Mutation analysis of the Smad6 and Smad7 gene in human ovarian cancers.

The Smad6 and Smad7 genes are members of the Smad family, involved in the transforming growth factor-beta (TGF-beta) signaling pathway. Mutations in TGF-beta receptors and their cytoplasmic elements of transduction signals commonly accompany various cancers. Using PCR-SSCP analysis we searched for the presence of Smad6 and Smad7 gene mutations in 30 human ovarian cancers and 4 ovarian cancer cell lines, and found that 12 cases (35.3%) had a polymorphism in intron 2 of the Smad6 gene and that 8 cases (23.5%) had a polymorphism at codon 208 in the Smad7 gene. Because these polymorphisms were not accompanied by amino acid substitution, the present results show that the mutations in the Smad6 and Smad7 genes are unlikely to be involved in human ovarian cancers.

Computer-assisted three-dimensional surgical planning and simulation: 3D color facial model generation.

A scheme for texture mapping a 3D individualized color photo-realistic facial model from real color portraits and CT data is described. First, 3D CT images including both soft and hard tissues should be reconstructed from sequential CT slices, using a surface rendering technique. Facial features are extracted from 3D soft tissue. A generic mesh is individualized by correspondence matching and interpolation from those feature vertices. Three digitized color portraits with the "third" dimension from reconstructed soft tissue are blended and texture-mapped onto the 3D head model (mesh). A color simulated human head generated from frontal, right and left real color portraits can be viewed from an arbitrary angle in an inexpensive and user-friendly conventional personal computer. This scheme is the basic procedure in 3D computer-assisted simulation surgery.

Computer-assisted three-dimensional surgical planning and simulation: 3D virtual osteotomy.

A computer-assisted three-dimensional virtual osteotomy system for orthognathic surgery (CAVOS) is presented. The virtual reality workbench is used for surgical planning. The surgeon immerses in a virtual reality environment with stereo eyewear, holds a virtual "scalpel" (3D Mouse) and operates on a "real" patient (3D visualization) to obtain pre-surgical prediction (3D bony segment movements). Virtual surgery on a computer-generated 3D head model is simulated and can be visualized from any arbitrary viewing point in a personal computer system.

[Normal and pathological facial mimicry: objective analysis].

INTRODUCTION: Objective evaluation methods for facial nerve function are yet to be standardised. Unresolved issues include what should be measured for different facial movements and how these measurements should be performed. METHODS: 5 facial mimics were analysed in normal subjects and in patients with facial paralysis. For each mimic the best measurement was determined and two global indexes derived. RESULTS: The best measurements were of surfaces close to the moving facial zone. These measurements and the global indexes correlated well with gradings by the House-Brackmann scale.

Inhaled isotretinoin (13-cis retinoic acid) is an effective lung cancer chemopreventive agent in A/J mice at low doses: a pilot study.

