJECTIVE: To study the clinical results and stability after correction of dento-maxillofacial deformities by using both of orthodontics and orthognathic surgery. METHODS: A group of 45 patients with dento-maxillofacial deformities were corrected by preoperative orthodontics-orthognathic surgery-postoperative orthodontics procedure. As a control, another group of 64 patients with the deformities were corrected by orthognathic surgery only. The facial appearance, occlusion and stability of two groups after correction were investigated and compared. RESULTS: In orthodontics and orthognathic surgery group, 62.2% of patients declared that their mastication were improved. Relapse rate of deformities was 4.4%. In orthognathic surgery group, 42.2% of patients declared that their mastication were improved. Relapse rate of deformities was 9.4%. CONCLUSIONS: Using both of orthodontics and orthognathic surgery to correct dento-maxillofacial deformities is a good approach to improve results of correction, keep balance of occlusion and reduce the relapse of the deformities.

[Effects of various growth factors on human mandibular condylar cartilage cell proliferation].

OBJECTIVE: To investigate the effects of transforming growth factor-beta 1 (TGF-beta 1) insulin-like growth factor I (IGF-I) and basic fibroblast growth factor(bFGF) on human mandibular condylar cartilage (MCC) cell proliferation. METHODS: Isolated human MCC cells were cultured in DMEM supplemented with 10% newborn calf serum(NCS). The second passages were used in order to avoid chondrocyte dedifferentiation. Cells were seeded at 2 x 10(4)/well on 96-well plate. After synchronization, medium was replaced by DMEM containing 0.4% NCS or 10% NCS with various growth factors, concentrations and combinations. Dose-response and time-course were studied by MTT colorimetric method. RESULTS: In 0.4% serum containing medium, bFGF stimulated the proliferation moderately, whereas TGF-beta 1 and IGF-I had less effect. In 10% NCS condition, all three growth factors had mitogenic effect and acted dose-dependently. The effects were significant after three days. Among them, bFGF was a potent mitogen(increased by 65%), IGF-I the next(24%). The effect of TGF-beta 1 (13%) might be mediated by some other factors in the serum. The synergetic effects were achieved when they were used in combination. CONCLUSION: It is suggested that optimal combination of growth factors can promote the proliferation of MCC cells significantly, this might be an ideal way in dealing with cartilage damage during pathogenesis.

[Histopathological study of temporomandibular joint in early stage of polyarthritis rats].

OBJECTIVE: To furnish an experimental basis of the early effects of rheumatoid arthritis (RA) on the temporomandibular joint (TMJ) by observing the histopathological changes of TMJ in the early stage of polyarthritis (PA) rats. METHODS: The animal model of PA was established by injecting Freund's complete adjuvant (FCA) into rats. The histopathological changes of the rats' ankle joint and TMJ were studied. RESULTS: (1) The paw volume and the articular index of PA rats increased significantly, and symptoms of pronounced inflammation were present in the ankle joint specimens, which included proliferation of the synovium, infiltration with inflammatory cells, erosion into the articular cartilage and bone, and inflammatory exudation into the articular cavities. (2) The histopathology of TMJ of PA rats showed pronounced proliferation of the synovium cells, some lytic collagenous fibers and degenerative chondrocytes in the condyles, and exudated protein and desquamating cells in the articular cavities with some fibrinous adhesions. CONCLUSION: Pronounced inflammation occurs in bilateral TMJ in early stage of PA rats. The examination, protection and treatment of TMJ for RA patients should be taken into account in the early stage.

[Construction and expression of single-chain Fv antibody against human VEGF].

