Securing a biomedical communications future: thinking strategically.

Ensuring continued growth and viability of the biomedical communication function has become a critical task of the biomedical communications director. Thinking strategically is a cognitive process which assists a director in visualizing programs and tactics which meet clients needs, creates competitive advantages for the biomedical communications unit and builds on existing unit strengths. Thinking strategically can be divided into five phases: strategic vision, strategy development, strategic plan implementation, strategic plan dissemination, and strategic plan evaluation. Each sequence leads the biomedical communications director through a process designed to increase the effectiveness of the biomedical unit and to meet the challenges posed by an environment characterized by diminished financial, material, and human resources as well as respond to threats and opportunities posed by increased competition in the biomedical communications product and marketplace.

Protein hydrophobicity and stability support the thermodynamic theory of protein degradation.

Rat liver enzymes were used to study the relationship between their in vivo half-lives and their apparent hydrophobicity or their resistance to inactivation by mechanical shaking. The apparent hydrophobicity of these enzymes, measured as the percent of the protein recovered from an octyl-Sepharose column, is correlated with their known half-lives (r = 0.75, P less than 0.01). The presence of specific ligands which are known to increase compactness by impeding unfolding of proteins decreased the apparent hydrophobicity of fructose-1,6-bisphosphatase, glucose-6-phosphate dehydrogenase, glyceraldehyde-3-phosphate dehydrogenase, and pyruvate kinase. Resistance of enzymes to inactivation by mechanical shaking correlated well with their in vivo half-lives (r = 0.90, P less than 0.01). When the shaking experiments were done in the presence of substrates, fructose-1,6-bisphosphatase, glucose-6-phosphate dehydrogenase, glyceraldehyde-3-phosphate dehydrogenase and lactate dehydrogenase were protected from inactivation.

Ineffectiveness of vasopressin in the treatment of memory impairment in chronic schizophrenia.

A vasopressin derivative or placebo was administered to 21 chronic schizophrenia patients for 3 weeks in a randomized crossover double-blind design. The patients were divided into those above and below the median on baseline memory measured by the Wechsler memory scale. Vasopressin treatment did not improve memory either in those patients with below median baseline memory or in the group as a whole.

[Traumatic injuries to the ileosacral joint; late results (author's transl)].

Injury to the ileosacral joint is the result of particularly severe traumatic violence. On the average, it is accompanied by 3 additional injuries to the osseous pelvis. Treatment depends on the nature and seriousness of the trauma to the anterior or posterior pelvic girdle and concomitant injuries. Operative stabilization is necessary if the accompanying injuries are severe. Conservative treatment was aimed at in all other cases. Of the 45 patients followed-up, 40 complained of persistent subjective discomfort. Clinical examination of the ileosacral joints revealed that it was the cause of late complaints in 32 patients, while in the remaining 8 the cause could be traced to other parts of the pelvis. X-ray findings showed no correlation to the complaints. Even in those cases where the interarticular space appears normal in the X-ray, complaints can be expected with the same frequency and intensity as in arthrotic or ankylotic joints. With regard to the late results, the surgically treated patients can only be conditionally compared with those treated conservatively. Nevertheless the late morbidity of these patients appears to be particularly high.

Treatment of severe hypertension with minoxidil: advantages and limitations.

Minoxidil, a new peripheral vasodilator, was used in the therapy of 26 hypertensives who were previously uncontrolled on conventional medications or who had dose-limiting side effects. Minoxidil provided (1) therapeutic advantages in all patients, regardless of the etiology of their hypertension, (2) no symptoms of orthostatic hypotension or sympathetic nervous system depression, (3) a simplification of medical regimens and perhaps costs, and (4) regression of impotence in four out of seven patients. The major limiting factors encountered were (1) fluid retention with the development of congestive heart failure in three patients, (2) pericardial effusion in three patients, and (3) hypertrichosis, which reduced its acceptability in female patients.

Clinical assessment of sympathetic tone: orthostatic blood pressure responses in borderline primary hypertension.

Orthostatic changes of systolic, diastolic and pulse pressure, and pulse rates were evaluated as clinical indicators of sympathetic nerve tone in 28 patients with uncomplicated primary hypertension. After sixty minutes in the upright position, systolic blood pressure increased in 13 and decreased in 15 of the patients, while pulse pressure diminished in 19, was unchanged in 4 and increased in 5 of the patients. The changes of both systolic blood pressure and pulse pressure showed a significant positive correlation with basal (supine) levels of norepinephrine (r = 0.40, p less than 0.05 and r = 0.54, p less than 0.01, respectively). In the total sample of patients, 25% had basal plasma norepinephrine concentrations above the normal range. Of the patients with increased systolic blood pressure after standing, 38% had elevated basal norepinephrine. Similarly in patients with increased orthostatic pulse pressure or in those with pulse pressure reductions less than 10 mm Hg, 45% had elevated basal norepinephrine. On the other hand, of patients with reductions of pulse pressure of more than 10 mm Hg, only 6% had elevated basal norepinephrine. Of the patients with similar systolic blood pressure reductions, none had increased plasma norepinephrine. Both diastolic blood pressures and pulse rates always rose during orthostatic stress, but they did not correlate with basal sympathetic tone. In conclusion, hypertensive patients with orthostatic increases of systolic and pulse pressure are more likely to have elevated basal sympathetic tone than unresponsive patients. The former may comprise a subset of hypertensives who have a causal neurogenic component. They may be more sensitive to drugs which suppress sympathetic nerve function.

Colonic necrosis complicating pancreatitis.

Bowel necrosis is an uncommon complication of pancreatitis. Two patients are reported who developed ileocecal and descending colon changes as a result of ischemia following severe pancreatitis. Arteriography in one patient confirmed the presence of intraluminal thrombi.

Valvular heart disease in osteogenesis imperfecta.

Aortic and mitral valve abnormalities have been reported which clearly appear to be related to the underlying connective tissue disorder in two patients, a father and daughter, with osteogenesis imperfecta. Although this appears to occur with a much lower prevalence and lesser severity than in the Marfan syndrome, the true prevalence of cardiac connective tissue involvement is not known, and the orthopedic complications of osteogenesis imperfecta may have overshadowed attention to cardiovascular abnormalities. In evaluating patients with osteogenesis imperfecta, careful attention should be paid to cardiovascular findings and if valvular lesions are noted, patients should be instructed regarding the need for antibiotic prophylaxis for dental and surgical procedures. The valvular lesions can progress, and regular follow-up cardiovascular evaluation should be planned. Finally, despite potential problems with tissue friability and healing and a possible tendency for increased bleeding, successful valve replacement can be carried out if necessitated by cardiac disability.

Carcinoma of the thoraco-abdominal oesophagus.

A new approach in the management of carcinoma of the thoraco-abdominal oesophagus, by means of pre-operative chemotherapy, in the form of Methotrexate and irradiation, followed by a one-stage resection, using a thoracic/abdominal approach, is described. This resulted in a marked tumour regression without leading to difficulty with operative management.