In previously treated head-and-neck cancer patients, p.o. administered isotretinoin (13-cis retinoic acid) reduced the occurrence of second aerodigestive tumors, including lung tumors, but side effects made chronic therapy problematic. We reasoned that inhaled isotretinoin might provide sufficient drug to the target cells for efficacy while avoiding systemic toxicity, and we proceeded with the pilot study reported here. Male A/J mice were given single i.p. doses of urethane, a common experimental lung carcinogen, or benzo[a]pyrene (BaP) or 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanone (NNK), putative major carcinogens in tobacco smoke. The following day, exposures to isotretinoin aerosols for 45 min daily at 1.3, 20.7, or 481 microg/l were initiated. After 2 weeks, the high dose caused severe toxicity on the snout skin, necessitating a reduction of dose frequency to twice a week. As a precaution, the mid dose was reduced to three exposures per week. The weekly total deposited doses after the dose frequency reductions were calculated to be 0.24, 1.6, and 24.9 mg/kg for the low, mid, and high doses, of which 16% was estimated to be deposited in the lungs. The weekly deposited pulmonary drug doses were calculated to be 0.01, 0.07, and 1.1% of a previously reported ineffective oral dose in urethane-treated A/J mice. After 10-16 weeks, mice were sacrificed to count areas of pulmonary hyperplasia and adenomas. For all carcinogens, the mice exposed to the high isotretinoin dose showed reductions of tumor multiplicity ranging from 56 to 80% (P < 0.005). The mid dose was associated with reductions of tumor multiplicity by 67 and 88% (P < 0.005) in BaP- and NNK-treated mice, respectively, and was tolerated until approximately 12 weeks, when both these and the high-dose mice began losing weight. The low-dose mice had nonsignificant reductions of 30% (P < 0.13) and 16% (P < 0.30) for BaP- and NNK-treated mice, respectively without any evidence of side effects. For BaP- and NNK-treated mice, numbers of hyperplastic areas directly correlated to dose level and inversely to tumor number, suggesting arrested progression. Inhaled mid-dose isotretinoin caused up-regulation of lung tissue nuclear retinoic acid receptors (RARs) relative to vehicle-exposed mice, RARalpha (3.9-fold vehicle), RARbeta (3.3-fold), and RARgamma (3.7-fold), suggesting that these receptors may be useful biomarkers of retinoid activity in this system. The encouraging results from this pilot study suggest that inhaled isotretinoin merits evaluation in people at high risk for lung cancer.

Computer-assisted three-dimensional surgical planing and simulation. 3D soft tissue planning and prediction.

The purpose of this paper is to report a new technique for three-dimensional facial soft-tissue-change prediction after simulated orthognathic surgical planning. A scheme for soft tissue deformation, "Computer-assisted three-dimensional virtual reality soft tissue planning and prediction for orthognathic surgery (CASP)", is presented. The surgical planning was based on three-dimensional reconstructed CT visualization. Soft tissue changes were predicted by two newly devised algorithms: Surface Normal-based Model Deformation Algorithm and Ray Projection-based Model Deformation Algorithm. A three-dimensional color facial texture-mapping technique was also used for generating the color photo-realistic facial model. As a final result, a predicted and simulated patient's color facial model can be visualized from arbitrary viewing points.

Comparative population pharmacokinetic-pharmacodynamic analysis for piroxicam-beta-cyclodextrin and piroxicam.

Piroxicam (Feldene) is indicated for osteoarthritis and rheumatoid arthritis but not analgesia due to its delayed onset of pain relief. Piroxicam-beta-cyclodextrin (PBCD) was developed for pain indication by virtue of the increased absorption rate of piroxicam. Forty-eight patients received a single dose of PBCD or Feldene (10, 20, and 40 mg) in a randomized study, and piroxicam plasma concentration and pain relief were measured. The purpose of the study was to investigate the PK-PD relationship of piroxicam, determine the optimal dose, and evaluate the effect of increased absorption rate on analgesic effect of piroxicam for the pain model studied. The pharmacokinetic data were best described by a two-compartment model with first-order absorption. The absorption rate of PBCD (5/h) was faster than Feldene (1.41/h). Pain relief was found to be increasing with drug concentration in a hypothetical effect compartment (Emax model). The estimated half-life of the equilibration between plasma and effect site was about 2.34 hours. Monte Carlo simulation showed that the time when at least 50% of the patients have a 75% probability of achieving meaningful pain relief (pain intensity difference (PID > or = 1) for PBCD and Feldene at a dose of 20 mg was about 0.5 and 1.5 hours, respectively. PBCD demonstrated an advantage with an onset of pain relief 1 hour earlier than Feldene.

Model-based immunization information routing.

We have developed a model for clinical information routing within an immunization registry. Components in this model include partners, contents and mechanisms. Partners are classified into senders, receivers and intermediates. Contents are classified into core contents and management information. Mechanisms are classified into topological control, temporal control, process control and communication channel control. Immunization reminders, forecasts and recalls in e-mail, fax and regular mail format are routed based on this model. Algorithms for deducing patient provider and patient clinical site relationships are developed to facilitate routing. A dummy provider within each site and a dummy site are created to manipulate routing for incomplete data. 46.63% of patients' primary providers and 92.58% of patients' clinical sites are successfully identified.