OBJECTIVE: Binding specificity of a monoclonal antibody (MAb) (gamma. k 2b) E11 which can react with the peptide of human VEGF was determined by ELISA and immunohistochemistry. The E11 primarily proved to be potential activity of suppressing tumor growth in vivo. The construction and expression of engineering antibody (ScFv) could widen application of this MAb in both diagnosis and treatment of human cancer. METHODS: From the hybridoma cell line secreting MAb against VEGF, total RNA was prepared and used as a template for cDNA synthesis and cloning. Reverse transcription and polymerase chain reaction (RT-PCR) were used to clone the immunoglobulin variable region gene (VH, VL). The cloned VH and VL were linked by well documented flexible peptide bridging sequence (GGGGS) 3 in the VH-linker-VL orientation. The constructed ScFv gene was inserted into pET-15b and expressed in E.coli BL21 (DE3). Biding activity of ScFv was evaluated by ELISA and immunohistochemical methods after its denature and renature treatment. RESULTS: The sequence analysis revealed that the full length of VH was 369 bp, and VL was 333 bp. According to Kabat classification, VH and VL are the member of mouse Ig variable gene heavy chain subgroup II (A) and light chain subgroup III, respectively. The ScFv was highly expressed in the form of inclusion bodies which activity was recovered after being renatured. Immunohistochemical results showed that ScFv retained almost the same antigen affinity and specificity as its parent monoclonal antibody. CONCLUSIONS: Recombinant antibody ScFv is potential in both diagnostic and therapeutic application of human malignant tumors.

[The role of nitric oxide in the pathogenesis of portal hypertension].

OBJECTIVE: To investigate the role of nitric oxide in the pathogenesis of portal hypertension. METHODS: We measured the portal venous pressure of 75 patients via a catheter and detected the levels of plasma endotoxin and NO(2)(-)/NO(3)(-). RESULTS: The mean levels of plasma endotoxin and NO(2)(-)/NO(3)(-) (0.249 +/- 0.112 Eu/ml and 55.9 +/- 26.2 micromol/L) in portal hypertensive patients were significantly higher than those in the controls, and the highest values were observed in the portal venous blood. The mean portal venous pressure of the portal hypertensive patients was 35.5 +/- 4.4 cm H(2)O and was positively correlated with the levels of NO(2)(-)/NO(3)(-) (n = 25, r = 0.55, P < 0.01). Positive correlation existed between them during the postoperative period (r = 0.57, P < 0.05). There was a negative correlation between the levels of albumin and NO(2)(-)/NO(3)(-) (n = 75, r = -0.35, P < 0.01), and the mean levels of NO(2)(-)/NO(3)(-) were 72.4 +/- 20.3 micromol/L in the patients with ascites, much higher than those (50.3 +/- 21.0 micromol/L) without ascites. CONCLUSIONS: The levels of plasma endotoxin and nitric oxide were elevated in patients with portal hypertension, and NO might play an important role in the abnormal elevation of portal venous pressure and the damage to liver function.

[Surgery for lung cancer complicated by bronchial asthma].

OBJECTIVE: To assess surgery for lung cancer complicated by bronchial asthma. METHODS: Lung hilum denervation was performed for asthma during radical resection of lung cancer. One patient with peripheral bronchogenic carcinoma in the right upper lobe underwent upper lobectomy and extensive dissection of lymph-nodes. Simultaneously, lung hilum denervation was done by complete amputation of the sympathetic and parasympathetic nerves around the lung hilum. Another patient with central bronchogenic carcinoma in the left upper lobe underwent lung autotransplantation. Pneumonectomy was performed, and the tumor parts of the lung were resected subsequently and the preservable parts of the lung were replanted into the bed of the upper lobe. RESULTS: Follow-up showed that the two patients had been alive with tumor free for 80 months and 17 months respectively and the time of postoperative mechanical ventilation was 4 and 18 hours. The frequency of asthma paroxysm decreased and the severity attenuated. Parameters of blood gas and lung function were improved. CONCLUSIONS: The poor lung function caused by asthma should not be regarded as a contradiction for the removal of lung cancer. Complete denervation around the lung hilum during the radical resection of lung cancer may contribute to the better treatment of lung cancer and asthma.

[Relationship between tumor suppressor gene p16 and Rb and early diagnosis of lung cancers].

OBJECTIVES: To study the incidences and mechanisms of tumor suppressor gene p16 and Rb inactivation in lung cancers, and to investigate their relation to biological characters, clinical pathological diagnosis and gene classification diagnosis of lung cancers. METHODS: Immunohistochemistry, double in situ hybridization, PCR, PCR-SSCP and sequencing were applied to study the changes of p16 and Rb genes in 106 lung cancers and 23 lung tissues of benign diseases. RESULTS: Total expression rates of p16 and Rb proteins and mRNAs in lung cancers were lower than those in normal lung tissues and benign lung diseases, and they were specially related to histological type, metastasis and clinical stage of lung cancers. In stage I and II lung cancers, the obvious inactivation of tumor suppressor gene p16 or Rb was examined (32.6% or 28.3%); p16 inactivation was detected mainly in non-small cell lung cancers, and Rb inactivation mainly in small cell lung cancers. There were three mechanisms of homozygous deletions, methylations and mutations for p16 gene inactivation. The deletion rate of p16 exon1 and/or exon2 was 25.8%, mainly took place in p16 protein negative cases of non-small cell lung cancers. 15 cases (16.9%) took part in methylations on SmaI sites of CpG island of p16 gene. PCR-SSCP and sequencing showed that 9 cases had p16 gene mutations. CONCLUSIONS: p16 and Rb genes may play important roles in genesis and progression of lung cancers. Inactivation of p16 or Rb gene may be is an early link of lung canceration, that is very important for early diagnosis of lung cancers. A new gene classification model for lung cancer diagnosis would be set up based on the research of p16 and Rb genes.

[CDKN2/p16 gene cloning and its probe preparation and application in lung cancer].

OBJECTIVES: To clone CDKN2/p16(INK4a) gene, prepare its probe, and to study the change of CDKN2/p16(INK4a) gene in lung cancers. METHODS: Total RNA of normal lung tissue was extracted, CDKN2/p16(INK4a) gene cDNA synthesized, and CDKN2/p16(INK4a) gene recombinant vector, constructed. Southern blot was used to study CDKN2/p16(INK4a) gene in 46 cases of lung cancers, 3 cases of normal lung tissues, 6 cases of lung tissues near cancer, and 3 cases of lymph nodes with lung cancer metastasis. RESULTS: Cloned CDKN2/p16(INK4a) cDNA was proved by enzyme digestion and sequencing. Southern blot showed 4.3 kb band in normal lung tissues and lung tissues near cancers, and deletion of CDKN2/p16(INK4a) gene in cancer tissues and lymph nodes with lung cancer metastasis, with a deletion rate of 17.4% (8/46). CONCLUSION: CDKN2/p16(INK4a) gene may play a role to some extent in progression of lung cancers.

[Multiple organ dysfunction syndrome after open chest wound and seawater immersion: experimental study].

OBJECTIVE: To evaluate the effect of seawater immersion on multiple organ dysfunction syndrome (MODS) after chest trauma. METHODS: Twenty health dogs were divided into two groups. Right open pneumothorax was induced in both control group (n = 10) and experimental group (n = 10). After induction of chest trauma, animals in the experimental group were immersed in artificial seawater. Blood samples were taken at seven different intervals for assessing blood gas, plasma level of TNFalpha, and IL-1 beta. Changes of serum alanine aminotransferase (ALT), aspartate amino-transferase (AST), creatine kinase (CK) lactate dehydrogenase (LDH) and creatinine (Cr) were measured. At the end of study, lung was harvested for assessing lung water content and ratio of wet weight and dry weight. RESULTS: A significant elevation of ALT, AST, CK and LDH was observed in both groups. Organ functional parameters in the experimental group were consistent with the failure standard at 30 minutes after seawater immersion, and those in the control group at 4 hours. Post-trauma mortality and incidence of MODS were much higher in the experimental group than those in the control group (P < 0.01). The plasma levels of TNFalpha and IL-1 beta significantly increased at 30 minutes and reached the highest level at 60 minutes after seawater immersion. The time of peak level appeared earlier in the experimental group than that in the control group. CONCLUSION: Seawater immersion after open chest trauma results in high incidence of MODS and high mortality rate due to progressive dysfunction of multiple organs.

[Hemodynamic and pathologic changes in open abdominal wound after seawater immersion in dog].