Regulation of cyclooxygenase isoforms in the renal thick ascending limb: effects of extracellular calcium.

We previously showed that primary cultures of mTAL cells express cyclooxygenase 2 (COX-2) when challenged with tumor necrosis factor alpha (TNFalpha) or phorbol myristate acetate (PMA). Moreover, expression of COX-2 was linked to decreases in TNFalpha-mediated 86Rb uptake, an in vitro correlate of natriuresis. mTAL cells in primary culture express calcium sensing receptor (CaR), a G-protein coupled receptor that senses changes in extracellular calcium concentration and ultimately increases intracellular calcium concentration ([Ca2+]i) and protein kinase C (PKC) activity. PGE2 synthesis by mTAL cells increases in a dose- and time-dependent manner after exposure of these cells to extracellular Ca2+. Similar effects were observed when cells were challenged with the CaR-selective agonist, poly-L-arginine. These data suggest that intracellular signaling mechanisms initiated via activation of CaR contribute to mTAL PGE2 synthesis. As TNF production is calcium-sensitive in some cells types, we postulate that these effects involve the regulation of COX-2 expression via a TNF-dependent mechanism. The functional implications of these studies relate to a cytokine-mediated mechanism that contributes to salt and water balance, and suggests that small changes in Ca(2+)o may contribute to the regulation of these events. The possibility that the effects of Ca(2+)o involve activation of CaR suggests that novel calcimimetic molecules might be useful in conditions, such as hypertension or other conditions, in which manipulation of extracellular fluid volume provides beneficial effects.

[Effects of tumor necrosis factor-alpha on proliferation and intracellular free calcium concentration in endothelium of human umbilical vein endothelial cells].

OBJECTIVE: To Observe the effects of tumor necrosis factor-alpha (TNF-alpha) on cell proliferation and Intracellular free calcium concentration ([Ca2+]i) in endothelium of human umbilical vein endothelial cells (HUVEC) and investigate the pathogenesis of pregnancy induced hypertension syndrome (PIH). METHODS: Confluent monolayer of HUVEC was directly incubated with TNF-alpha at following final concentrations: 500, 1,000, 2,000 U/ml for 24 hours. The percentages of different cellcycles and [Ca2+]i were measured by flow cytometry and fluorospectrophotmetry. RESULTS: Incubated with TNF-alpha, the endothelial cells were elongated and transformed into fibroblast-like cells. Border of nucleus was sharp, clarity, and cells were in regular shape. But there were abnormal granules in cytoplasma and some cells detached at the concentrotion of 2,000 U/ml of TNF-alpha. Stimulated by TNF-alpha, the percentage of cellcycles from phase G0 + G1 to S and G2 + M decreased significantly and it was dose-dependent [Ca2+]i increased significantly and dose-dependent as well. CONCLUSION: TNF-alpha may injure endothelium directly and inhibit its proliferation and repair through the changes of [Ca2+]i level. It may play an important role in the pathogenesis of PIH.

[Expression of vascular endothelial growth factor receptor kinase inser domain containing receptor in human cervical carcinoma].

OBJECTIVE: To observe the expression of vascular endothelial growth factor receptor (KDR) in human cervical carcinomas, to clarify the relationship between the expression and invasion, metastasis, prognostic value in human cervical carcinoma. METHODS: KDR was stained immunohistochemically in 80 cases of human cervical carcinoma. RESULTS: The expression of KDR in the endothelial cells of tumor vasculature is: the high KDR expression rate of clinical cancer stages < or = I b and II a-III a was 21.4% and 60.6% (P < 0.01). The high KDR expression rate of the high, middle and low pathohistologic grading was 7.7%, 27.8% and 84.1% respectively (P < 0.01). The high KDR expression rate of adenocarcinoma and squamous-cell carcinoma was 0/5 and 57.3% (P < 0.05). The high KDR expression rate of the cases with lymphonodus metastasis and without lymphonodus metastasis was 77.8% and 46.8% respectively (P < 0.05). The survival rate of patients with high KDR expression tumors was significantly lower than that of patients with low KDR expression (P < 0.05). CONCLUSION: KDR is highly related to growth, invasion and metastasis of human cervical carcinoma, it may be one of the predictors for the biological behavior of human cervical carcinoma.