OBJECTIVE: To study the experimental basis for the treatment to the sick and wounded with abdominal open wound in land warfare and naval battle. METHODS: Animal model was made with adult dogs to observe the changes of mean artery pressure (MAP), pulmonary capillary wedge pressure (PCWP), central vein pressure (CVP), cardiac output (CO), cardiac index (CI) and pathologic changes of heart, pulmonary, hepatic, nephritic, intestinal, and gastric tissues under the optical and electrical microscope. RESULTS: Comparing with simple open abdominal, the hemodynamic disorder in open wound after seawater immersion was more serious; with a higher death rate. CONCLUSIONS: There was differences in hemodynamic and pathologic changes in seawater immersion wound and general wound. The body damaged is seriously when open abdominal wound immersed in seawater. Special measure must be taken in treating open abdominal wound after seawater immersion.

[An occlusive evaluation proposal to intra-saccular embolization of intracranial aneurysm].

OBJECTIVE: To explore the imaging criteria for evaluating intracranial aneurysm embolization. METHODS: Angiographic occlusive criteria for intracranial aneurysm embolization (aneurysm not opcified angiographically after embolization, 100% occlusion; a little part of aneurysm neck residual, 95%; neck residual, 90%; neck and a little part of aneurysm cavity residual, 80%; some cavity residual, < 80%) were used by 6 specialists to evaluate 121 aneurysms embolized with MDS (mechanical detachable spiral) and/or GDC (guglielmi detachable coil) from March 1995 to July 1999. The rationality, feasibility and limitation of the criteria were discussed. RESULTS: Among the 121 aneurysms, 100% occlusion was reached in 53 aneurysms (43.8%), 95% in 27 (22.3%), 90% in 16 (13.2%), 80% in 15 (12.4%), and less than 80% in 10 (8.3%) respectively. Good accordance was obtained among different doctors in the occlusive evaluation of aneurysm embolization. CONCLUSIONS: The suggesting criteria are simple and feasible in clinical practice, although the effect of imaging follow-up of embolized aneurysm is unknown and the numerical value of occlusive percentage is arbitrary.

Relations of tumor suppressor gene p16 and RB to early diagnosis of lung cancer.

OBJECTIVES: To study the incidence and mechanism of the tumor suppressor gene p16 and RB inactivation in lung cancer, and to investigate their relations to biological characteristics, clinical pathological diagnosis and gene classification. METHODS: Immunohistochemistry, double in situ hybridization, PCR, PCR-SSCP, and sequencing were used to study the changes in the p16 and RB genes in 106 patients with lung cancer. The specimen of normal lung tissue from the patients with lung cancer and specimens of the 23 patients with benign lung diseases were studied in contrast to those of the same patients. RESULTS: The total expression rate of the p16 and RB genes in the lung cancer tissues of the 106 patients was lower than that in normal lung tissues and tissues of benign lung diseases and was specially related to histological type, metastasis, and clinical stage of lung cancer. In stage I and II lung cancer, the obvious inactivation rate of the tumor suppressor gene p16 or RB was 32.6% or 28.3%. The p16 inactivation appeared largely in non-small cell lung cancer (50.1%), and the RB inactivation largely in small cell lung cancer (88.2%). Homozygous deletion, methylation, and mutation were three mechanisms for the p16 gene inactivation. The deletion rate of the p16 exon 1 and/or exon 2 was 25.8% in the patients with non-small cell lung cancer. Fifteen patients (16.9%) showed methylation at the SmaI sites of CpG island of the p16 gene. PCR-SSCP and sequencing revealed p16 gene mutation in 9 of the 106 patients with lung cancer. CONCLUSIONS: The p16 and RB genes may play an important role in genesis and progression of lung cancer. The inactivation of the p16 or RB gene may be an early sign of lung canceration, that is important for the early diagnosis. A new gene classification model for lung cancer diagnosis would be set up based on the research of the p16 and RB genes.

[Reversal of severe left ventricular dilatation after operation and its correlation with cardiac function].