[Correlation between methylation on SmaI locus of the CDKN2/p16 gene CpG island and lung cancer].

OBJECTIVE: To study the relationship between the status of methylation of the CDKN2/p16 gene 5'-CpG island and lung cancer. METHODS: Genomic DNA was digested with methylation-sensitive enzyme SmaI and Southern blotting was applied to analyze the status of methylation of the CDKN2/p16 in 89 cases of lung cancer and 10 cases of normal lung tissue. RESULTS: Of the 89 cases of lung cancer studied, the CDKN2/p16 gene was shown to be methylated in 15 cases (16.9%). It occurred in 12 of 42 p16 protein negative cases (28.6%), and in 3 of 47 p16 protein positive cases. The CDKN2/p16 gene was not shown to be methylated in 10 cases of normal lung tissue. CONCLUSION: Methylation of the CDKN2/p16 gene 5'-CpG island may be an important mechanism for gene inactivation in lung cancer progression.

Three-dimensional virtual reality surgical planning and simulation workbench for orthognathic surgery.

A new integrated computer system, the 3-dimensional (3D) virtual reality surgical planning and simulation workbench for orthognathic surgery (VRSP), is presented. Five major functions are implemented in this system: post-processing and reconstruction of computed tomographic (CT) data, transformation of 3D unique coordinate system geometry, generation of 3D color facial soft tissue models, virtual surgical planning and simulation, and presurgical prediction of soft tissue changes. The basic mensuration functions, such as linear and spatial measurements, are also included. The surgical planning and simulation are based on 3D CT reconstructions, whereas soft tissue prediction is based on an individualized, texture-mapped, color facial soft tissue model. The surgeon "enters" the virtual operatory with virtual reality equipment, "holds" a virtual scalpel, and "operates" on a virtual patient to accomplish actual surgical planning, simulation of the surgical procedure, and prediction of soft tissue changes before surgery. As a final result, a quantitative osteotomy-simulated bone model and predicted color facial model with photorealistic quality can be visualized from any arbitrary viewing point in a personal computer system. This system can be installed in any hospital for daily use.

[Photodynamic inactivation of model virus in human plasma by methylene blue and its influence on plasma proteins].

OBJECTIVE: To determine whether photochemical method can inactivate viruses in human plasma efficiently, and to what extent it affects the protein in human plasma. METHODS: Vascular stomatitis virus (VSV) Indiana strain was used as the indicating virus. It was inoculated on Vero cell monolayer, incubated in 37 degrees C for 24 hours, microtitered by cytopathogenic effect. Small amount experiments were carried out to observe virus inactivation and the influence on plasma proteins. RESULTS: Methylene blue (MB) in combination with visible light can inactivate the model virus in human plasma efficiently. The dye in very low concentration combined with light was sufficient to achieve a reduction of VSV titer of more than 6 log10 TCID50. Compared with the untreated plasma, neither the activities of blood clotting factors nor the immunological properties of the plasma proteins and the blood albumin were reduced significantly. In this experiment, the dose of MB used for photoinactivation of the virus was very low, about a hundred fold lower than that of methemoglobinemia used in clinical treatment. Furthermore, heat of the light and photochemical reaction are two import factors for decreasing of clotting factor's activity. CONCLUSIONS: The results of this preliminary study suggest the potential application of MB-photodynamic inactivation of plasma virus is an useful approach in clinical practice.

[A prospective study of posttransfusion hepatitis B virus infection].

OBJECTIVE: To understand the status of HBV posttransfusion infection after blood HBsAg screening of the blood donors. METHODS: Serum samples of blood recipients and donors were detected for HBV-M and HBV DNA by ELISA technique and polymerase chain reaction respectively. RESULTS: There were 326 sera positive for HBV-M and 57 for HBV DNA in 583 blood donors' sera. 39 sera were positive for HBV-M and 23 positive for HBV-DNA in 136 sera of blood recipients, the positive seroconversion rates were 28.68% and 16.9% respectively. CONCLUSIONS: There is high incidence of HBV infection in blood donors though they have been screened for HBsAg. The posttransfusion HBV infections are still occurring, therefore, the blood donors should be strictly tested for HBV-M. If conditions are possible, the HBV DNA had better been screened.

[Some comments on ecological field].

Based on the data of plant ecological field studies, this paper reviewed the conception of ecological field, field eigenfunctions, graphs of ecological field and its application of ecological field theory in explaining plant interactions. It is suggested that the basic character of ecological field is material, and based on the current research level, it is not sure whether ecological field is a kind of specific field different from general physical field. The author gave some comments on the formula and estimation of parameters of basic field function-ecological potential model on ecological field. Both models have their own characteristics and advantages in specific conditions. The author emphasized that ecological field had even more meaning of ecological methodology, and applying ecological field theory in describing the types and processes of plant interactions had three characteristics: quantitative, synthetic and intuitionistic. Field graphing might provide a new way to ecological studies, especially applying the ecological field theory might give an appropriate quantitative explanation for the dynamic process of plant populations (coexistence and interference competition).

Mannitolum infusion on cirrhotic patients with tense ascites treated by paracentesis.

OBJECTIVE: To investigate whether albumin can be substituted by mannitolum in cirrhotic patients with tense ascites treated by paracentesis. METHODS: Sixty-eight patients admitted to this therapeutic procedure were randomly assigned to receive intravenous albumin (36 patients) and mannitolum (32 patients) infusion. In repeated large-volume paracentesis (3-6 L/day), intravenous albumin 20 g or intravenous 20% mannitolum 250 ml were added. RESULTS: In 24 and 48 hours after paracentesis the mean value of electrolytes, liver and renal functions and various indicators of systemic circulation either in Group 1 or in Group 2 cases were found without changes (P > 0.05). As compared with that before paracentesis, the diameter of spleen vein was increased significantly (P < 0.05). The complications occurring after paracentesis were similar in both groups. CONCLUSIONS: It was suggested that paracentesis with intravenous infusion of mannitolum is an effective and safe method in treating cirrhotic patients with tense ascites.

Induction of intestinal metaplasia in stomach of dogs and expression of tumor-related proteins in animal gastric mucosa lesions.

OBJECTIVE: To observe the development and progression of intestinal metaplasia (IM) in dog's stomach and expression of tumor-related proteins in gastric mucosa lesions. METHODS: IM animal model was induced in stomach of Beagle dog by combining treatment of oral administration of N-methyl-N'-nito-N-nitrosoguanidine (MNNG) and ranitidine (R) with X-ray irradiation to the target organ. Expression of APC, p53, K-ras and bcl-2 gene proteins in animal gastric mucosa lesions were determined with immunohistochemical method. RESULTS: IM animal model was successfully induced and the dynamic pathological changes were observed by means of this model. It was confirmed that the progression from normal epithelial cells to IM cells may require several stages, including superficial gastritis, chronic atrophic gastritis, slight focal IM and moderate or severe IM. Aberrant bcl-2 protein can be detected in the atrophic mucosa epithelium and the abnormal expression of APC, K-ras and bcl-2 can be found in IM of mucosa. CONCLUSIONS: IM model in stomach of Beagle dog can be successfully induced by our method (MNNG + R + X-ray). The progression from normal to IM in dog resembles that of human being and the expression of tumor-related proteins (APC, bcl-2, K-ras) may play a role in the malignant transformation of IM.