OBJECTIVE: To investigate the relationship between the reversal of left ventricular dilatation and postoperative cardiac function, and prognosis. METHODS: 16 patients with severely dilated left ventricle underwent echocardiographic examinations preoperatively and at discharge (7 - 10 days after operation). Left ventricular (LV) diameters were measured (LVEDD, LVESD) and matched to body surface area (LVEDDI, LVESDI) and fractional shortening (FS); left ventricular ejection fraction (EF) was calculated. RESULTS: Left ventricular dimensions significantly decreased at discharge after operation. Preoperative LVEDD was (78.54 +/- 6.30) mm and postoperative LVEDD (63.77 +/- 13.69) mm (P < 0.01). Preoperative LVESD (60.03 +/- 8.95) mm and postoperative LVESD (53.23 +/- 12.68) mm (P < 0.05). Preoperative LVEDDI was (44.10 +/- 5.24) mm/m(2) and postoperative LVEDDI (36.01 +/- 9.53) mm/m(2) (P < 0.01). Preoperative LVESDI was (34.78 +/- 6.22) mm/m(2) and postoperative LVESDI (30.90 +/- 9.19) mm/m(2) (P < 0.05). Left atrium (LA) dimension was shorter after operation than before operation. Left ventricular construction didn't improve postoperatively, even temporarily decreased at discharge. Preoperative EF was (48.00 +/- 11.71)% and postoperative EF (40.15 +/- 8.69)% (P < 0.01). Preoperative FS was (22.82 +/- 6.27)% and postoperative FS (19.63 +/- 3.42) (P > 0.05). The incidence of postoperative cardiac arrhythmia was 75%. No dimension changes were observed in 2 patients with severely dilated left ventricles after operation: one (LVEDDI 56 mm/m(2)) died, and the other had severe low cardiac output. Six of 14 who had reduced dilated left ventricle didn't reach normal limits, still in the range of severe dilated left ventricule (LVEDDI > 37 mm/m(2)). CONCLUSIONS: Most patients decreased the dimension of left ventricular size early after operation, but some had partial recovery from severe ventricular dilatation. Although left ventricular dimensions (LVEDD, LVESD, LVEDDI, LVESDI) significantly reduced, cardiac function didn't improve at once with the reversal of left ventricular dilatation, but temporarily decreased after operation and the incidence of cardiac arrhythmia was high.

[Study on the epidemiological features of Kawasaki disease in Jiangsu].

OBJECTIVE: To investigate the incidence of Kawasaki disease and epidemiological characteristics in Jiangsu province. METHODS: Questionnaire and diagnostic guidelines for Kawasaki disease with color - printed photographs of typical clinical symptoms were sent to all county hospitals with pediatric beds. All the in - patients with Kawasaki disease hospitalized from January, 1993 to December, 1997 were studied regarding age, sex, season of illness, cardiac sequelae, outcome etc. RESULTS: There were 509 cases (329 males and 180 females) diagnosed with 489 cases (96%) clinically typical. The incidence of the 5 - year period was averaged 2.53 per 100,000 children under 5. The number of male patients with Kawasaki disease was higher than that in female. The age peak of incidence was between 6 month and 2 - year olds. 13.6% of all cases developed cardiac sequelae which mostly occurred in males and in those younger than 2 years of age. There was one patient recurred among total patients reported (recurrence rate 0.2%) and 2 patients died (fatality rate 0.4%). CONCLUSION: The epidemiological characteristics of Kawasaki disease in Jiangsu province were similar to those in Japan in sex, season distribution and cardiac sequelae. The number of patients in male was higher than in female. Patients were more commonly seen in spring than in winter. However, the incidence rate in Jiangsu was significantly lower than that in Japan and the age peak was between 6 month and 2 years old.

[Analysis on the intervention effect of HIV/AIDS cases].

OBJECTIVE: To study the effect of a comprehensive intervention strategy on the HIV infected individuals and their families. METHODS: Eleven HIV positive persons and their families were included to a comprehensive intervention strategy including routine surveillance, safer sex education and improving the status of hygiene. The outcome was compared with 2 fully developed AIDS cases who were not included to the intervention strategy. RESULTS: Of those HIV positive persons, that subjected to the intervention strategy, none had developed to AIDS, with only 1 transmitted HIV to his spouse among the 6 married individuals. All of their families were stable. Of those that not had been subjected, 2 individuals developed to AIDS and transmission in family was confirmed. CONCLUSION: The comprehensive intervention strategy on HIV/AIDS was effective which could the possibility of HIV in-family transmission and enhance the quality of life and the stability of their families.
